Introduction
On April 6, 2016 Mr Paul Williams, Environmental Specialist, DAQ WSRO contacted Mr Kenneth
Waegerle, Corporate EHS Manager, of Chandler Concrete/Piedmont, Inc -Plant I I7-Elm Street for an
announced targeted multi -media compliance inspection The facility was previously inspected on
October 7, 20I4 by this inspector and was found to be operating in compliance with thee' Air Quality
Permit regulations at that time The facility is a concrete batehmg and truck -mix facility that currently
operates 8-10 hours a day, five days a week, and 50 weeks per year During the winter months, they
experience a reduction in production and operating hours Mr Waegerle stated that production has
increased since this time last calendar year Mr Waegerle stated that the listed facility contact, Mr
Robert McDaniel, is no longer with the company and that Brian Marsh, Plant Manager, should be added
as the new facility contact This has been updated in IBEAM All other IBEAM contacts were verified
as being accurate Mr Waegerle also stated that no new equipment has been added to the facility
processes since the prior inspection
Safety Equipment
Safety shoes, eye protection, hard hat and safety vest are required within manufacturing areas Hearing
protection is recommended
Latitude/Longitude Verification
The latitude and longitude coordinates were verified to be accurate as documented in IBEAM
Applicable Regulations
Applicable regulations listed in the air permit include Title 15A NCAC, Subchapter 2D 0202, 2D 05I5,
2D 0521, 2D 0535, 21) 0540, 21) 1100, 2D 1806, 2Q 0309 and 2Q 0711 This facility is not required
to implement a risk management plan under the federal regulation 40 CFR 68, Section 112(r) because
they do not produce, use or store any of the regulated chemicals in quantities above the threshold limits in
the rule, but the facility is subject to the General Duty Clause of the rule
Discussion
The facility produces batch concrete in a truck -mixing process This plant currently produces mainly a
3,000-4,500 psi strength concrete used in road construction and residential purposes The facility has four
outside bins containing the aggregates and sand, which are loaded by a front end loader These materials
are weighed and then dispensed in measured amounts from the bottom of each hopper onto a conveyor
that takes them to the weigh hopper Measured amounts of cement and flyash are dispensed from the
silos into the weigh hopper Water and designated amounts of additives are then combined prior to the
concrete being dropped into the mixing truck
After the initial interview, Mr Waegerle presented Mr Williams with bagfilter maintenance records and
monthly production records, which were found to be complete The bagfilter records indicated the
facility performs internal maintenance inspections on a monthly basis, with the most recent being
performed on April 4, 2016 Mr Waegerle stated the facility receives flyash approximately twice per
week (on average) and cement approximately eight times per week
The facility operates a cement storage silo (ES-1), flyash storage silo (ES-2), weigh hopper (ES-3) and a
truck loadout operation (ES-4) Particulate matter, from all four permitted sources, is controlled by a
central bagfilter (CD-1), which contains a total of 77 bags During the inspection, one mixing truck was
observed being loaded and the cement storage silo being filled Minimal dust was observed escaping the
loadout skirt as it was being disengaged No visible emissions were observed coming from the bagfilter
system during the cement transfer On this day, the plant was operating at approximately 45 cubic yards
per hour The loadDUt skirt appeared to be in good condition and fitted well to the trucks being loaded
with concrete The bagfilter (CD-1) is equipped with a magnehelic gauge, and was reading 4 9 inches of
water column This gauge is located near the bottom of the bagfilter and can only be read at the bagfilter
The facility operates two 1,000-gallon aboveground storage tanks used for additives in the concrete mix
Both tanks (IES-2, IES-3) contain Calcium Chloride Both tanks are exempt from permitting in
accordance with regulation 2Q 0 1 02(c)(1)(D)(iii)
The facility also operates a natural gas -fired water heater (IES-1) used to heat process water, mainly in the
winter months It is exempt from permitting in accordance with 2Q 0102(c)(2)(B)(ii), which exempts
natural gas -fired combustion sources with maximum hourly heat inputs less than 65 million Btu This
heater is located outside and adjacent to the truck loadout area
Permit Conditions/ Applicable Regulations
Condition A 2 contains the 2D 0202 requirements, which include submitting an emissions inventory at
least 90 days prior to the expiration date of the permit The facility's permit expires on December 31,
2016 and must include an emissions inventory for the 2015 calendar year Mr Williams discussed the
renewal process and emissions inventory with Mr Waegerle and compliance with this condition is
anticipated
Condition A-3 contains the 2D 0515 requirement associated with particulates from miscellaneous
industrial processes According to the facilities emissions table published in Permit Review R04,
emissions fall well below the allotted threshold for particulate matter Also, the emission sources,
baghouse, and connecting ductwork appeared to be in good condition during this inspection Compliance
with this condition is indicated
Condition A 4 contains the 2D 0521 requirement associated with the control of any visible emissions
from errussion sources that may be discharged from vents or stacks No visible emissions were observed
during this inspection Compliance with this condition is indicated
Condition A 5 contains the requirements for 2D 0535 which requires the facility to notify the DAQ of
any malfunctions associated with a source causing an excess of emissions lasting more than four hours
Mr Waegerle stated there had been no such incidents and that operations are halted immediately if there
are any malfunctions of any kind Compliance with this condition is indicated
Condition A 6 contains the requirements for 2D 0540 which requires the facility to control any fugitive
dust emissions that may travel beyond the property boundary and cause a complaint Approximately half
of the production area is paved and Mr Waegerle stated that all non -paved surfaces are wetted, using
water from the onsite retention pond, during periods of dry conditions No fugitive dust was observed
traveling beyond the property boundaries during this inspection Compliance with this condition is
indicated
Condition A 7 contains the requirements for 2D 1100 toxic air pollutant emissions limitation and
reporting requirements This condition limits the facility's emissions of arsenic & compounds to 0 07233
pounds per year and the total production of concrete to 150,000 cubic yards per calendar year They must
keep records of monthly concrete production on -site and submit annual reports containing the monthly
and yearly production summaries This condition also contains the requirements for the annual internal
inspection and maintenance of the bagfilter system, CD-1 Mr McDaniel provided Mr Williams with
records showing the most recent internal inspection had been performed on April 4, 2016 The last
annual report from the facility was received on January 29, 2016 listing the following production data

prohibition of odorous emissions No objectionable odors were present during this inspection Review of
the facility file showed that no odor complaints have been made Compliance with this condition is
indicated
Condition A 10 contains the requirements for 2Q 071 which requires the facility to limit the toxic air
pollutants (TAPS) in accordance with the Toxic Permit Emission Rates (TPERs) This condition requires
the facility to apply for a permit modification prior to exceeding the toxic permitted emission rates
(TPER) listed in regulation 2Q 0711 1 he facility's actual TA1' emissions, which were calculated using
the 2011 emissions inventory, are listed with their corresponding TPER limits in the following table, from
permit review R03, indicating compliance with this condition

permit an inspection of Industrial facilities under authority granted by City of Greensboro

of two parts a review of the facility files/records and an on -site facility inspection

We have received an application for coverage of an existing facility under NCG 14 from VCNA There will be significant
site rework and grading from how is existing The facility drains to an UT to South Buffalo Creek (class WS-V, NSW) in
Greensboro (Guilford County)
They have vehicle maintenance, but no haz waste They also have all 3 types of ready mix ww, but only aggregate
wetting will discharge directly to stormwater The other 2 types of ww will be directed to a recycle system that will
overflow beyond a 10-yr storm As proposed, it will not meet the 2T rules, so it will be covered as a ww discharge under
the NCG14 The ww recycle system is also partially within the 100-yrflood zone
Since it discharges to a water supply classified stream, I'll also have to be getting public water supply to sign -off on it
Does the WSRO have any concerns about issuing this facility a COC for this general permit?

This correspondence is In response to your letter of August 6, 2012 requesting a discharge
determination for process wastewater generated at the Prestige Concrete Products facility located at
3107 South Elm Eugene Street in Greensboro NC.
The City of Greensboro Sewer Use Ordinance prohibits the routine discharge of storm water to the
sanitary sewer system. This prohibition includes storm water discharges from sheet run-off,
discharges/overftows from storm water retention ponds and from other storm water impound devices.
Therefore, the wastewater generated at the facility from wetting ras4 material stockpiles that ultimately
flows to the site's stormwater retention pond is not acceptable for discharge to the City of Greensboro
Sanitary Sewer System.

VCNA Prestige Ready Mix owns and operates a ready mix concrete facility located at
3107 R3 S. Elm Eugene Street in Greensboro, North Carolina. A Notice of Intent (NOI)
for coverage under the National Pollutant Discharge Elimination System General Permit
Number NCG 140000 has been prepared for this facility (Attachment A).
A ready mix concrete batch plant, office/maintenance building, material storage areas,
closed -loop wastewater treatment system, and wet detention pond are located on the site.
The facility intends to demolish and reconstruct the existing closed -loop wastewater
treatment system; regrade the site; reconstruct the wet detention pond; and remove
underground storage tanks and ancillary equipment including pumps, piping and an
oiVwater separator. A recycle system will be used for treatment of wastewater
generated from vehicle and equipment cleaning and mixing drum cleaning. Wastewater
generated from wetting raw material stockpiles will be treated with stormwater prior to
direct discharge. Drawings prepared by Triad Design Group including the demolition plan,
grading and drainage plan, site plan and details are attached as required by item 10 of the
NOI.
To complete item 10 of the NOI, a narrative description of facility operations and a line
drawing of water flow are included as Attachment B. Also included in Attachment B is a
USGS map with the location of the facility marked. As part of the wastewater treatment
alternatives review, the City of Greensboro was contacted regarding acceptance of the
commingled raw material stockpile wastewater and stormwater. A determination from
the City of Greensboro that they will not accept commingled stormwater and wastewater
is included as Attachment C. The Stormwater Pollution Prevention Plan (SPPP)
prepared for this facility is included as Attachment D.

NARRATIVE DESCRIPTION, WATER FLOW, AND USCS MAP
GENERAL DESCRIPTION
PRESTIGE CONCRETE PRODUCTS
GREENSBORO, NORTH CAROLINA
VCNA Prestige Ready Mix operates a ready mix concrete production facility at 3107 R3 S.
Elm Eugene Street, Greensboro, North Carolina. This facility is located on 5.43 acres.
Ready mixed concrete containing water, cement, fly ash, aggregates and supplementary
additives is produced at this site.
Cement and fly ash are stored in silos. These materials are transported from the delivery
truck to the silos using a pneumatic system. The silos are each vented to a baghouse.
The bags are periodically shaken and collected material is returned to the silos.
Aggregate, consisting of sand and three types of stone, are stored in four concrete bins.
These bins are three sided concrete structures. The aggregate is delivered near the
appropriate bin by a dump truck and a front ender loader is used to move the material into
the bin. As needed in the concrete production process, aggregate is loaded into the front
end loader bucket and placed into a feeding hopper that discharges to an elevated
conveyor belt that uses a radial stacker to feed one of four elevated bins.
Supplementary additives are stored in seven plastic aboveground storage tanks located in
the chemical storage facility. The tanks are filled from a commercial transport truck using
fillports located within the storage facility. The storage facility has a grated floor which has
a discharge to a containment tank located under the floor.
A computerized control system is used to produce the required concrete mix. When a
concrete mix truck is ready to be filled it is parked under the weigh hopper. A shroud
surrounds the truck feeding hopper and emissions from the filling process are vented to
the baghouse.
To produce truck -mixed concrete, all of the ingredients are charged to and mixed in the
truck mixer. The process begins with the discharge of approximately ninety -percent of
the required water to the truck mixer. The required aggregate materials are then fed
from the batch plant to a scale followed by transport on a conveyor belt to the truck
mixer. Required amounts of cement and fly ash are weighted on a separate scale
system and pneumatically conveyed to the truck mixer. Liquid additives and the
remaining water are then pumped to the truck mixer. Prior to leaving the facility, the truck
is driven to the wastewater recycle system. The truck driver uses clean water from a
tank located on the truck to clean the truck feeding hopper. Wastewater generated from
this cleaning procedure is discharged to the wastewater recycle system.
The facility has a closed loop recycle system that treats process wastewater generated
from cleaning of external truck surfaces and mixing drum cleaning. All cleaning will be
conducted in an area constructed to discharge to the closed loop recycle system.
Wastewater flows from the cleaning pad to a three celled settling basin, with overflow from
each cell flowing over a weir to the next cell. The water from the last cell is recycled and
used as mixing drum wastewater. Based on storm volume, there may be overflow during
storm events. If the system overflows, overflow will flow by overland flow to the facility
detention pond.
General Description

Residuals collected in the closed loop recycle system and the detention pond are
periodically removed to maintain settling capacity. Residuals will be removed from each
cell of the recycle system on a weekly basis and as needed from the detention pond.
Residuals will be placed on a concrete pad that drains to the closed loop recycle system.
Dry residuals will be sold.
The detention pond incorporates a pH adjustment system. This system will produce an
effluent with a pH within the acceptable range and pH adjustment will also produce
precipitation of some component of the dissolved and suspended solids.
The facility stores sand and aggregate material in four three sided uncovered concrete
bins. In the summer these materials are sprayed with water to reduce their temperature.
Water from spraying the sand and aggregate piles will either evaporate or flow by
overland flow to the wet detention pond.
The facility has one building in which the vehicle and equipment maintenance facility, the
office, and the control room are located. There are oil storage containers located within
the vehicle and maintenance facility area and two additional aboveground diesel fuel
tanks are located in the vicinity of the concrete plant. Debris and trash are stored in one
dumpster for subsequent off -site disposal.

process for the NCG020000, NCG140000, and NCG240000 General Permits, The Stormwater Permitting Unit will be
revising and posting more specific information and guidance in the coming weeks.
All Raleigh needs with the application submittal is a site plan described below:
So, now our review of a new N01 from a NCG14 facility is just to review the Site Plan to be sure that they are correctly
identifying stormwaters vs. wastewaters, and that ali areas draining off the plant site are accounted far by the
applicant. We hove replaced the 'engineering review' required in previous rules (i.e., the previous basis for the issuance
of the AtQ with a cursory Site Plan review for just the very basics in a Site Plan instead of an assessment of the true
capabilities of their treatment system to meet the permit limits on wastewater discharges.

Yes, he has it right.
New state law passed in the summer of 2011 ( SL 2011-394) took away the previous authority in state law/rules for DWQ
to require an AtC for: industrial, wastewater facilities, that already have a discharge permit. AtCs may still be required
for municipal WWTPs, and our Construction Grants and Loans folks (sorry, can't recall their new name), still issue those
as appropriate for municipal WWTPs.
Point of DWQ implementation: In the summer of 2011, 1 argued internally in DWQ that the Session Law strictly states
this prohibition as pertaining to facilities that already have a discharge permit, and so DWQ could continue to require an
AtC from facilities that were not yet permitted, like o facility applying for coverage under NCG14 for the first time (lust
Exactly the Case you have now, l think). I was not successful internally with that argument in front of the new Director,
based on the perspective that we would be guilty of going around the intent of the new low if we took those actions
based on the fact that the legislature couldn't get the language exactly right, and consequently left a loophole you could
drive a truck through due to their ignorance of the programs we administer. (In calmer retrospect that was probably o
good decision up the chain, and I'm glad to lose that internal argument.) So, now our review of a new NOI from a NCG14
facility is just to review the Site Plan to be sure that they are correctly identifying stormwaters vs. wastewaters, and that
all areas draining off the plant site are accounted for by the applicant. We have replaced the 'engineering review'
required in previous rules (i.e., the previous basis for the issuance of the AtQ with a cursory Site Plan review far just the
very basics in a Site Plan instead of an assessment of the true capabilities of their treatment system to meet the permit
limits on wastewater discharges.

Dear Mr. Wolfe:
The City of Greensboro Stormwater Management Division has found that A B Prestige Ready
Mix, LLC, located at 3107 S. Elm -Eugene Street in Greensboro, NC, was in violation of the
City of Greensboro's Code Chapter 27, Article III Section 27-31(b), which states as follows:
• "it shall be unlawful for any person to discharge non-stormwater to any stormwater
conveyance..."
The violation was determined by Debbie Shoffner of Stormwater Management on August 12,
2008, following an on -site observation at 3028 Shady Lawn Drive, which revealed the
following condition(s):
A truck driver from AB Prestige Ready Mix was washing out the truck's chute and
allowing the wastewater to discharge down the curbline and into the stormwater
conveyance system.
To comply with Chapter 27 of the City of Greensboro Code of Ordinances, it will be
necessary to conduct the following corrective action(s)-
Provide this office with written notification before September 22, 2008, explaining
actions taken to clean up the affected area and restore it to its original existing
condition and actions taken to prevent future unlawful discharges to the storm
drainage system.
In accordance with the City of Greensboro Code of Ordinances, failure to comply with this
notice may result in a civil penalty of up to $5,000 per day. Each subsequent violation is

subject to incrementally increasing penalties of up to an additional $5,000 per day.
Furthermore, any penalties assessed against the City by the State as a result of the violation
will be added to the civil penalty.
Stormwater Management Division staff will conduct a follow-up inspection/investigation on
September 22, 2008, to ensure compliance with the aforementioned ordinance.
