Inlerim Direclor On July 31, 2018, Isaiah Reed from the Asheville Regional Office of the Division of Energy, Mineral and Land Resources, conducted a site inspection for the American Concrete facility located at 2698 Mount Home Church Road, Burke County, North Carolina. Your time and assistance during the inspection were greatly appreciated. A copy of the Compliance Inspection Report is enclosed for your review. The inspection and file review revealed that the subject facility is covered by NPD>; S Stormwater General Permit-NCG 140000, Certificate of Coverage-NCG 140435. Permit coverage authorizes the discharge of stormwater fi•om the facility to receiving waters designated as the East Prong Hunting Creek, a Class WS-IV rvaterbody in the Catawba River Basin. As a result of the site inspection, the following permit violation(s) are noted: 1) Stormwater Pollution Prevention Plan (SPPP) Failure to provide and properly implement a Stormwater Pollution Prevention Plan (SWPPP) as outlined in North Carolina General Permit NCG 140000 (Part III, sections 1-9) -At the time of the inspection, no Stormwater Pollution Prevention Plan (SWPPP) was available on site. 2) Qualitative Monitoring Failure to conduct Qualitative monitoring as outlined in the NCG 140000 permit (Part IV, Sections A, B, C, and D). -At kite time of the inspection, no qualitative data was available on site, 3) Analytical Monitoring Failure to conduct Analytical monitoring as outlined in the NCG 140000 permit (Part IV, Sections A, B, C, and D). -At the time of the inspection, no analytical monitoring data was available on site. 4) Site Maintenance Failure to conduct all necessary site maintenance as outlined in the NCG 140000 permit (Part V, Section C). -At the time of the inspection, there was visible off site deposition of sediment and concrete waste in the roadside drainage ditch adjacent to the south side of the facility. -At the time of the inspection, the basins observed were not appropriately maintained. My review of the NOI indicates we won't have any problems issuing it, but just can't get to it in a quicker time frame. These folks are already in operation, and we are going to issue them a permit. They need help, however. I think ARO should skip the site visit as a part of the permit issuance process, but should follow up right away after issuance to get these folks moving in the right direction. KBP opinion - Suggest RO confirmation of the physical facts of drainage during first site visit. o DA3 -- Surveyor suggests this is sheet flow or small rill flow. KBP opinion - Probably right, but suggest RO confirmation at their first inspection. Surveyor reports a tree line and berm to the east. KBP suspicion that there might also be a drainage feature in the tree line. We can process the NO1 without resolution on this point, imb. s o DA4 — Piped flow, from the hot area of mixing and wash down. Pathetic sediment ponds unlikely to be controlling to permit limits or benchmarks. No pH control apparent. (ACI) property. This area serves as employee and customer parking, concrete truck, and equipment parking and maintenance. This area also has office and a frame maintenance building, and product form areas, product storage, and material bins, and sand and gravel stockpiles. This is the area of most heavy traffic on the site. It is almost entirely a well graveled surface with areas of spilled concrete. The Lail property located to the northeast serves as a watershed area of 0.5 acres which slopes 7% to the southwest corner of the of the Edward Lail lot. This watershed area is entirely a lawn. This runoff as well as runoff from the sand/gravel stockpiles and materials bins crosses the center of D/A #2 flowing to the southwest. Additional runoff comes from the northern portion of D/A #2 (area north of the frame building, and east and west of the office building) and converges in the truck parking area just south of the frame building and the office building. The grades along these areas varies a lot, but is near 5% thru the truck parking area. Runoff rxits the property at _a Rravel driveway_ entrance at a 10% grade dear the end of asphalt of Hill Street (and is the point of interest of calculations of flow).Runoff crosses Hill Street and runs down a vegetated road bank into a hayfield owned by Delores Stroune to the South. This area serves as product storage area, concrete form area, and a portion of the sand and gravel stockpile. DIA #3 has practically no watershed to the north and the east, as the runoff from the fail yard travels west, and a berm along the eastern property line blocks water from entering from the east. A ditch along the west side of this berm serves to carry much runoff from the east side of A #3 to the south, as well as the whole area drains to the south into a natural woode where it crosses the Stroupe property to the south. A small portion of D1A 3 western most point is a concrete pad used for drum storage of chemicals and contains a metal outbuilding. Runoff from this area flows south toward Hill Street.
