On July 24, 2013, Jenny Graznak of this office performed a Compliance Evaluation Inspection
at Kayser -Roth Corporation located at 515 W. Balfour Avenue in Asheboro, North Carolina. This
facility holds General Stormwater Permit No. NCG170234 under the National Pollutant Discharge
Elimination System (NPDES) to discharge stormwater associated with textiles manufacturing. Phillip
Williams, Head of Maintenance, was present for the inspection. This type of inspection consists of
two basic parts: a review of facility files and self -monitoring data and an inspection of the facility's
storage areas and discharge points. This particular inspection evaluated five (5) areas, and
observations from each area are addressed below:
1. Permit
The permit became effective August 1, 2009 and expires July 31, 2014- A complete and current
copy of the Permit was available during the inspection.
2. Records/Reports
As required by the Permit, the facility has a Stormwater Pollution Prevention Plan (SPPP) that was
available for review during the inspection. All required components of the SPPP were present, but it
has not been updated since 1999. The permit requires that "all aspects of the SPPP shall be
reviewed and updated on an annual basis." Please see Part II, Section A, #7 for further information
regarding annual updates. HAZMAT training occurs on an annual basis at this facility, and employee
training on stormwater is included during that event. Please make sure that the employee training is
documented in the SPPP.
3. Facility Site Review
This Kayser -Roth facility manufactures socks. The 28-acre site appeared clean and well managed
with excellent housekeeping. Bleach and hydrogen peroxide tanks are stored outside under a cover
and with secondary containment that drains to sanitary sewer via a sump pump when needed. A
25,000-gallon #2 fuel oil tank is stored outside and surrounded by an earthen containment dike that
can be pumped out when necessary. Some empty totes and pallets are also stored outside, awaiting
pick-up from different vendors. There are also truck loading and unloading areas on the site.

4. Effluent/Receiving Waters
The site has four outfalls. Stormwater from the property discharges to Hasketts Creek (Class C) in
the Cape Fear River Basin. No issues were noted with, respect to the outfalls during the inspection.
5. Self -Monitoring Program
The stormwater permit requires semi-annual qualitative monitoring at stormwater outfalls. This
facility does perform visual observations at� the plant's four outfalls on a semi-annual basis. An on -
site review of the monitoring data during the inspection showed that all parameters are evaluated per
the specifications of the permit. The last monitoring event occurred on April 18, 2013. On -site
vehicle maintenance monitoring requirements at the site are waived, since these activities are not
performed at the site.

Inspection Summary:

Dear Permittee:
Your facility is currently covered for stormwater discharge under General Permit NCG 170000. This permit will
expire on July 31, 2004. North Carolina General Statute 143.215.1(c) requires that an application for permit
coverage renewal be filed in a timely manner. A prior notice of renewal was sent for this site on December 1,
2003. To date, no response has been received by the Division of Water Quality (DWQ). In order to assure your
continued coverage under the general permit, you must apply to DWQ for renewal of your permit coverage.
Enclosed you will find a general permit renewal application form. The application must be completed and returned
by April 19, 2004 in order to assure continued coverage under the general permit.
Failure to request renewal by April 9, 2004 can result in a civil assessment of at least $250.00. Larger penalties
may be assessed depending on the delinquency of the request. Discharge of stormwater from your facility without
coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and could
result in assessments of civil penalties of up to S 10,000 per day.

In compliance with the provision of North Carolina General Statute 143-215.1, other lawful standards and
regulations promulgated and adopted by the North Carolina Environmental Management Commission, and the
Federal Water Pollution Control Act, as amended,

is hereby authorized to discharge stormwater from a facility located at
Asheboro Plant
515 W. Balfour Ave.
Asheboro
Randolph County
to receiving waters designated as Hasketts Creek in the Cape Fear River Basin
in accordance with the effluent limitations, monitoring requirements, and other conditions set forth in Parts 1, II, III
and IV of General Permit No. NCG170000 as attached.
This Certificate of Coverage shall become effective July 14, 1995.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.
