Your assistance throughout the process was greatly appreciated. An inspection checklist is attached for your records and the inspection findings are summarized below. 1. PPrmit This facility holds General Storm water Permit No. NCG160166 to discharge Storm water from activities associated with Asphalt Paving Mixtures and Blocks under the National Pollutant Discharge Elimination System (NPDES). The permit became effective October 1, 2014 and expires September 30, 2019. Your permit renewal package must be received in the Central Office in Raleigh no later than 180 days prior to the expiration date of the permit. 2. Records/Reports Part II, Section.A of the permit requires the development of a Stormwater Pollution Prevention Plan (SPPP). The plan must include all nine (9) subsections of Part 11, Section A of the permit. Qualitative monitoring is required to be completed and documented twice yearly as defined in Part 11, Section C of the permit. The SPPP plan must be updated annually and qualitative monitoring must be conducted and properly documented semi-annually to retain compliance with the permit. The site has not developed an SP3 (Stormwater Pollution Prevention Plan) and has not been conducting its required qualitative or analytical monitoring. Facility Site Review This site manufactures asphalt paving mixtures and operates on a full-time basis. The site was operating and itself appeared to be well maintained at the time of inspection. 4. Effluent/Receiving Waters All stormwater from this site drains into the Ararat River, Class C waters of the Yadkin River Basin. 5. Self -Monitoring Program Analytical Monitoring: This site is required to provide analytical monitoring. No monitoring has been performed. A sample needs to be taken for testing as soon as the next rainfall event. Test pH at the time of collection. 6. Qualitative Monitoring: All qualitative records do not need to be submitted to DEMLR, however, the records must be kept on file at the facility for a period of five years. Bi-annual monitoring has not been done per the permit. Please provide monitoring at the next rainfall to remain compliant. The following deficiencies must be addressed to regain permit compliance: 1. Develop an SP3 (stormwater Pollution Prevention Plan) that includes operating and maintenance procedures, spill prevention and response procedures, training and safety procedures. 2. Begin qualitative monitoring at next rainfall event. Monitor twice annually. Keep records with SP3 for minimum of 5 years. 3. Begin analytical monitoring at next rainfall event. Test pH at time of collection. Send samples to be tested at a qualified lab. Provide monitoring twice annually. Keep result with SP3 for min. 5 years. This facility will be re -inspected in mid-2016 to evaluate for compliance. This will allow the opportunity to correct deficiencies before further action is taken which may include fines and fees. Director Division of Water Quality On May 16, 2006, Mike Mickey of this office met with Tom Sproule to perform a General Stormwater Permit Inspection at the Adams Asphalt facility located on Quarry Road in Surry County. This inspection evaluated five (5) areas as designated on the attached EPA inspection form. Observations from each area are addressed below: 1. Permit Adams Construction holds Certificate of Coverage # NCG160166 issued for the discharge of stormwater from the asphalt paving operation. This facility began operation in June 2005, The General Permit expires July 31, 2009. 2..Records/Reports Part 11, Section A of the permit requires the development of a facility specific Stormwater Pollution Prevention Plan (SPPP). A copy of the SPPP was available for review during the inspection. The plan appeared thorough and complete. 3. Facility_ Site Review The site is bermed to direct stormflow to a single outlet located near the diesel storage tank. The analytical and visual monitoring should occur at this outlet. The entire site appeared clean and well managed. No concerns or problems were noted. 4. Effluent/Receiving Waters Stormwater from the site flows overland before reaching a tributary to the Ararat River, Class C waters in the Yadkin Pee Dee River Basin. 5. Monitoring Program Part II, Section B of the permit requires annual collection of a stormwater sample during a representative storm event (>0.1"). These results must be submitted to the Division no later than 30 days from the date you receive the results back from a certified laboratory (See form attached). Monitoring Program (continued) Part Il, Section C of the permit requires semi-annual qualitative (visual) monitoring to be performed and documented. This monitoring should occur once in the spring (April- June) and once in the fall (September — November). You are not required to submit this information to the Division, however, you must be keep these records on file at the facility for a period of five years (See form attached). Monitoring under Section D of the permit is not required since on -site vehicle maintenance does not use more that 55 gallons of new motor oil per month when averaged over the calendar year. Please make sure that the required analytical and visual monitoring is performed in 2006 per the permit requirements. Materials Include: Liquid Asphalt Diesel Fuel Release Agent Various Aggregates Aggregates are stored in stockpiles that are received by truck. During production, the aggregates are transferred to cold feed bins by means of a front-end loader. This material is then transferred to the dryer by means of a conveyor to a mixing drum where a process burner is used to remove moisture and heat the aggregate to the desired temperature. Liquid asphalt is then introduced to the heated aggregate in the mixing drum to produce the desired mixture. Process dust is controlled by the use of a bag house where the dust is suppressed by filters and introduced back into the mixing drum by an enclosed augering system. Liquid asphalt and fuel are unloaded on site. Tanker loads are delivered as needed and are off loaded into our storage tanks by the tanker driver. Visual inspections are performed during the unloading process. Based on the site assessment that was conducted on June 10, 2005, the following items are considered to be the most likely sources of pollution: Diesel Fuel Liquid Asphalt Aggregates Poor Housekeeping Storm Water Management Plan To reduce the amount of sediment from leaving the asphalt plant site during storm events, we have directed the flow of water away from the stockpile areas. Bermed areas are used to eliminate the flow of sediment before it leaves the site. Best Management Practices Source: Aggregates Keep area graded to prevent storm water from passing through this material. Keep flow of storm water from concentrating in a specific area. There is currently no concentrated runoff area or areas on -site and no drainage ditches. Daily visual inspections. Source: All Tanks Visual Inspections during all off-loading. Daily visual inspections. Source: House Keeping Training meetings scheduled on a regular basis. Keep site organized and graded to prevent concentrated flows. Daily visual inspections.
