On March 13, 2014, Sue White of this office performed a Stormwater Permit Inspection
at Forsyth Redi-Mix located at 100 Anderson Road, Rural Hall, NC in Forsyth County This
facility holds General Stormwater Permit No NCG140423 to discharge Stormwater from
activities associated with Ready Mix Concrete under the National Pollutant Discharge
Elimination System (NPDES)

The permit became effective July 1, 2011 and expires June 30, 2016
2 Records/Reports
During the inspection, it was determined that your facility has a developed Stormwater Pollution
Prevention Plan (SPPP) The SPPP was not organized in a manner that made the
information readily available It was discussed that the binder would be organized so
that employees and State Inspectors would be able to find the various components of the
plan
3 Facility Site Review
This site is a concrete ready mix plant with a recycle system for their wastewater The site is
well cared for and it is obvious that the owners and employees implement permit requirements
This facility has one outfall, which is identified in the SPPP
4Effluent/Receiving Waters
Stormwater from the property discharges to Beaver Dam Creek (Class C waters) in the
Yadkin/Pee Dee River Basin
5 Self -Monitoring Program
It was determined during the inspection that you have been performing the analytical self -
monitoring requirements of the General Permit for Stormwater Monitoring requirements for
qualitative monitoring at each stormwater discharge outfall and the wastewater discharge were
discussed Both types of stormwater monitoring must be done once per each 6 month period
and the wastewater discharge monitoring must be done quarterly using the date that the plant
was opened as your starting date The wastewater discharge and qualitative monitoring

should be done as soon as possible to get caught up with the requirements All
analytical monitoring results from stormwater and wastewater must be submitted to the
Raleigh address as shown on the forms

attached some photos of the pond located at 8094 Glade Street in Rural Hall The family that lives there is the Serbers
Julie Serber is who I mostly spoke to, but Marty Serber is who called in the complaint Their phone numbers are 969-
6652 and 407-6388 if you should want to contact them for whatever reason Julie sent me some photos at the following
link from when they first started documenting the stuff on Friday 2/21

Anyway, the facility is called Forsyth Redi-Mix (NCG140423), located at 100 Anderson Street in Rural Hall The owner is
Jason Speer, 336-345-3202 That permit was issued on 2/15/13, so their "grace period" for developing the SWPPP and
doing monitoring should be over by now I looked in BIMS and they only have one outfall that discharges to UT to
Beaverdam Creek

under the subject state - NPDES general permit This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215 1 and the Memorandum of
Agreement between North Carolina and the US Environmental Protection Agency dated
October 15, 2007 (or as subsequently amended)
Please note that NCG140000 contains effluent limitations for process wastewater
discharges (see Part IV, Section D) Wastewater discharges must meet the
requirements of these effluent limitations An exceedence of any of these limitations
is a violation of permit conditions Please also note that groundwater standards in 15A
NCAC 02L and 02T regulations must also be met for recycle systems or any discharges to
groundwater

notice to the Division of Water Quality The Division of Water Quality may require

Waste Sources
Speer Concrete Design, Inc intends to open a ready mixed concrete facility, using a
standard portable mixing plant to be installed onsite The company will operatE a small
fleet of transport trucks 'for distribution, which will also be housed onsite Domestic
wastewater from the office building will be collected and treated by the onsite septic
system (located along the eastern property line) 'Industnal-wastewater will.be' created
during typical plant operations,These sources include wetting of raw material stockpiles,
vdhictelequipmcnt clean ing„mixing drum cleaning, as well as stormwater runoff from the
site
Solid Waste Management
Solid waste, in the,form of excess or improperly mixed concrete, will be dumped onsite
and ali6wed to cure When,feasible, said excess concrete will be formed into precast
blocks, dividers, barriers, wheelstops, etc In other instances, cured concrete will be
ground and recycled as aggregate In no case shall raw, uncured concrete be dumped Into
the -wash pit,or outside of the watershed area which drains to the wash pit
Fluid Waste Management
Fluid waste -is largely a combination of clearung efforts and stormwater, both of which
result in runoff that picks up contaminants from the site Through the use of directional
grading and 'diversion berms, all runoff from the' -facility will be collected and transported
to the wash pit The wash pit is a three -chamber device where contaminated `water enters
through,the first chamber and travels through subsequent chambers via a series of screens
and filters I his system treats the runoff for Total Suspended Solids (TSS) and settleable
solids As a result, the quality of waters improved from one chamber to the next In
addition, the wash pit will be monitored for pH balance and treated with muriatic acid or
other chemical additives as necessary to control, the alkalinity
A pump and piping system located in the final chamber of the wash pit will allow water
to be,reused, for vehicle and equipment cleaning For cleaning of thermixing drums the
facility will use Delvo ESC or similar chemical additive to limit the volume of water
required I his alternative solution does not need to be emptied from the drum before the
next use, which means less runoff into the wash pit

During"unusually heavy rainfall events and periods of little water reuse it is possible for
the wash pit to fill to capacity A rectangular weir on the final chamber allows excess
F(treated) water to be released from the wash pit 'This water ishcollected by an,existing
ddiversibn berm and is transported to an existing detention basin (along the western
property line) Ihese existing features remam'in place as a secondary defense to ensure
no untreated wastewater is discharged from the site

accordance with the permit conditions This facility must, at all times, meet the requirements of
Chapter 3, "Air Quality Control"', of the Forsyth : ounty Code
