Aana Taylor -Smith of the Winston-Salem Regional Office of the NC Division of Water Quality (DWQ) conducted a
compliance evaluation inspection at the subject facility on 18 April 2013. Your assistance and cooperation during
the inspection were greatly appreciated. An inspection checklist is attached for your records and the inspection
findings are summarized below.
This facility is located at 712 Henry Street in Eden, Rockingham County, North Carolina. Stormwater related to
industrial activity is discharged from the site to the Dan River, which is currently classified as WS-IV (Water
Supply) CA (Critical Area) waters, in the Roanoke River Basin.
Documentation & Monitoring Review
The Stormwater Pollution Prevention Plan (SPPP) is complete, well -established, and current. it is updated on a
yearly basis by a corporate environmental officer and all required records were available for inspection.
Problems with the SPPP were described in the previous inspection, conducted 9 December 2009, with the
resulting recommendation that the plan be updated to include stormwater discharge outfall (SDO)
recertification and DWQ emergency numbers. Additionally, it was noted in 2009 that employee training focused
only on spills and it was recommended that training be updated to include stormwater pollution prevention
practices. During the inspection, Ms. Taylor -Smith found that all recommendations had been addressed.
Documentation for employee training was available and a copy of the training program itself was included.
Please continue to conduct and record employee training as required. The facility has had no significant spills in
the past three years.
Qualitative monitoring has been conducted and documented as required. The facility has nine SDOs at which
qualitative monitoring is performed on a semi-annual basis. Facility inspections are also completed and recorded

as required. The facility is exempt from analytical monitoring requirements, as no vehicle maintenance is
conducted on -site.
Site Review
The facility was found to be very clean and well -maintained. All necessary secondary containment is
implemented as required. Each of the nine outfalls was examined during the inspection. The SDOs were clean
and well -maintained. The receiving waters at each outfall looked clear with no visible foam or floating solids.
During the inspection, it was noted that exposure of industrial activities and/or materials to precipitation is
minimal_ Mohawk Industries, Inc. may wish to consider applying for a No Exposure Certification for the facility,
which would preclude the need for the stormwater permit, SPPP, and any qualitative and/or analytical
monitoring, for as long as the No Exposure condition is maintained at the site. For more information on this

1. Ron Boone of the Winston-Salem Regional Office (WSRO) of the NC Division of Water Quality
(DWQ or the Division) conducted a compliance evaluation inspection (CEI) at the subject facility
on 9 December 2009. Your assistance and cooperation during the inspection was greatly
appreciated. An inspection checklist is attached for your records and the inspection findings are
summarized below.
2. This facility is located at 712 Henry Street, in Eden, Rockingham County, North Carolina.
Stormwater related to industrial activity is discharged from the site.to the Dan River, this section of
which is currently classified as Class WS-IV (water supply), critical area (CA) waters, in the
Roanoke River Basin.
Documentation & Monitoring Review
3. The Stormwater Pollution Prevention Plan (SP3) is quite old and well established/implemented.
Inspections and monitoring is being accomplished as required. The SP3 meets most of the
minimum expectations to be compliant with the permit, however the following slight changes need
to be made -
a. Stormwater discharge outfalls (SDO) have not been recertified annually for the absence of
non-stormwater flows (wastewater). Please institute this requirement immediately.
b. The spill plan should be updated to reflect the Division emergency numbers in the case of a
reportable spill.
c. The SP3's training program currently concentrates on spills. This should definitely be a part of
the SP3 training, however, it should concentrate on stormwater pollution prevention and how
spills relate to that. Please update the SP3's training program to do so.

4. Mohawk has been performing the required qualitative monitoring (QM) at all outfalls as required.
No problems have been detected or identified through the monitoring. Facility inspections are
also completed and recorded more than required. Analytical monitoring (AM) is not required.
Site Review
5. The site appeared quite clean and well maintained. Exposure of industrial activities and/or
materials to precipitation is minimal. Mohawk may wish to consider achieving a No Exposure
status at the facility, which would preclude the need for the stormwater permit, the SP3, and any
qualitative and/or analytical monitoring, for as long as the No Exposure condition is maintained at

6. You are reminded that pursuant to NC General Statute 143-215.6A, the Division may assess civil
