Quality received your request to rescind your coverage under Certificate of Coverage Number NCG140238 In accordance with your request, Certificate of Coverage Number NCG140238 is rescinded effective immediately Operating a treatment facility, discharging wastewater or discharging specific types of stormwater to waters of the State without valid coverage under an NPDES permit will subject the responsible party to a civil penalty of up to $25,000 per day It is the intention of DWQ that enforcement proceedings will occur for persons that have voluntarily relinquished permit coverage when, in fact, continuing permit coverage was necessary If, in retrospect, you feel the site still requires permit coverage, you should notify this office, immediately Furthermore, if in the future you wish to again discharge to the State's surface waters, you must first apply for and receive a new NPDES permit If the facility is in the process of being sold, you will be performing a public service if you 2010, I'Alke Mickey of this office met with you and Steve Naylor to perform a General Stormwater Permit Inspection at the Concrete Supply Company facility in Mocksville, NC The inspection found the facility to be in compliance vN ith the state issued storrnwater permit This inspection evaluated the five (5) areas designated on the attached inspection form Observations from each area are addressed below 1 Permit Concrete Supply Company holds Certificate of Coverage 9 NCG14O358 for the discharge of stormnater and process wastewater from the site The permit expires next on June 30, 2011 2 Records/Reports Part II, Section A of the permit requires the development of a Storm-,Nater Pollution Prevention Plan (SPPP) A copy of the SPPP was reviewed during the inspection This plan was found to be thorough and complete and addressed all items as outlined by the permit 3 Facility Site Review Observations at the site found the facility to be clean and well managed No concerns or problems were noted A single 10,000 gallon diesel tank is located inside a concrete block containment structure I he handle to the release valve is removed once draining is complete 4 Efflucnt/Receiving Waters The SPPP site map identified one stortmvater/wastewater outfall for the, facility located at the overflow spillway of the wet detention basin All stormwater and process wastewater is routed through the basin The mixing drum cleanout wastewater is routed through a recycle. pit ahead of the basin I he pH of recycle, pit wastewater can be adjusted prior to pumping to the wet detention basin The pit also receives stormv�ater off the site Excessive rainfall can cause the pit to overflow into the basin without pH adjustment Stormwater from the property eventually reaches an unnamed tributary to Page #2 5 Monitoring Prop -ram Part 1I, Section B of the permit requires the collection of annual analytical samples of stormwater runoff from the site Part II, Section C of the permit requires the collection of annual analytical samples of the following process wastewaters generated at the site 1) vehicle and equipment cleaning, 2) recycle system overflow, 3) raw material stockpile wetting and, 4) mixing drum cleanout Since all stormwater and process wastewater are routed through the wet detention basin prior to discharge, a single sample can be collected from the overflow spillway of the wet detention to satisfy the above requirements As you are aware, all analytical monitoring results must be submitted to the Division on form SWU-241-080109 no later than 30 days from the date the facility receives the sampling data from the laboratory It was noted that a pH value of 9 3 on 219/10 exceeded the cutoff concentration of 9 0 This was due to excessive rainfall this winter that caused the pit to overflow into the basin w ithout pH adjustment The monitoring requirement (Table 3) specific for on -site vehicle maintenance is not required since the facility uses < 55 gallons of new motor oil when averaged over the calendar year Routine truck maintenance occurs at the main shop in Charlotte Part 11, Section F of the permit requires that qualitative (visual) monitoring be performed and documented semi-annually of the combined stonnwater/process wastewater discharge This monitoring was last performed on 1113109 and 2/9/10 The qualitative records do not need to be
