As we discussed, the Hamlet plant "NCG140343" failed to sample for TSS and SS during their fourth quarter process water
sample. To correct this mistake, the Hamlet plant will sample twice during the first quarter of 2015. One to make up for the error in
the the 4th quarter of 2014, and the other to satisfy the 2015 first quarter requirement.

Dear Mr. Tapia:
On June 26, 2013, Jenny Graznak of this office performed a Compliance Evaluation
Inspection at the Cemex facility located at 2250 Hatchery Road in Burlington, North Carolina. This
facility holds General Permit No. NCG140342 under the National Pollutant Discharge Elimination
System (NPDES) to discharge Stormwater and Wastewater from Ready Mix Operations. You and
Mike Miller were present for the inspection. This type of inspection consists of two basic parts: an in -
office review of facility files and self -monitoring data and an on -site inspection of the facility's
discharge outfalls. This particular inspection evaluated five (5) areas, and observations from each
area are addressed below:
1. Permit
The permit became effective July 1, 2011 and will expire June 30, 2016. A complete copy of the
current permit was available for review during the inspection.
22.Records/Reports
All required records and reports were available for review during the inspection. Included in these
reports are monthly facility inspection forms, secondary containment discharge reports, and a history
of significant spills. The plant manager visually observes accumulated stormwater in the secondary
containment areas prior to release; and records of these inspections and releases are maintained
within the facility's Stormwater Pollution Prevention Plan (SPPP) for the site. According to the SPPP,
Employee Training occurs on an annual basis. The last training occurred on October 11, 2012.
3. Facility Site Review
Process water from vehicle and equipment cleaning areas drains to wastewater pits on site.
Sediment and silt settle to the bottom of the pits, which are routinely cleaned out. The sediment from
the pits is moved to drying beds and then sent to a landfill or distributed to local landowners to use
for fill material.
Materials stored outside include piles of sand and gravel, additive materials used for mixing
concrete, diesel fuel, motor oil, and cleaning chemicals. The tanks all have secondary containment.
Used oil is kept in barrels in a storage shed.

A sprinkler is used to keep the gravel piles wet according to NC Department of Transportation
specifications. The water that drains from -the gravel piles is considered process wastewater and
does drain offsite via the outfall.
4. Effluent/Receiving-Waters
The facility has one discharge outfall. Stormwater and wastewater from the property discharge to
Gum Creek (Class C; NSW) in the Cape Fear River Basin.
It was noted during the inspection that the property owner (CEMEX leases the property) has been
re -grading some areas. This could impact drainage on the site and could even create a second
outfall depending on the final grade. Please keep a close eye on this situation. You may want to
apply for Representative Outfall Status if another outfall is created from the discharge from the
wetted gravel pile.
5. Self -Monitoring Program
Due to the process wastewater generated on site associated with the wetting of raw material
stockpiles, this facility is required to perform quarterly analytical monitoring for process wastewater.
The process wastewater comingles with stormwater prior to discharge. Therefore, sampling the
combined discharge meets the monitoring requirements of this permit. The facility is also required to
perform the qualitative monitoring requirements of the permit. An on -site review of the monitoring
data during the inspection showed that all parameters were evaluated per the specifications of the
permit. All sampling results were within the effluent limitations set forth in the permit.

herewith the subject certificate of coverage to discharge under the subject state - NPDES general permit. This permit is
issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the Memorandum of Agreement
between North Carolina and the US Environmental Protection agency dated December 6, 1983.
The stormwater permit you are receiving, NCG140000, does not permit wasting concrete, dumping excess
concrete, directly into storm sewer outfalls or into waters of the state. Any facility which either dumps excess concrete or
washes excess concrete into storm sewers or waters of the state will be operating in direct violation to both the terms of
this permit and the North Carolina General Statutes. Such a discharge shall be considered an illegal discharge and may
subject the owner to enforcement actions in accordance with North Carolina General Statutes 143-215.6A
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to
you, you have the right to request an individual permit by submitting an individual permit application. Unless such
demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division of Water
Quality. The Division of Water Quality may require modification or revocation and reissuance of the certificate of
coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the Division
of Water Quality or permits required by the Division of Land Resources, Coastal Area Management Act or any other
Federal or Local governmental permit that may be required.

sample To correct this mistake, the Hamlet plant will sample twice during the first quarter of 2015 One to make up for the error in

On June 26, 2013, Jenny Graznak of this office performed a Compliance Evaluation
Inspection at the Cemex facility located at 2250 Hatchery Road in Burlington, North Carolina This
facility holds General Permit No NCG140342 under the National Pollutant Discharge Elimination
System (NPDES) to discharge Stormwater and Wastewater from Ready Mix Operations You and
Mike Miller were present for the inspection This type of inspection consists of two basic parts an in -
office review of facility files and self -monitoring data and an on -site Inspection of the facility's
discharge outfalls This particular inspection evaluated five (5) areas, and observations from each
area are addressed below
1 Permit
The permit became effective July 1, 2011 and will expire June 30, 2016 A complete copy of the
current permit was available for review during the inspection
2 Records/Reports
All required records and reports were available for review during the inspection Included in these
reports are monthly facility inspection forms, secondary containment discharge reports, and a history
of significant spills The plant manager visually observes accumulated stormwater in the secondary
containment areas prior to release, and records of these inspections and releases are maintained
within the facility's Stormwater Pollution Prevention Plan (SPPP) for the site According to the SPPP,
Employee Training occurs on an annual basis The fast training occurred on October 11, 2012
3 Facility Site Review
Process water from vehicle and equipment cleaning areas drains to wastewater pits on site
Sediment and silt settle to the bottom of the pits, which are routinely cleaned out The sediment from
the pits is moved to drying beds and then sent to a landfill or distributed to local landowners to use
for fill material
Materials stored outside include piles of sand and gravel, additive materials used for mixing
concrete, diesel fuel, motor oil, and cleaning chemicals The tanks all have secondary containment
Used oil is kept in barrels in a storage shed

A sprinkler is used to keep the gravel piles wet according to NC Department of Transportation
specifications The water that drains from the gravel piles is considered process wastewater and
does dram offsite via the outfall
4 Effluent/Receiving Waters
The facility has one discharge outfall Stormwater and wastewater from the property discharge to
Gum Creek (Class C, NSW) in the Cape Fear River Basin
It was noted during the inspection that the property owner (CEMEX leases the property) has been
re -grading some areas This could impact drainage on the site and could even create a second
outfall depending on the final grade Please keep a close eye on this situation You may want to
apply for Representative Outfall Status if another outfall is created from the discharge from the
wetted gravel pile
5 Self -Monitoring Program
Due to the process wastewater generated on site associated with the wetting of raw material
stockpiles, this facility is required to perform quarterly analytical monitoring for process wastewater
The process wastewater comingles with stormwater prior to discharge Therefore, sampling the
combined discharge meets the monitoring requirements of this permit The facility is also required to
perform the qualitative monitoring requirements of the permit An on -site review of the monitoring
data during the inspection showed that all parameters were evaluated per the specifications of the
permit All sampling results were within the effluent limitations set forth in the permit

this permit and the North Carolina General Statutes Such a discharge shall be considered an illegal discharge and may
subject the owner to enforcement actions in accordance with North Carolina General Statutes 143-215 6A
If any parts measurement frequencies or sampling requirements contained in this permit are unacceptable to
you you have the right to request an individual permit by submitting an individual permit application Unless such
demand is made, this certificate of coverage shall be final and binding
