On January 20, 2010 George Smith, both of this office, performed a Compliance Evaluation Inspection. Ms. Lori Cat -ter,
Environmental Representative and Mr. Rodney Murry, Plant Foreman, were present for the inspection.
Permit
This compliance inspection used the previous NTDES permit that expired on July 31, 2009 in order to judge the
compliance of 2009 analytical data.
Beginning in 2010 you will be required to perform Total Petroleum Hydrocarbons semi annually as required in the new
permit that expires on September 30, 2014, Please remember that this permit is in effect, and that some monitoring
requirements were changed.
Records & Reports
All elements of the Stortmvater Pollution Prevention flan were satisfactory.
2. Monitoring was performed as required per Part 11, Section 13, "Cable 1 & 2 - Analytical Monitoring.
3. Monitoring was performed as required per Part 11, Section C, Table 3 - Qualitative Monitoring.
Facility Site Review
The facility has two stormwater discharge outfalls.
Outfail 002 discharges adjacent to the front entrance.
Outfall 001 is a stormwater pond that discharges to the stream. At the time of the inspection the outfall was, discharging

On April 15, 2008, Jenny Graznak of this office performed a General Stormwater Permit
Inspection at the APAC-Atlantic, Inc. Asphalt Plant located on Riverdale Drive in Jamestown,
North Carolina. This facility holds General Stormwater Permit No. NCG160122 under the
National Pollutant Discharge Elimination. System (NPDES) to discharge Stormwater from
activities associated with Asphalt Paving Mixtures and Blocks. You were present for the
inspection. This type of inspection consists of two basic parts: a review -of facility files and self -
monitoring data and an inspection of the facility's discharge points. This particular inspection
evaluated five (5) areas, and observations from each area are addressed below:
1. Permit
The permit became effective August 1, 2004 and expires July 31, 2009.
2. Records/Reports
The facility has a Stormwater Pollution Prevention Plan (SPPP) that includes a location map,
site plan, storage practices, and a list of Best Management Practices on the site. Engineering
Consulting Services, Ltd. (Greensboro, NC) created the SPPP in 2005, Employee training in
stormwater pollution prevention occurs on an annual basis.
3. Facility Site Review
The site includes an asphalt production area and an aggregate storage area (sand, gravel,
debris, etc.), The asphalt production area houses liquid asphalt, hydraulic and fuel oils
protected with secondary containment. The foreman is required to observe the standing water
in the containment area for signs of contamination (oil sheen, color, foam, etc.) prior to its
release. The water is restrained by manually operated and locked valves.
4. Effluent/Receiving Waters
Stormwater from this site drains to Cooper Branch (Class WS-IV) in the Cape Fear River Basin.
There are two stormwater oufalls and one retention pond on the site. Outfall 001 drains from
the on -site retention pond into Cooper Branch. Outfall 002 discharges to a ditch along
Riverdale Drive that ultimately drains to Cooper Branch. This outfall is protected by a sediment
control measure to keep silt and sediment on the property. Cooper Branch borders the property
line of the plant.

5. Self -Monitoring Program
The stormwater permit requires semi-annual qualitative monitoring at each stormwater
discharge outfall. An on -site review of the qualitative monitoring data during the inspection
snowed that all parameters were evaluated per the specifications of the permit. The plant
foreman Rodney Murray made the most recent visual observation on April 4, 2008.
Analytical monitoring is also required on an annual basis. Paradigm Analytical Laboratories
performed the analytical monitoring on October 7, 2005, and the results were as follows:

requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the U. S. Environmental Protection Agency dated December 6, 1983.
This permit modification does not affect the legal requirement to obtain other permits which may be
required by the Division of Water Quality, the Division of Land Resources, Coastal Area Management Act, or any

Water Quality (DWQ) is forwarding herewith the reissued stormwater general permit. This permit is reissued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
1+et%veen the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.
The following information is included with your permit package:
* A copy of general stormwater permit NCG160000
* A Stormwater Pollution Prevention Plan (SPPP) Certification Form. Completion of this form is required to
certify that you have developed and implemented the SPPP as per the conditions of the permit. This form
must be completed and returned to the Division within 30 days of receipt of this letter.
DO NOT SEND the SPPP with the signed form.
* Five copies of the Analytical Monitoring form and five copies of the Qualitative Monitoring form
* A copy of a Technical Bulletin for the general permit which outlines changes in the permit, key requirements,
and addresses frequently asked questions
* A Certificate of Coverage
Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require
modification or revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by DENR or relieve the permittee from responsibility
for compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order, judgment, or
decree.
