The spill happened in conjunction with a fire that occurred in the boiler room located near the western side of the main building. A timeline of events can be found in Appendix A of this report. Due to heat from the fire, multiple sprinkler heads were fused open, mixing fire protection water with heat transfer oil that had escaped from the hot oil system. Prior to the fire being completely extinguished, the mixture of heat transfer oil and fire protection water exceeded the capacity of the boiler room containment structure, and began to overflow onto the surrounding impervious surface and into several nearby stormwater drains. A copy of the property stormwater flow diagram is located in Appendix B of this report. All of the oily water mixture was contained on Seiren property, and no contamination reached the receiving stream adjacent to the property. Further containment measures were implemented immediately, and remediation is in process. Further details regarding the ongoing remediation are outlined in Appendix C of this report. Figure 1: Underflow dam constructed in drainage ditch on NE corner of property 0 • In order to ensure further protection in the event of a significant rainfall, the height of the earthen dam at the outfall of the holding pond was increased by an extra foot (Fig. 2). STAT, Inc. constructed a road down the embankment to the outfall of the holding pond to accomplish this. Figure 1: Height of earthen dam increosed at outfoll of holding pond JUN 5 )0?9 �J The receiving stream, Hunting Creek, was frequently monitored to ensure no contamination had migrated off of the 5eiren property (Fig. 3). Figure 3: Receiving stream, Hunting Creek, showing no oil contamination after spill f, !C-d J U N 5 2019 CI 9 Construction of the underflow dam at the outfall of the holding pond was completed on May 30, 2019 (Fig. 4). Additional oil containment booms were placed downstream of the outfall. Figure 4: Underflow dam constructed at pond outfall 0 0 Prior to final washing of impervious surfaces surrounding Boiler Room and flushing underground piping, a second underflow dam was constructed in the drainage ditch at the NW corner of the property as an additional precaution (Fig. 5). Figure 5: Second underflow dam constructed in drainage ditch on NE corner of property All concrete surfaces which had been in contact with oil contamination have been cleaned utilizing oil soak pads and pressure washers. Following this, drains have been flushed to remove residual oil. All grass, dirt, and gravel surfaces which had been in contact with oil contamination have been scraped clean by utilizing oil soak pads, skid steers, excavators, and shovels. Where oil -stained gravel was removed, new gravel will be installed. To remove the oil from the holding pond on Seiren property, the pond will be drained, and the floor of the pond will be scraped clean. At that time, a 5TAT, Inc. geologist will test the remaining soil to ensure all oil residue has been removed. It is estimated that this entire process will take 1-2 weeks. J U N 5 ?019 Reed, Isaiah L From: Reed, Isaiah L Sent: Thursday, May 30, 2019 9:44 AM To: Aiken, Stan E Cc: Wiggs, Linda; Georgoulias, Bethany; Morman, Alaina Subject: Emergency Inspection at Seiren America LLC Sta n, After being informed by Linda Wiggs (DWR) that a spill had occurred at the Seiren facility (NCG170400, 1500 E Union Street, Morganton). A site inspection was performed. Linda Wiggs and myself arrived on site at 4:15 pm on May 29, 2019. We initially met with Cecil Huffman (Burke County), Mike Willis (Burke County) and Gary Sparks (STAT). We then met with Kermit Hollman (Seiren) and Kevin Heape (Seiren). When we arrived on site, STAT was pumping out the large basin on the South side of the facility. Oil was visible in the pond. I walked down to where the stormwater pipe entered the pond and found it to be 50% submerged. We went to where the fire had happened and found that there was a large amount of oil still visible around the spill site, in the grass, and being carried to the stormdrains by a brief rainfall that occurred while on site. We observed oil residue in the drainage channel leading North from the facility as well, to the second outfall. It was communicated to Mr. Heape that measures are needed to prevent further migration of that material off site. The description given of the spill by Mr. Heape is the following: 1) At 10:30 am a large oil tank used for heat transfer (Chevron 22) began leaking from a connection inside the facility. 2) Personnel in the facility went to turn the power off to the tank, and for a reason not quite understood at this point, shutting off the power started a fire. 3) 6-8 sprinkler heads then opened for a duration of —30 minutes filling the area with water and oil, resulting in the spill. 4) The oil/water mix made its way to at least 3 storm drains and a drainage ditch. 5) The tank that drained out has a total capacity of 500 gallons. 6) The stormwater basin never discharged (confirmed during the inspection). 7) STAT will be removing the spilled oil from the pond, and contaminated soils will be removed from the spill area. After getting a description of the events, Mr. Heape and myself walked down to the outlet end of the basin and confirmed that no water or oil residue was visible in the spillway. I confirmed with Mr. Heape that he was aware of all reporting requirements in the permit, and he will be submitting a report to the department as required by the permit. I will be following up with Mr. Heape in the coming days to confirm the situation on site. A follow up inspection will be performed next week if possible.
