Aana Taylor -Smith of the Winston-Salem Regional Office of the NC Division of Energy, Mineral, and Land
Resources (DEMLR) conducted a compliance evaluation inspection at the subject facility on 10 January
2014. Your assistance and cooperation, as well as that of Mr. Mack Johnson, were greatly appreciated.
An inspection checklist is attached for your records and the inspection findings are summarized below.
This facility is located at 7209 Highway 158 in Stokesdale, Guilford County, North Carolina. Stormwater
related to industrial activity is discharged from the site to an unnamed tributary to the Haw River in the
Cape Fear River Basin.
Documentation & Monitoring Review
The facility's Stormwater Pollution Prevention Plan (SPPP) is extremely thorough, up to date, and fully
implemented. Records of inspections, training, and monitoring are very well kept. The plan was last
updated on 6 December 2013. Qualitative monitoring has been completed as required. Analytical
monitoring is not required because this facility does not perform on -site vehicle maintenance. The
facility has four Stormwater Discharge Outfalls (SDOs) that discharge stormwater related to industrial
activity.
Site Review
The facility is clean and well maintained. Secondary containment is implemented as required and
records are very well kept. Outfalls have been inspected and were found to be in good order. No
evidence of erosion or instability at the SDOs was observed.

The Winston-Salem Regional Office of the Division of Water Quality performed a storm
;eater inspection of Culp Ticking on August 18, 2009. This inspection falls under the
provisions of North Carolina General Statute 143-215.1. Marc Stokes of this office
performed the aforementioned inspection.
Permit
Culp received a Certificate of Coverage (COC), NCG 170377, under the NPDES General
Permit NCG 170000 in August 1999, renewed in 2004 and 2009 and will expire in July
2014. The permit is associated with Standard Industrial Classification SIC 22.
Facility
Culp is a manufacturer of mattress fabrics. The facility does weaving production, printing
atul finishing operations.
The facility is approximately 43 acres in size of which approximately seven is impervious
surface. The plant consists of one major building housing the production operations, a
building housing the wastewater operations and a small structure containing used oil in
drums. The facility employs 187 personnel and is a 24/7 operation.
All floor drains in the facility discharge to the wastewater operation. No vehicular
maintenance is performed on site. Chemicals used in production are contained within the
facility. The facility has one open top dumpster and one compactor.

In accordance with your application for discharge permit received on February 3, 1998 we are forwarding
herewith the subject certificate of coverage to discharge under the subject state - NPDES general permit. This
permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the Memorandum
of Agreement between North Carolina and the US Environmental Protection agency dated December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable
to you, you have the right to request an individual permit by submitting an individual permit application. Unless
such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division of
Water Quality. The Division of Water Quality may require modification or revocation and reissuance of the
certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Water Quality or permits required by the Division of Land Resources, Coastal Area Management Act
or any other Federal or Local governmental permit that may be required.
