Inspection Summary:
NCDEQ - Multi -Media Inspectors Blair Palmer, Ryan Swaim, Charles Gerstell, Jack Kitchens and Glen
White met with Joel Greene of Carl Rose & Sons to conducted an inspection of the Carl Rose Facility located at 155
Asphalt Trail in Elkin, NC on May 2, 2017.
The following deficiencies were found and request is being made to address/correct the issues:
1. The current permit requires both Qualitative Monitoring to be conducted twice (2) annually.
The facility is currently conducting once (1) annually as the old permit required. Please begin monitoring semi-anually as
per the current stormwater permit.
2. Analytical samples not collected correctly. Samples need to be collected from "flowing"
stormwater leaving the outfall of the basin or somewhere below the outfall of the basin before
water leaves the property boundary. This may require a pipe to be installed at the outlet so a
clean catch can be made or installation of a v-notch or stone that water will run over as it empties from the basin. Sampt(
need to be collected within the first 30 minutes of flow during a qualifying storm event.
3. The basin has substantial sediment accumulation. Sediment needs to be cleaned out and new rip -rap installed at the
outlet.
4. Address housekeeping issues. Clean-up grounds near the shop/maintenance area.
5. Address solid waste issues as requested by Solid Waste Inspector, Charles Gerstell. His letter
will follow under separate cover.

Stormwater Permit Inspection at the Carl Rose & Son's Elkin facility located on Elkin Wildlife
Road in Surry County. This inspection evaluated the areas as designated on the attached
inspection form. Observations from each area are addressed below:
1. Permit
Carl Rose & Son's holds Certificate of Coverage # NCG160025 for the discharge of
stormwater from the facility. The General Permit became effective October 1, 2009 and
expires September 30, 2014,
2. Records/Reports
Part II, Section A of the permit requires the development of a Stormwater Pollution
Prevention Plan (SPPP). A copy of the SPPP was available for review. The plan contained
all of the items required by the stormwater permit.. Please note that the permit requires the
plan to be reviewed and updated on an annual basis.
3. Facility Site Review
Observations of the one outfall at the facility did not reveal any concerns. The site appeared
very clean and well managed.
4. Effluent/Receiving Waters
Stormwater runoff from the facility reaches an unnamed tributary to the Yadkin River, Class
"C" waters in the Yadkin/Pee-Dee River Basin.

— November). A review of the facility files found that the qualitative monitoring has been
conducted as required.
Part II, Section B of the permit requires that analytical monitoring be performed. A review of
the facility files found that the analytical monitoring has been conducted as required.
The analytical monitoring for on -site vehicle maintenance listed in Section B of the permit is
not required since the Carl Rose & Son's plant uses less than 55 gallons of new motor oil per
month when averaged over the calendar year.

On May 30, 2008, Gary Hudson of this office met with you to perform a General
Stormwater Permit Inspection at the Carl Rose & Son's Elkin facility located on Elkin Wildlife
Road in Surry County. This inspection evaluated the areas as designated on the attached
inspection form. Observations from each area are addressed below:
1. Permit
Carl Rose & Son's holds Certificate of Coverage # NCG160025 for the discharge of
stormwater from the facility. The General Permit became effective August 1, 2004 and
expires July 31, 2009.
2. Records/Reports
Part ii, Section A of the permit requires the development of a Stormwater Pollution
Prevention Plan (SPPP). A copy of the SPPP was available for review. The plan contained
all of the items required by the stormwater permit. Please note that the permit requires the
plan to be reviewed and updated on an annual basis.
3. Facility Site Review
Observations of the one outfall at the facility did not reveal any concerns. The site appeared
very clean and well managed.
4. Effluent/Receiving Waters
Stormwater runoff from the facility reaches an unnamed tributary to the Yadkin River, Class
"C" waters in the Yadkin/Pee-Dee River Basin.

5. Monitoring Program
Part II, Section C of the permit requires that semi-annual qualitative (visual) monitoring be
performed and documented once in the spring (April - June) and once in the fall (September
-- November). A review of the facility files found that the qualitative monitoring has been
conducted as required.
Part 11, Section B of the permit requires that analytical monitoring be performed. A review of
the facility files found that the analytical monitoring has been conducted as required.
The analytical monitoring for on -site vehicle maintenance listed in Section B of the permit is
not required since the Carl Rose & Son's plant uses less than 55 gallons of new motor oil per
month when averaged over the calendar year.
