Aana Taylor -Smith of the Winston-Salem Regional Office of the NC Division of Water Quality (DWQ or the
Division) conducted a compliance evaluation inspection at the subject facility on 11 April 2013. The assistance
and cooperation provided by Mr. Jody Jarrell and Mr. John Booth during the inspection were greatly
appreciated. An inspection checklist is attached for your records and the inspection findings are summarized
below.
This facility is located at 170 Shakey Road in Mayodan, Rockingham County, North Carolina. Stormwater related
to industrial activity is discharged from the site to an unnamed tributary of the Dan River, which is currently
classified as WS-V (Water Supply) waters, in the Roanoke River Basin.
Documentation & Monitoring Review
The Stormwater Pollution Prevention Plan (SPPP) is complete and current. It is updated on a yearly basis and all
required records were available for inspection. Problems with the SPPP were described in the previous
inspection, conducted 24 November 2008, with the resulting recommendation that the plan be updated to
include several missing sections. During the inspection, it was noted that all recommendations had been
addressed. The facility had one spill in the past three years that was well -documented, but records of the spill
were not included in the SPPP. Please be sure to update the SPPP with a list of significant spills as required by
Part II, Section A, Paragraph d of the permit. Additionally, although the SPPP has been reviewed and updated on
a yearly basis, please rememberto include a statement that the plan has been updated with the plan itself.
Qualitative monitoring has been conducted and documented as required. The facility has 14 Stormwater
Discharge Outfalls (SDOs) at which qualitative monitoring is performed on a semi-annual basis. The facility has
one "switcher" vehicle that receives yearly preventative maintenance on -site, but as less than 55 gallons of new
motor oil are used per month, the facility is exempt from analytical monitoring requirements.

Site Review
The facility is clean and well -maintained. All necessary secondary containment is implemented as required. Each
of the 14 outfalls was examined during the inspection. With one exception, the SDOs were clean and well -
maintained. Please note that Outfall 2 should have the grass removed from the mouth of the pipe. As discussed,
keep an eye on Outfall 14, as it may be prone to erosion. Additionally, you may want to consider updating the
description of Outfall 13 in the SPPP, as it did not appear as described, but was found at the location given in the
SPPP. The receiving waters at each outfall looked clear with no visible foam or floating solids. Outfall and access
maintenance is performed regularly and well -documented.
We appreciate your efforts to comply with the stormwater general permit, as well as the improvements made to
the facility's SPPP and practices since the last inspection. No additional response to this letter is required. Please

In response to your renewal application for continued coverage under stormwater General Permit NCG170000 the Division of
Water Quality (DWQ) is forwarding herewith the reissued stormwater General Permit. This permit is reissued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North
Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as subsequently amended.)
The following information is included with your permit package:
• A new Certificate of Coverage
• A copy of stormwater General Permit NCG170000
• A copy of a Technical Bulletin for the General Permit
• Five copies of the Discharge Monitoring Report (DMR) Form
• Five copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater only, and it specifies your obligations with respect to stormwater
discharge controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself with all the
changes in the reissued permit.
The more significant changes in the General Permit since the your -last Certificate of Coverage include the following:
• Permit cover page — Coverage is expanded to include industrial activities similar to the categorically identified industries.
• Part I Section A — This part explains that the No Exposure Exclusion from permitting option may be available to the permittee.
• Part II Section A — Several minor expansions and clarifications of the required content of the Stormwater Pollution Prevention
Plan have been added. Please be sure that your plan is updated to comply with the new requirements.
• Part II Section B — Qualitative monitoring must now be conducted during a representative storm event.
• Part II Section B — The permittee is required to keep a written record of his response to problems identified by his qualitative
monitoring observations.
• Part II Section C - For facilities with vehicle maintenance activities using more than 55 gallons of new motor oil per month, new
provisions require that discharges from the vehicle maintenance areas be sampled twice per year, and that the sampling
results be compared to new numerical benchmark values. (The previous cutoff concentrations have been eliminated.)
• Part 11 Section C — For facilities with vehicle maintenance activities using more than 55 gallons of new motor oil per month,
new provisions require the permittee to execute Tier One and Tier Two response actions, based on the first benchmark
exceedence (Tier One) and the second consecutive benchmark exceedence (Tier Two). Tier Two requires that the permittee
institute monthly monitoring instead of twice -per -year monitoring, until three consecutive monitoring events show no
benchmark exceedences.
• Part 11 Section C —This part contains a new provision that four exceedences of any particular benchmark will trigger increased
DWQ involvement in the permittee's stormwater management and control actions. DWQ may direct the permittee to apply for
an individual permit, or may direct the implementation or installation of specific stormwater control measures.

Part III Section E — Items 1. and 2. require submittal of a new reporting form, the Annual Summary DMR. This annual
summary of only the analytical monitoring results (if any) should be submitted to the local DWQ Regional Office. The Annual
Summary DMR reporting form will be posted on the Stormwater Permitting Unit's website in the near future.
Your coverage under the General Permit is transferable only through the specific action of DWQ_
This permit does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it relieve the
permittee from responsibility for compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order,
judgment, or decree.

Based on an inspection of your facility, Winston-Salem Regional Office staff found that materials on your site either are
exposed, or have the potential to be exposed. DWQ cannot grant your requested exemption from permit coverage

as provided an implementation date as required by the November 24t' memo. I would also
like to take this opportunity to commend Mr. Boone on his knowledge and willingness to help
identify and resolve the issues of concern during the inspection.

discrepancies were noted during the inspection:
a. The general location map does not reflect the locations of the stormwater

1. Ron Boone of the Winston-Salem Regional Office of the NC Division of Water Quality (DWQ) conducted
Compliance Evaluation Inspections (CEI) and evaluations for No Exposure Certification (NEC) at the
subject facilities on 18 November 2008. Your assistance and cooperation were greatly appreciated.
Inspection checklists are -attached for your records and inspection findings are summarized below.
2. Certificate of coverage (COC) NCG170357 covers the Frontier facility located at 170 Shakey Road, in
Mayodan, Rockingham County, NC. This facility discharges stormwater to receiving waters designated as
an unnamed tributary (UT) of the Dan River, this section of which is currently classified as Class WS-IV
(water supply) waters in the Roanoke river basin. COC NCG170136 covers the Frontier facility located at
109 Turner Road, in Mayodan, Rockingham County, NC. This facility discharges stormwater to receiving
waters designated as Big Beaver Island Creek, currently classified as Class C waters, and the Mayo River,
this section of which is also currently classified as Class C waters. Both of these receiving streams are
located in the Roanoke river basin.
Documentation & Monitoring Review - NCG170357 (170 Shakey Road)
3. Frontier Spinning Mills, Inc. is being issued a Notice of Violation (NOV-2008-PC-0807) for failure to meet
multiple permit conditions as noted during this inspection and specified below.
4. The facility has a Stormwater Pollution Prevention Plan (SP3). The following SP3 discrepancies were
noted during the inspection:
a. The general location map does not reflect the locations of the stormwater discharge outfalls (SDO) with
latitudes/longitudes, as required in Part If, Section A, Paragraph 1a of the permit.
b. The site map does not show the separate drainage areas with their corresponding SDOs, directions of

inspections are currently being conducted in accordance with permit requirements.
Site Review - NCG170357 (170 Shakey Road)
5. The site was fairly clean and well maintained. The only issues noted were the small spill and the SDOs, as
mentioned above.
No Exposure Certification - NCG170357 (170 Shakey Road)
6. Frontier Spinning Mills, Inc. recently submitted to the Division a No Exposure Certification (NEC) for this
facility. The NEC is hereby denied for the following reasons:
a. The facility is non -compliant with the current NCG170000 stormwater permit. The facility will have to
attain complete permit compliance and correct all discrepancies listed above in paragraphs 3a — 3g,
prior to an NEC being granted.
b. There is a material storage area located near the southeast corner of the facility where unused parts
are being stored outside, exposed to the -elements. These materials must be removed, permanently
moved inside the facility, or placed under permanent cover to qualify for the NEC.
The waste oil tank and its secondary containment (SC) structure are located outside, exposed to
precipitation. In order to qualify for an NEC, this tank and its SC structure will have to be placed under
permanent cover or removed.

d. There are two large recyclables dumpsters on site that are currently open top. These dumpsters will
have to be permanently covered or removed to qualify for the NEC.
e. There are two large trash compactors on site. The compaction compartments of these units are
sealed, however the ends of the units, where trash is loaded into the compactors, is open and exposed
to precipitation. These units will have to be permanently covered or removed in order to qualify for the
N EC.

10. You are reminded that the Director of the Division of Water Quality may assess civil penalties not to exceed
$25,000 per day for violations of the NCG170000 general stormwater permit. Please reply to this
letter/Notice of Violation within 30 days of receiving it. Your response should include corrective actions
with implementation schedules for each of the discrepancies noted above in paragraphs 4a — 4g for
NCG170357 and 7a 7g for NCG170136. As stated above, before either facility will be granted NEC
status, complete permit compliance will have to be attained and demonstrated and all conditions listed
above in paragraphs 6a — 6e and 9a — 9d, will have to be met.

This modification documents the change in ownership at the subject facility. All other terms and conditions in the original
permit remain unchanged and in full effect. This permit modification is issued under the requirements of North Carolina
General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U. S. Environmental
Protection Agency dated December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit modification are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following
receipt of this letter. This request must be a written petition conforming to Chapter 150B of the North Carolina General
Statutes, filed with the Office of Administrative Hearings, Post Office Drawer 27447, Raleigh, North Carolina 27611-7447.
Unless such demand is made, this decision shall be final and binding.

Permitting form, which we received on October 7, 2008. The Division is unable to concur with your certification of no

Based on an inspection of your facility, Winston-Salem Regional Office staff found that materials on your site either are
exposed, or have the potential to be exposed. DWQ cannot grant your requested exemption from permit coverage
when materials are exposed or have the potential to be exposed. Please continue to operate under the General Permit
