On April 15, 2008, Jenny Graznak of this office performed a General Stormwater Permit
Inspection at the APAC-Atlantic, Inc Asphalt Plant located on U S Highway 158 in Stokesdake,
North Carolina This facility holds General Stormwater Permit No NCG160126 under the
National Pollutant Discharge Elimination System (NPDES) to discharge Stormwater from
activities associated with Asphalt Paving Mixtures and Blocks You were present for the
inspection This type of inspection consists of two basic parts a review of facility files and self -
monitoring data and an inspection of the facility's discharge points This particular inspection
evaluated five (5) areas, and observations from each area are addressed below
1 Permit
The permit became effective August 1, 2004 and expires July 31, 2009
2 Records/Reports
The facility has a Stormwater Pollution Prevention Plan (SPPP) that includes a location map,
site plan, storage practices, and a list of Best Management Practices on the site Engineering
Consulting Services, Ltd (Greensboro, NC) created the SPPP in 2004 Employee training in
stormwater pollution prevention occurs on an annual basis
3 Facility Site Review
This plant is not operating at present The site includes an asphalt production area and an
aggregate storage area (sand, gravel, debris, etc ) The production area houses liquid asphalt
and fuel storage protected with secondary containment The foreman is required to observe the
standing water in the containment area for signs of contamination prior to its release
4 Effluent/Receiving Waters
Stormwater from this site drains to an unnamed tributary to Haw River (Class C, NSW) in the
Cape Fear River Basin There are two stormwater ponds on the property that operate in series
The second pond has two outfalls and only one is monitored If a facility has more than one

5 Self -Monitoring Program
The stormwater permit requires semi-annual qualitative monitoring at each stormwater
discharge outfall An on -site review of the qualitative monitoring data during the inspection
showed that all parameters were evaluated per the specifications of the permit The plant
foreman Ronnie Hairston made the most recent visual observation on April 6, 2008
Analytical monitoring is also required on an annual basis Paradigm Analytical Laboratories
performed the analytical monitoring on October 7, 2005, and the results were as follow
