Mike Thomas of the Winston-Salem Regional Office of the NCDivision of Water Quality
(DWQ) conducted a compliance evaluation inspection (CEI) at the subject facility on 2
December 2010. Your assistance and cooperation during the inspection was greatly
appreciated. An inspection checklist is attached for your records and the inspection findings
are summarized below.
The facility is located at 2920 Vance Street, in Reidsville, Rockingham County, North
Carolina. Stormwater related to industrial activity is discharged to an unnamed tributary to
Little Troublesome Creek, which is currently classified as Class WS-V waters in the Cape Pear
River Basin.
Documentation and Monitoring Review
The Stormwater Pollution Prevention Plan (SP3) is complete and well implemented.
Training sessions are conducted annually to keep staff informed and updated. You also
indicated To Mr. Thomas that you often work with employees one-to-one to ensure that the
SP3 is being followed.
Unifi Incorporated — Reidsville has two stormwater discharge outfalls (SDO) and both
were well marked and easily accessible for inspection. Records of Qualitative Analysis were
available and appeared to be complete. Routine maintenance and daily site inspections were
also recorded and made available for inspection. Analytical Monitoring is not required under
NCG170000.

The Unifi Incorporated - Reidsville facility was very clean and well maintained at the
time of inspection. Outfalls were easily accessible and well maintained. No problems or
concerns were observed during the inspection.
We greatly appreciate your efforts to develop and implement an effective stormwater

Your facility is currently covered for stormwater discharge under General Permit NCG 170000. This permit will
expire on July 31, 2004. North Carolina General Statute 143.215.1(c) requires that an application for permit
coverage renewal be filed in a timely manner. A prior notice of renewal was sent for this site on December 1,
2003. To date, no response has been received by the Division of Water Quality (DWQ). In order to assure your
continued coverage under the general permit, you must apply to DWQ for renewal of your permit coverage.
Enclosed you will find a general permit renewal application form. The application must be completed and returned
by April 19, 2004 in order to assure continued coverage under the general permit.
Failure to request renewal by April 9, 2004 can result in a civil assessment of at least $250.00. Larger penalties
may be assessed depending on the delinquency of the request. Discharge of stormwater from your facility without
coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and could
result in assessments of civil penalties of up to $10,000 per day.
