We will get the MSDS today. There was a clean-up crew yesterday. There was a fish kill, of 100 to 200 counted by the Health Dept. Ron and I decided to sample yesterday around 3:30 pm for Metals (dissolved, total) and Hardness. Upstream and Downstream. The samples will be COC and put in the courier today. At 3:30 pm yesterday, we sampled 300 feet below the discharge and the stream was clear. At the discharge point there was a slight reddish dye color hugging the bank for about 100 feet. Glen, Could you evaluate this site for compliance with their stormwater permit? At this point in time, the spill was stopped and it has dissipated and diluted downstream. We will continue to visually monitor. We will obtain the MSDS. Stormwater related to industrial activity is discharged to Peters Creek, which is currently classified as Class C waters in the Yadkin Pee Dee River basin. Permit Hanes Dye & Finishing holds Certificate of Coverage # NCG170009 for the discharge of stormwater from the facility. The General Permit became effective August 1, 2009 and expires July 31, 2014. 2. Records/Reports Part II, Section A of the permit requires the development of a Storrriwater Pollution Prevention Plan. A copy of the SPPP for Hanes Dye & Finishing was available for review during the inspection. The plan appeared thorough and complete. 3. Facility Site Review The site appeared clean and well managed. No concerns or problems were noted. Monitoring Program Part II, Section B of the permit requires semi-annual qualitative (visual) monitoring to be performed and documented once in the spring (Ap(l- June) and once in the fall (September - November). A records review during the inspection showed that Hanes Dye & Finishing had performed and documented the required qualitative monitoring in 20010 and 2011. Please note that you are not required to submit these forms to the Division, however, the records must be kept on file at the facility for a period of five years. The analytical monitoring listed in Section C of the permit for on -site vehicle maintenance operations is not required since Hanes Dye & Finishing does not use more than 55 gallons of new motor oil per month when averaged over the calendar year. For several years, OutfA 2 (0-2) and Outfall 3 (0-3) have been inspected at the last known locations on the facility grounds as described in my letter of June 21 submitted to the NC DENR - Waughtown office preNriously. Due to safety reasons and limited access to outfaUs 2 and 3, stormwater discharges have been observed at the junction boxes located on the facility's property prior to the discharge outfalls located along Peter's Creek. The issue of inspection locations, other than at the outfalls, was discussed with NC DENR several years ago via the telephone. Other than recording the location of the observation during the inspection, no other recommendations were received regarding permit or site map changes to include the alternative observation locations. Therefore, information regarding the outfall observation locations was recorded in the completed forms. However, since these observation points were not shown in the facility site map, reviewed during the stormwater inspection completed by NC DENR on May 26, revisions in the facility map were recommended by NC DENR. In response to the Notice of Violation (NO'\0 of June 1 received after the May 26 inspection by NC DENR, the facility site map was revised and a stormwater feasibility study was completed by Delta Environmental Consultants. As requested, this information was submitted to NC DENTR - Waughtown location on July 9, 2004. After a spill of water and residual starch on the ground, the site map was revised further on September 10 to include an additional stormwater drainage area via a sump tank located outside the dry storage warehouse (see Figure 1). r Based on the findings of the feasibility study, a stormwater berm was installed in August 2004 to prevent or reduce stormwater runoff from the coal and coal ash storage areas into the stormwater system Please refer to Figure 1. As provided in previous correspondence with the Waughtown division of NC DENR, the installation of the bulk tank liquid level cutoff devices is continuing and should be completed by the target date of October 20, 2004.
