operates a ready mix concrete production facility at 3107 R3 S Elm Eugene Street, Greensboro, North Carolina This facility is located on 5 43 acres Ready mixed concrete containing water, cement, fly ash, aggregates and supplementary additives is produced at this site Cement and fly ash are stored in silos These materials are transported from the delivery truck to ' the silos using a pneumatic system The silos are each vented to a baghouse The bags are periodically shaken and collected material is returned to the silos aggregate, vn7Q iilg of Sails arld three L] pt* of sLol le, are SLOred In four COrlCrete bins These bins are three sided concrete structures The aggregate is delivered near the appropriate bin by a dump truck and a front ender loader is used to move the material into the bin As needed in the ' concrete production process, aggregate is loaded into the front end loader bucket and placed into a feedinn hnnnPr that riicharnAc to an elnira+nil nnnntirnr hnl+ +ham+ nnn located in the chemical storage facility The tanks are filled from a commercial transport truck using fillports located within the storage facility ' A computerized control system is used to produce the required concrete mix When a concrete mix truck is ready to be filled it is parked under the weigh hopper A shroud surrounds the truck feeding hopper and emissions from the filling process are vented to the baghouse To produce truck -mixed concrete, all of the ingredients are charged to and mixed in the truck t mixer The process begins with the discharge of approximately ninety -percent of the required water to the truck mixer The required aggregate materials are then fed from the batch plant to a scale followed by transport on a conveyor belt to the truck mixer Required amounts of ' cement and fly ash are weighed on a separate scale system and pneumatically conveyed to the truck mixer Liquid additives and the remaining water are then pumped to the truck mixer Prior to leaving the facility, the truck is driven to the wastewater recycle system The truck driver 6 i Prestige Concrete Products ' Stormwater Pollution Prevention Plan ECS Project 09-21436 December 6, 2d12 uses clean water from a tank located on the truck to clean the truck feeding hopper Wastewater generated from this cleaning procedure is discharged to the wastewater recycle system The facility has a closed loop recycle system that treats process wastewater generated from cleaning of external truck surfaces and mixing drum cleaning All cleaning will be conducted in an area constructed to discharge to the closed loop recycle system Wastewater flows from the cleaning pad to a three celled settling basin, with overflow from each cell flowing over a weir to the next cell The water from the last cell is recycled and used as mixing drum washwater Based on ' storm volume, there may be overflow during storm events If the system overflows, overflow will flow by overland flow to the facility detention pond Residuals collected in the closed loop recycle system and the detention pond are periodically ' removed to maintain settling capacity Residuals will be removed from each cell of the recycle system on a weekly basis and as needed from the detention pond Residuals will be placed on a concrete pad that drains to the closed loop recycle system Dry residuals will be sold The detention pond incorporates a pH adjustment system This system will produce an ettluent with a pH within the acceptable range and pH adjustment will also cause precipitation of some ' component of the dissolved and suspended solids The facility stores sand and aggregate material in four three sided uncovered concrete bins In ' the summer these materials are sprayed with water to reduce their temperature Water from spraying the sand and aggregate piles will either evaporate or flow by overland flow to the wet detention pond ' The facility has one budding in which the vehicle and equipment maintenance facility, the office, and the control room are located There are oil storage containers located within the vehicle ' and maintenance facility area and two additional aboveground diesel fuel tanks are located in the vicinity of the concrete plant Debris and trash are stored in one dumpster for subsequent off -site disposal ' 2 3 2 Site Map Figure 2 is the site map for the facility following proposed demolition and construction activities Figure 2 identifies facility boundaries, stormwater discharge outfalis, site topography, drainage features, approximate direction of stormwater flow, structures and impervious surfaces Figure 3 identifies the locations of industrial activity and bulk storage tanks 2 3 3 Site Drainage and Receiving Water Impairment Classifications At this site, surface topography controls the facility drainage Approximate drainage patterns are indicated on Figure 2 The area of industrial activity drains by overland flow or through swales to a wet detention soil are identified in the future they should be covered with gravel The construction activities to be conducted at this facility will disturb 125 acres Construction activities that disturb more than one acre require a stormwater permit issued by the NCDENR or a delegated local regulatory agency The stormwater permit requires development and implementation of a project specific Erosion Control Plan As this project does not increase the built -upon areas, the delegated local authority, the City of Greensboro, does not require submission of an Erosion Control Plan An Erosion Control Plan for the proposed construction project has, however, been prepared and will be submitted to the NCDENR ' If additional construction activities are considered in the future, prior to initiating such activity erosion control measures must be identified Larger construction activities conducted by outside contractors require inclusion of project -specific erosion control measures as part of the contracted ' activities The necessity for erosion control activities for facility conducted activities will be dependent on the project size, duration and location Projects conducted by facility personnel which disturb small areas may only require limited erosion control measures which may include 9 Prestige Concrete Products ' Stormwater Pollution Prevention Plan ECS Project 09 21436 must prevent or minimize exposure of all storage areas, material ' handling operations and vehicle/equipment maintenance and fueling operations Due to the nature of some of the facility activities, it is neither economically feasible nor operationally practical to cover or contain all operations In such cases, efforts should be made to limit run-on and run-off in ' areas where materials are exposed to stormwater The feasibility and constraints of various measures are discussed below 3 2 SECONDARY CONTAINMENT AND PROCEDURES ' 3 21 Secondary Containment To prevent leaks and spills from contaminating stormwater, the discharge permit requires secondary containment for 1) liquid materials stored in bulk, 2) any quantity of a substance designated as a water priority chemical by Section 313 of Title III of the Superfund Amendments and Reauthorization Act, and 3) any quantity of a substance designated as hazardous under the Code of Federal Regulations Tale 40, Part 116 There is bulk storage of liquid materials at this facility, but substances designated as water priority chemicals or Part 116 hazardous substances are not expected to be stored at this facility Concrete production additives are stored in four 925-gallon ASTs and three 525-gallon ' chemical ASTs, with occasional additional storage in totes Petroleum products are stored in bulk at the site in aboveground storage tanks, drums, vehicle reservoirs and small containers ' Bulk storage is defined by the stormwater permit as a single above -ground storage container having a ranaoty of greater than 660 gallons or multiple ahnV,e-ground Ustoru''',() container] of a substance located in close proximity to each other with a total combined storage capacity of greater than 1,320 gallons The Oil Pollution Prevention Regulation addresses facilities with an aggregate aboveground oil ' storage capacity exceeding 1,320 gallons This regulation requires secondary containment for oil storage containers with a capacity greater than or equal to fifty-five gallons Vehicle tanks (trucks, loaders, etc) used to hold fuels or fluids required to operate the vehicle are not required by the Oil Pollution Prevention Regulations to have secondary containment Both the stormwater discharge permit and the Oil Pollution Prevention Regulations require secondary containment provide sufficient capacity for the largest single tank within the containment area and sufficient freeboard for precipitation (if exposed) AlthoUnh the rerpiations do not set a standard for "sufficient" freeboard, the EPA recommends sufficient freeboard for a 25-year, 24-hour storm event For the site, a 25-year storm event is 5 65 inches The USEPA has 0 accepted shop built double -walled tanks as an equivalent preventative system of the location and use of spill control and containment equipment Contractors and temporary personnel must also be informed of facility operation and design features in order to prevent discharges or spills from occurring 5 6 MONITORING AND REPORTING REQUIREMENTS ■ The facility is required to conduct analytical monitoring and qualitative monitoring of stormwater discharges Analytical monitoring requires sample collection followed by laboratory analyses Qualitative monitoring requires visual observation of site stormwater Analytical monitoring requirements are described in Sections 5 61 through 5 6 3 Qualitative monitoring requirements are described in Section 5 6 4 ' 5 61 Analytical Monitoring Sample Collection ' The permit to discharge stormwater and process wastewater from ready mix concrete facilities has monrtonng requirements specific to the type of stormwater or wastewater discharged The permit categorizes facility discharges as follows 1 Stormwater that is not commingled with process wastewater, 2 Stormwater from vehicle maintenance areas that is not commingled with process wastewater, and, 3 Process wastewater associated with rinsing the external surfaces of vehicle and equipment, wetting of raw material stockpiles, and mixing drum cleanout, including commingled stormwater and/or stormwater from vehicle maintenance areas The Prestige Ready Mix facility has one point source discharge from industrial operations at the facility consisting of overflow from the detention pond (outfall 001) Facility stormwater and process wastewater from wetting raw material stockpiles flow directly to the detention pond, as does stormwater from the vehicle maintenance area Wastewater generated from vehicle and equipment cleaning and mixing drum cleanout is treated in a closed loop recycle system and reused for mixing drum cleanout In the event the closed loop recycle system overflows, water discharged from that system would also flow to the detention pond Since the discharge from outfall 001 will consist of process wastewater commingled with stormwater, the requirements for analytical monitoring of process wastewater apply Analytical monitoring for process wastewater must be conducted four times per year, with an increased frequency if laboratory analytical results exceed permit limits or benchmark values Samples must be collected during each of the following collection periods SAMPLE COLLECTION PERIOD Jams 1 to March 31 April 1 to June 30 July 1 to September 30 October 1 to December 31 29 Prestige Concrete Products Stor—owaier Poliuton Prevention Plan ECS Project 09-21436 December 6, 2012 If the facility fails to perform quarterly monitoring, immediately increases to monthly monitoring After monitoring requirement reverts to quarterly monitoring, Quality requires continued monthly monitoring the analytical monitoring requirement six months of monthly monitoring, the unless the NCDENR Division of Water As samples from outfall 001 are considered process wastewater, they must be analyzed for the parameters listed in the following table Flow from outfall 001 is expected to occur only during precipitation events Samples collected during precipitation events must be collected during the first 30 minutes of discharge from outfall 001 MONITORING AND REPORTING REQUIREMENTS DISCHARGE CHARACTERISTIC UNIT H standard Total Suspended Solids m Settleable Solids mgA Total Petroleum Hydrocarbons mgA Discharge Duration minutes Total Flow Ions pqr day Total Precipitation* inches *Total precipitation is not a repotted monitoring requirement, but is necessary to estimate total flow For each analytical monitoring event, the total discharge duration and total precipitation must be recorded and the total flow must be calculated or measured Samples must be analyzed for pH, total suspended solids (TSS), settleable solids and total petroleum hydrocarbon co^.caI, —1;o^s The NCDENR requires pH be analyzed within fifteen minutes of sample collection using a calibrated meter which meets NCDENR specifications 5 6 2 Analytical Monitoring Reporting The laboratory analytical results must be entered onto a discharge monitoring report (DMR -- Appendix IX) The DMR must be submitted to the NCDENR within thirty days of receipt of data from the laboratory If no discharge occurs during a sampling period, the monitoring report must be submitted within thirty days of the end of the monitoring period with the statement "no flow " rIn addition to the results of laboratory analyses, the DMR requires reporting of the total discharge duration (minutes) and total flow (gallons) The facility must install a rain gauge and estimate discharge duration for each monitored stormwater event As discussed below, the Simple Method may be used for estimating stormwater flow from outfall 001 The Simple Method calculates the runoff from a watershed based on a relationship between the percent of impervious area and the amount of rainfall ---veiled to runoff The Striple Method estimates flow as follows 1 30 Prestraa Concrete Products Stonrvater Pollution Prevention Plan ECS Project 09-21 436 December 6 2012 V= 3630 * Rd ' Rv * A *7 48 V = Stormwater flow (gallons) Rd = Storm depth (inches) Rv = Runoff coefficient, where Rv=005+09*lA IA = Impervious fraction (2 23 acres/2 75 acres = 0 8109) A = Watershed area (acres) = 2 75 The DMR requires flow (V) be reported in million gallons Based on the Simple Method, the flow in million gallons would be calculated by multiplying the following V(outfall ool) = 0 058228(I), where I = Total Rainfall (inches) V = Stormwater Flow (MGD) 5 6 3 Effluent Limits and Benchmark Values Wastewater discharges from outall 001 are subject to the effluent limits for process wastewater discharges listed in the following table An exceedence of any of these limitations will result in a violation of the permit conditons Violation of permit limits twice in a row immediately increases the monitoring frequency to monthly monitoring for all wastewater parameters After six months of monthly monitoring, the monitoring requirement reverts to semi-annual monitoring, unless the the No Exposure Exclusion, or • Require the permittee to perform upstream and downstream monitoring to characterize impacts on the receiving waterbody 5 6 4 Qualitative Monitoring Semi -Annual Qualitative Monitoring must be conducted twice each year during an analytical monitoring event The sample collection penods are as follows MONITORING SCHEDULE Sample Collection Period January 1 to June 30 July 1 to December 31 A minimum of sixty days must separate each monitoring event For qualitative monitoring, sampling is conducted by collecting a grab sample at each permitted outfall dunng a "measurable storm event" A "measurable storm event" is a storm event that results in an actual discharge from the permitted site outfall, and occurs at least 72 hours after the previously measurable storm event The samples must be collected during the first 30 minutes of discharge from ouffall 001 33 Prestige Concrete Products Etormwater Polloon Prevention Plan ECS Project 09-21436 December 6, 2012 Qualitative monitoring requires an assessment of color, odor, clarity, floating solids, suspended solids, foam, oil sheen, erosion or deposition at the outfall and other obvious indicators of stormwater pollution Qualitative monitoring must be documented on forms contained in Appendix IX Completed forms must be maintained on -site with the Plan If qualitative monitoring indicates existing stormwater BMPs are ineffective or significant stormwater contamination is present, the permittee must investigate causes, evaluate the ' feasibility of corrective actions and implement corrective actions within 60 days This investigation may be conducted using the general sections of the Comprehensive Site Compliance Evaluation and the sections applicable to the outfall of concern (Appendix VII) A written record of the Iinvestigation, evaluation and response actions must be kept in the SPPP if the permitee repeatedly fads to respond effectively to correct problems identified by qualitative ' monitoring, or if the discharge causes or contributes to a water standard violation, DWQ may take, but is not limited to, the following actions Require that the permittee revise,
