Inspection Summary:
Susan Wilson met with Don Mason at the APAC Hendersonville plant on April 25, 2013. The site is well maintained and
operated.
During this inspection - 3 potential outfalls were noted (and only 2 are noted on the site plan). This should be clarified
(and modified in the plan - as noted in the Permit and Outfalls section). The basin with the valved outlet near the RAP
piles appears to need maintenance.
A berm and stabilization is needed near the new power pole.
APAC should consider ways to reduce residual solids from impacting the entrance (although DWQ understands the
difficulty of this due to the nature of the property boundaries).

Comment: The main comment with regard to the outfalls (which wasn't apparent
during the previous inspection) - is that only 2 outfalls have been designated and are
part of the SWPPP. There appear to be 3 areas where a discharge could occur: at the
concrete basin area near the 'silos', at the trap with rip -rap overflow, and at the valved
basin area behind the RAP piles nearest to Clear Creek Road. Please confirm/clarify
the number of outfalls and ensure they are designated on the SWPPP (and monitored).

Inspection Summary:
Susan Wilson and Tim Fox (DWQ) met Greg Davis (APAC) to conduct a site inspection at APAC Hendersonville
The site appeared well operated and maintained. Records were well kept and presented.
The facility has 2 designated out -falls and APAC has the ability to control the release from these outfalls. Both areas
discharge to a roadside drainage swale and ultimately to an unnamed tributary to Mud Creek. No water quality issues
were noted near the discharge area.
The corner of the property (at Howard Gap Road and Clear Creek Road) is experiencing some erosion and may need to
be addressed in the future. However, per Greg, this area will be bought by DOT in the future and will likely not be an
issue for APAC. APAC should keep DWQ posted on the timeframe for the DOT acquisition.

certify that you have developed and implemented the SPPP as per the conditions of the permit. This form

In accordance with your application for discharge permit received on November 9, 1995, we are
forwarding herewith the subject certificate of coverage to discharge under the subject state - NPDES general
permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and
the Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated
December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division
of Environmental Management. The Division of Environmental Management may require modification or
revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Environmental Management or permits required by the Division of Land Resources, Coastal
Area Management Act or any other Federal or Local governmental permit that may be required.
