Glen White, multimedia inspector with NCDEQ - DEMtR, met with Russ Taggart (Plant Foreman) and George Bennett
(Equipment Engineer) at the Blythe Asphalt plant located at 2083 Bishop Rd in Greensboro on 7/20/2017 to conduct an
inspection that included compliance review of the facilities air and stormwater permits
The facilities new SPPP (Stormwater Pollution Prevention Plan) was developed by AARC Environmental and was signed off
by them on 9/22/2016 Blythe certification was signed off on 10/17/2016 Their SPPP is reviewed annually Training is
conducted at the corpoarte office in Charlotte on January 2nd of each year and includes all employees Qualitative and
Analytical Monitoring is conducted twice annually in accordance with the permit The last two analytical samples were
taken on 312/17 and 9129116 The sample location is the outfall below the settling pond at the rear of property The outfall is
very near the creek Creek was clear at the time of inspection '

front of the site This basin was dry at the time of inspection but it captures a large area of offsite

would not be representative of thews halt plant as most of water is coming from offsite
