On January 24, 2019, Brandon Wise, inspector with the North Carolina Department of
Environmental Quality (NCDEQ) — Division of Energy, Minerals, and Land Resources met with
you at the facility located at 146 Howard Woltz Jr. Way in Mount Airy, to conduct a compliance
inspection. The inspection consisted of review of the Stormwater Pollution Prevention Plan
(SWPPP) and each of its components, review of the facilities' outfalls, as well as the review of
the overall site conditions.
The compliance inspection was conducted to ensure all site practices and facilities are in
working order. The inspection was also conducted as assistance to Mount Airy — Surry County
Airport should there be any questions about the permit NCG150037.
Permit:
This facility has General Stormwater Permit NCG150037 to discharge stormwater from
industrial activity associated with Air Transportation Facilities [SIC 451 under the National
Pollutant Discharge Elimination System (NPDES). The facility became permitted on March
26th, 2010 and the current version of the permit expires on August 31 St, 2022.
Records/Reports:
This facility is required to develop and maintain a SWPPP in accordance with Part II of
the permit. All qualitative and analytical monitoring records are required to be maintained with
the SWPPP for a minimum of five (5) years. Under this permit, analytical monitoring is not
required under current operating procedures. The facility records are kept up to date and have
the required 5 years of previous monitoring. The SWPPP for this facility was very well put
together and had been implemented prior to the inspection. The facility is required to maintain
an annually updated SWPPP with an updated site map in accordance with Part I1, Section J of the
permit. Ensure that the SWPPP is properly updated to show changes to the site and a cover sheet
showing the date of the update.

Facility Site Review:
This facility operates an airport for small, non-commercial planes with hangers on site for
private tenants along with a community hanger. Fueling is done by airport personnel at the
southwest corner of the site. Two private hangers have their own fueling stations located outside
of the hangers. All hangers are located at the southwest corner of the site near the entrance and
water flows toward the south end of the site. All fueling stations have a spill kit for response to
any spilled liquids. Most potential pollutants are stored indoors, and are located in an area where
quick response is required by multiple agencies for safety reasons. At the time of the inspection
the site was in immaculate condition and no potential pollutants were observed by NCDEQ
personnel.
Multiple outfalls are located along the eastern edge that discharge to an un-named
tributary of Stoney Creek and three more outfalls are located at the southern edge of the site
discharging directly to Stoney Creek. At multiple locations water was observed entering and
exiting concrete pipes as it was redirected around the runways. These locations were monitored
along with the proper outfalls as a way to ensure an even more robust monitoring schedule.
Overall the outfalls and other exposed pipes appeared to be very well maintained and easily
accessible.
Effluent Receiving Waters:
Effluent from this facility drains into Stoney Creek, class "C" waters of the Yadkin Pee -
Dee River Basin.
Self -Monitoring Program:
This facility is required to provide Qualitative Monitoring twice annually. There are
multiple outfalls around the site that are properly maintained and monitoring is performed there.
Overall Brandon Wise, inspector for NCDEQ, has no issues with the site. Ensure that the
SWPPP is properly updated and continue to monitor as you have been. If you have any

Inspection Summary:
Site was very well maintained. No trash or signs of any spills on site. SWPPP was very well put together. Maps needed to
be updated as runways have been extended in recent months with addition drainage area and this is not shown in SWPPP.
SWPPP date is Dec. 2010, needs to be updated. SWPPP also showed that all tenants had reviewed the SWPPP and its
requirements. Outfalls inspected regularly and at a greater frequency then required by permit.

The NCG150000 General Permit expires on August 31, 2017. Our records indicate that your annual fees
are up to date; therefore, your coverage will be automatically renewed unless you submit a rescission
request for this permit. If you no longer need this permit, a rescission form can be found on our website

The proposed new General Permit NCG150000 is available for public comment on our website (as of
June 9, 2017). This notice process will conclude by August, so that the new general permit can be issued
effective September 1, 2017. Upon issuance of the final general permit, your coverage will be renewed,
and we will send you a new Certificate of Coverage (COC) with the new effective and expiration dates.
To facilitate this renewal process, you must review your permit contact information and
notify us of any changes no later than July 31, 2017.

E-Reporting Update
The U.S. Environmental Protection Agency (EPA) finalized the National Pollutant Discharge Elimination
System (NPDES) Electronic Reporting Rule in December 2015. The rule requires NPDES regulated
facilities to report certain information electronically, instead of filing written paper reports. The first
phase required regulated entities to submit Data Monitoring Reports (DMRs) electronically starting on
December 21, 2016. However, currently General Permit NCG150000 does not require any analytical
monitoring and therefore is not subject to electronic reporting requirements at this time.
This general permit may be affected in the future by electronic reporting requirements. For more

forwarding herewith the subject certificate of coverage (COC) to discharge under the subject state — NPDES
general permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1
and the Memorandum of Agreement between North Carolina and the US Environmental Protection Agency
dated October 15, 2007 (or as subsequently amended).
If any parts, measurement frequencies, or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division
of Water Quality. The Division of Water Quality may require modification or revocation and reissuance of the
certificate of coverage. This permit does not affect the legal requirements to obtain other permits which may be
