DEMLR releases Unifi Inc. from monitoring requirements at Outfalls 1-A and 1-13 because those two SDOs discharge stormwater related to industrial activity from a neighboring facility, Lydall Thermal/Acoustical. Please update the site map and SPPP to reflect this change. Thank you for your attention to this matter. Remedial actions must be taken to correct the aforementioned deficiencies. Stormwater related to industrial activity is discharged from the site to North Deep Creek, which is currently classified as Class C waters, in the Yadkin -Pee Dee River Basin. Documentation & Monitorin Review. The facility's Stormwater Pollution Prevention Plan (SPPP) is complete and has been implemented, but is out of date. Please complete the following items in order to bring the SPPP into compliance: • Review and update all responsible parties and their contact information. e Add the Winston-Salem Regional Office's phone number to the contact list in case of spills. o Include an updated list of significant spills occurring on site over the past three years. e Keep employee stormwater training records and qualitative and analytical monitoring data with the SPPP. e Evaluate all stormwater discharge outfalls (SDOs) for non-stormwater discharge once per year and include a signed statement that the evaluation has been completed. e Update site map to reflect any changes.
