INTRODUCTION
On July 18, 2017, Charles Gerstell, NCDEQ Multimedia Inspector, contacted Mr Mark Walser, Owner/President, of
Shoaf Coal & Sand Co, Inc — Shoaf Concrete for an announced targeted multi -media compliance inspection The facility
was previously inspected on July 19, 2016 by Paul Williams, Environmental Specialist, and was found to be operating in
compliance at that time The facility is a concrete batching and truck -mix facility that currently operates Monday through
Friday, 7 00am to 4 00 pm and 50 weeks/year Mr Walser stated that production has remained steady since the prior
inspection and that no equipment has been added to the facility process in that time period Mr Walser also confirmed the
IBEAM contacts were accurate
SAFETY
Safety shoes, hearing protection and a safety vest is required on site
LATITUDE/LONGITUDE VERIFICATION
The latitude and longitude coordinates of the facility were verified and did not need to be updated
APPLICABLE REGULATIONS
Applicable regulations listed in the air permit include Title 15A NCAC, Subchapter 2D 0202, 2D 0515 2D 0521, 2D
0535, 2D 0540, 2D 0611, 2D 1806, 2Q 0309 and 2Q 0711 This facility is not required to implement a risk
management plan under the federal regulation 40 CFR 68, Section 1 12(r) because they do not produce, use or store any of
the regulated chemicals in quantities above the threshold limits in the rule The facility is also subject to the General Duty
requirements contained in the General Duty Clause
DISCUSSION
The facility produces batch concrete in a truck -mixing process The facility utilizes a front-end loader to move needed
aggregate and sand These materials are then carried by a conveyor to storage bins which distribute the proper amount of
material to the weigh hopper Measured amounts of cement are then dispensed from the cement silo to the weigh hopper
Designated amounts of water, cement, aggregates and sand are then dispensed into a mixing truck at the Truck Loadout
operation (TL-1) The facility maintains a single Flyash Storage Silo (Silo-2) and associated bagfilter (CD-2) that is not
operational at the present time Mr Walser confirmed that the facility does not use flyash in their concrete mix and has no
plans to operated Silo-2 or bagfilter CD-2 in the foreseeable future None of the permitted equipment was being operated
during this inspection
Particulate emissions from the cement silo (Silo-1) are controlled by bagfilter CD-1, while emissions from the weigh
hopper (W-1) are controlled by bagfilter CD-3 After the initial interview, Mr Walser provided bagfilter inspection and
maintenance records which appeared to be complete The bagfilter records indicated the facility performs internal
inspections every three to six months, with the most recent internal inspections being performed on April 18, 2017
The facility operates several exempt sources which include a waste oil -fired boiler (IES-1), 8,000-gallon aboveground
diesel fuel storage tank (IES-2) and a 3,500 gallon above -ground gasoline storage tank These exempt items were
observed during this inspection
PERMIT CONDITIONSIAPPLICABLE REGULATIONS
Condition A 2 contains the 2D 0202 requirements, which include submitting an emissions inventory at least 90 days
prior to the expiration date of the permit The facility's permit is set to expire on June 30, 2024 The next emissions
inventor will be due for CY2023 The renewal process and emissions inventory were discussed with Walser Compliance
with this condition is indicated
Condition A 3 contains the 2D 0515 requirement associated with particulates from miscellaneous industrial processes
According to the facilities emissions calculations within Permit Review ROI, emissions, with controls, are below the
allowable rates Also, the emission sources, baghouses, and associated ductwork appeared to be in good condition during
this inspection Compliance with this condition is indicated
Condition A 4 contains the 15A NCAC 2D 0521 visible emissions control requirement This facility is limited to 20%
opacity visible emissions, for sources manufactured after July I, 1971, when averaged over a six -minute penod This
condition applies to cement silo (Silo-1) No visible emissions were observed during this inspection, since the facility was
not in operation Compliance with this condition is anticipated
Condition A 5 contains the 15A NCAC 2D 0521 visible emissions control requirement This facility is limited to 40%
opacity visible emissions, for sources manufactured as of July 1, 1971, when averaged over a six -minute period This
condition applies to the fly ash silo (Silo-2), truck loadout operation (TL-1) and weigh hopper (W-1) No visible
emissions were observed during this inspection, since the facility was not in operation Compliance with this condition is
anticipated
Condition A 6 contains the requirements for 2D 0535 which requires the facility to notify the DAQ of any malfunctions
associated with a source causing an excess of emissions lasting more than four hours Mr Walser stated there had been
no such incidents since the previous inspection Compliance with this condition is indicated
Condition A 7 contains the 15A NCAC 2D 0540 fugitive dust control requirement No fugitive dust was observed
traveling beyond the property boundaries during this inspection The facility owns and operates a water truck that is used
to wet the unpaved areas during periods of dry conditions Water was being applied to unpaved upon arrival at the site to
control dust According to the DAQ IBEAM database, no complaints have been received regarding fugitive dust
emissions at this facility Compliance with this condition is indicated
Condition A 8 contains the requirements for 2D 0611 bagfilter maintenance and recordkeeping for particulate matter
emissions control The facility is required to perform, at a minimum, an annual (for each 12-month period following the
initial inspection) internal inspection of the bagfilter systems and perform periodic inspections and maintenance as
recommended by the equipment manufacturer The facility is required to record the results of all inspection and
maintenance activities in a logbook The logbook was found to be complete with records indicating that the most recent
internal inspections were performed, on bagfilters CD -I and CD-3 on April 18, 2017 Bagfilter CD-2 is not operated at
this time Mr Walser was reminded that if the facility decides to operate this device and associated fly ash silo, in the
future, I/M requirements will be necessary Compliance with this condition is indicated
Condition A 9 contains the requirements for 2D 1806 which requires the facility to control and prohibition of odorous
emissions No objectionable odors were observed during this inspection There are no odor complaints recorded in
IBEAM Compliance with this condition is indicated
Condition A 12 contains the 15A NCAC 2Q 0711 toxic air pollutant emissions limitation requirement The following
table, from permit review ROI, shows the various TPER's emitted by this facility and indicates compliance with this
condition

Condition A 10 contains the federal rule(s) applicable to sources exempted from air permitting requirements This
condition states that the 3,500-gallon gasoline storage tank (IES-3) is subject to 40 CFR 63, Subpart CCCCCC (6C) The
facility is required to provide documentation within 24-hours of being requested, that less than 10,000 gallons per month
of gasoline is used by the facility Mr Walser provided the inspector with records showing that the facility purchased
2,200-gallons of gasoline on September 29, 2016 and 2,600-gallons on July 12, 2017 indicating likely compliance with
this rule

Comment Since the previous Inspection. Mr Walser located a SPPP which was developed by Concrete

be updated to clearly reflect how acitvities are carried out on site under the current ownership
Other items to be updated are employee training and list of responsible parties A detailed site
map was provided, but it was recommended that the outfall be labeled with a designated
number Mr Walser explained that no significant spills had occurred over the past 3 years The
wall of the secondary containment area surrounding the diesel and gas storage tanks had been
damaged by equipment leaving a crack in the wall Some staining was observed on ground
outsld the containment area below the crack Please ensure that all soils are properly
remedlated and the damaged containment wall repaired

On July, 19, 2016 Paul Williams, Multi -Media Inspector with the North Carolina Department of
Environmental Quality visited the facility located at 500 E Depot Street in Mocksville, to
conduct a compliance inspection as required by your industrial stormwater permit
This facility is covered under General Permit No NCG 140421 which allows the discharge of
stormwater point source discharges associated with activities classified as Asphalt Paving
Mixtures to the surface waters of North Carolina
Accordingly, the following observations and violations were noted during NCDEQ's Multi -
Media inspection and subsequent file review

Stormwater Pollution Prevention Plan (SPPP)
The General Permit NCG 140000, Part 1I1, No I thru 10 requires the permittee to
develop and implement a SPPP that includes all of the information required in No I thru
10 Inspection revealed that this facility has not developed and maintained a Stormwater
Pollution Prevention Plan in accordance with permit requirements

qualitative monitoring twice per year following the established guidelines The facility
has not been monitoring per requirements Monitoring must be completed twice annually
and records must be kept with the SPPP for a minimum of five (5) years

General Permit NCG 140000, Part IV Section A requires the facility to conduct analytical
monitoring twice annually at each of the facilities stormwater outfalls No analytical
monitoring has been performed in accordance with the permit No records exist
Required Response
Accordingly, you are directed to respond to this letter in writing within 30 calendar days of
receipt of this Notice Your response should outline how the violations will be addressed and
should be sent to this office at the letterhead address and include the following
Develop a Stormwater Pollution Prevention Plan (SPPP) to include all information
required per NCG 140000 Part III, No 1 thru 10 (site overview, location map, narrative
description, siteplan, record of spills and leaks, secondary containment record, BMP
summary, spill prevention & response, preventative maintenance & good housekeeping,
facility inspection records, employee training records, responsible party contact
information, SPPP annual update and amendment records
2 Begin Qualitative Monitoring and provide twice annually Keep records in the SPPP
binder
3 Begin Analytical Monitoring per permit requirements defined in Part IV, Section A
Keep records in SPPP and maintain for five (5) year minimum
Thank you for your attention to this matter This Office is considering sending a
recommendation for enforcement to the Director of the DEMLR (Division of Energy, Minerals,
& Land Resources) regarding these issues and any future/continued violations that may be
encountered This office requires that the violations, as detailed above, be abated
immediately and properly resolved Environmental damage and/or failure to secure proper
authorizations have been documented on the subject tract as stated above Your efforts to
undertake activities to bring the subject site back into compliance is not an admission, rather it is
an action that must be taken in order to begin to resolve ongoing environmental issues

This letter will verity that 1 preformed a field survey of the Shoat Coal And Sand Co site
on February 21, 2012
The operation of this business is the making of concrete on site Materials, such as sand,
gravel, and granite are stored on site for the making of concrete
Upon field observation, I show no disposal of any hazard waste, fluid waste, or chemical
waste on site There was no evidence of waste materials in drainage pipes or in the
branch as shown on the survey Storm water drainage is the only substance observed
The wash pit area only has storage of water used for rinsing of concrete from the delivery
trucks This water evaporates with no order There is no leaking of water into other areas
on the branch

A "state stormwater" permit is what we call the post -construction permits (coastal, phase II, ORW/HQW) that regulate
the flow coming from impervious surfaces, not the NPDES Federal Permits that we issue for industrial stormwater
discharges Make sense? So, just to be sure, they don't olso need a Phase II post -construction permit or ORW/HQW
permit — right? Only the Federal NPDES permit
What is your sense that they are not ever discharging wastewater from their site? We can no longer review plans per
Session Law 2001-394, so the way it stands now we are Just going to issue a permit for discharging stormwater—
because that is what they have applied for From your site visit, do you believe that is correct?

We've received an NOI from Shoaf Coal and Sand Company Inc for their ready -mixed concrete site in
Mocksville (Davie County) for coverage under NCG14 (ready -mixed concrete permit) The site discharges
stormwater into a UT to Leonard Creek (C) The NOI is attached
I noticed that you had sent them a letter on October 4, 2011 stating they don't have a "state stormwater
permit" but the you went on to write about NCG14000, so I just wanted to check that they don't also need
to have a "state stormwater" permit to control the flow and TSS coming off the site, correct
They also indicated that they are not discharging any wastewater (at all) I'm not sure if this is realistic
but I'd like them to provide some additional information anyway I'm going to send him a letter and ask
him to provide some more information I'll cc you on that

Does the Winston-Salem Regional Office have any concerns about issuing this facility a COC for this
general permit) If we don't receive any objections, we'll issue the COC in 30 days

Question 11
Our understanding of the meaning of `actively wet aggregate piles" is to place a
sprinkler hose or similar device on stockpiles and completely saturate with water for
the purpose of cooling or to achieve uniform moisture content We do not perform
this procedure However, during dry conditions we fog a small amount of water onto
the surface of the sand piles to control dust as required by the permit issued to us by
the Division of Air Quahty Water penetration into the surface of the aggregate piles
is small, and there is no material runoff of any kind

Wastewater from concrete production cannot be disposed into a municipal sewer
system and treated at a wastewater treatment plant (WWTP) The cement present in
the wastewater would stick to the sides of the sewer pipe The cement would build
up along the pipe walls until the sewer line was completely blocked Therefore, it is
not possible for the municipal WWTP to accept our wastewater

1 Introduction
9 9 Materials Management Narrative
This operation mixes aggregates, water, cement, fly ash, and other
admixtures to produce ready mixed concrete, which is hauled off -site to
construction projects The aggregates are transported by truck to the plant,
stored in stockpiles located outside and fed into the plant via loader and
conveyors Cement and fly ash are transported to the plant by truck and
pneumatically unloaded into silos where they are protected from exposure to
rain Chemical admixtures are transported by truck and are stored in 275-gal
or 55-gal containers inside where they are protected from exposure from rain
12 Plant Operations
A description of plant operations follows
1 2 1 Concrete Production
Aggregates, cement, fly ash, water, and admixtures are carefully
measured to pre -determine proportions and then gravity fed into the
ready mixed truck mixer The truck mixer then turns at a mixing speed
for approximately 70 revolutions to thoroughly mix the concrete prior to
delivery
1 2 2 Truck Washing
After loading, the truck moves to a raise down location where a hose is
used to rinse off the exterior of the drum The driver then delivers the
concrete
1 2 3 Drum Washing
At the end of the day, concrete clinging to the inside of the drums must
be washed out The procedure is to add water, mix to rinse out drums
and discharge into a settling pond/pit

1 2 4 Returned Concrete
It is common to have small portions of concrete leftover when returning
from a project During cool weather this is often incorporated into the
next load This cannot be done for all loads Therefore, one of the
following procedures is used to dispose of returned concrete
• Construction of precast concrete blocks
• On -site paving at the plant
• Discharged at pre -arranged locations off -site
• Wash out small loads at the wash pit
1 2 5 Wastewater Management
Wash water is discharged into a settling pit where solids settle out, are
removed to dry, and hauled off

3 Storm Water Management Plan
3 1 Feasibility of Modifying Operations
Aggregates used at this operation are stockpiled outside where they are
exposed to rain It is judged to be impractical to cover these stockpiles Since
the aggregates are all processed with water and required to contain less than
2% by weight passing the #200 sieve, there is a minimal contribution of
sediments from this source
3.2 Secondary Containment
Admixtures at this plant are not currently located within a secondary
containment structure

3 3 Best Management Practices (BMP)
Best management practices are physical, structural or management practices
that prevent or reduce pollution of waters The following BMP's are in effect at
this location
3 3 1 Drivers are assigned a specific parking location They must report any
fuel, hydraulic, or other fluid leaks to the plant lead man Spills are to
be cleaned up immediately and clean up materials disposed of
properly The leak is to be reported to the plant lead man for
correction
3 3 2 Drivers are not permitted to leave their trucks while fueling This will
minimize spills in the event of a malfunction of the fueling system
3 3 3 A log will be maintained in the office for any service or maintenance
performed on the dust collection system The air quality permit number
for this plant is 10193R00

4 3 Spill Prevention & Countermeasure Plan
The best defense for spill prevention is training and awareness by all plant
personnel This training should include the importance of operating
procedures to minimize potential spills and emergency responses to take in
the event of a spill
Liquid admixtures are stored in polypropylene totes inside the warehouse
These pose the greatest potential for a large spill either by rupture of the tank
or improper closure of the valve Any spills would be contained inside the
building The material would either be reclaimed or properly disposed
Diesel and hydraulic oil spills are also a concern with the high volume of
activity we have with our ready mixed truck, dump trucks, and tractor fleet
Any spills will be contained with sand and properly disposed The State will be
notified of any fuel/oil spill in excess of 25 gallons
Spills of other liquids are mostly likely to occur as a result of handling liquids
in 5-gal buckets or transferring liquids from 55-gal drums These spills should
be small enough to be easily manageable

5 Preventive Maintenance & Good Housekeeping
5 1 Preventive Maintenance
Preventive maintenance (PM) includes inspection and testing of production,
delivery and other plant facilities and equipment The purpose of PM is to
locate and correct equipment problems before they occur or cause major
problems PM will reduce truck breakdowns and plant shutdowns in addition
to reducing the nsk of polluting storm water The following lists items to be
checked and frequency
5 1 1 All trucks will be inspected daily for fuel, oil, and hydraulic leaks All
hoses and clamps will be inspected weekly
5 1 2 Dust collection systems, including bag houses will be inspected
annually as required by the air permit A log will be maintained to all
service and maintenance activities
5 1 3 Washout area, settling pond, and detention pond/pit will be maintained
in good operating condition
5 1 4 Secondary containment structures will be inspected semi-annually as
part of the Semi -Annual Inspection Report Any needed repairs will be
made immediately
5 1 5 Storm Dram Outfalls will be inspected semi-annually as part of the
Semi -Annual Inspection Report, signed and filed in Appendix B
5 2 Good Housekeeping
Good housekeeping means taking the time and effort to maintain a clean,
orderly, and safe environment in the work place These steps will help to
minimize any potential for storm water pollution in addition to promoting safety
and a sense of well-being at work
5 2 1 Trash and other waste will be picked up regularly A covered trash
receptacle will be strategically located outside and emptied on a
regular basis
5 2 2 Intenor floors will be kept clean and dry Brooms, mops, and dust pans
will be stored for convenient use
5 2 3 Empty drums and unused pallets will be disposed
5 24 The Good Housekeeping Checklist will be completed as part of the
Semi -Annual Inspection Report and retained in Appendix B
6 Training
Traimng of employees is essential to the success of any program The following
training will be conducted

The President of the Company shall be responsible for the overall coordination,
development, implementation, and revision of this plan Other responsible parties
and their responsibilities are listed in the Action Plan and the end of this plan
8 Plan Amendments
This plan will be amended whenever there is a change in design, construction,
operation, or maintenance that has a significant effect on the potential for the
discharge of pollutants to surface waters At a minimum the plan will be reviewed
and updated on an annual basis
9 Facility Inspections and Monitoring
Inspections and monitoring will be conducted as follows
9 1 Plant Lead Man
The Plant Lead Man will conduct visual inspections and monitoring of outfalls
as follows

9 1 1 Each outfall is identified on the site plan
9 1 2 Each outfall must be observed twice a year, once in the spring (April
through June) and once to the fall (September through November)
The Visual Monitoring Report forms should be filled out as part of the
Semi -Annual Inspection Report and retained in Appendix B
9 1 3 Each outfall must be sampled once a year during a representative
storm event unless representative status has been approved The total
suspended solids, pH, oil & grease, total lead, and detergents are the
test made and have target values A detailed worksheet for obtaining
and testing storm water samples is located in Appendix C
9 1 4 All outfalls that have a dry weather discharge must be sampled once a
year
10 Implementation
Implementation of this plan will include the required documentation of all monitoring,
measurements, inspections, maintenance activities, and training provided to
employees A copy of all sampling data and activities to implement BMP s will also
be maintained This information will be kept in Appendix B and maintained for a
minimum of five years An Action List summarizing required actions, frequency, and
who is responsible follows

discharge of stormwater into an unnamed tributary to Leonard Creek, class C" waters in the Yadkin -Pee
Dee River Basin The Mocksville operation falls under the Phase 1 permitting requirements established in
1990 for stormwater discharges from ready -mixed concrete facilities
Specifically, coverage under stormwater permit NCG140000 is applicable to all owners having
stormwater and/or process wastewater discharges associated with establishments primarily engaged in
ready -mixed concrete activities covered under Standard Industrial Classification (SIC) 3272 Continuing
to operate without a valid permit could subject Shoaf Concrete to enforcement action with the possible
assessment of a civil penalty of up to $25,000 per day, per violation
To comply with the above requirement Shoaf Concrete needs to apply for the above permit Attached is
the required application Please send the completed application and supporting information along with
the $100 application fee to the Raleigh address listed on page 7 of the application on or before November
18, 2011 A Technical Bulletin that gives an overview of the stormwater permit is also attached
