Introduction
On August 29, 2016, Mr Blair Palmer, Environmental Specialist and Multimedia Inspector, and Mr Chengqing Xiao,
Environmental Engineer, both of the WSRO DAQ contacted Mr Tony Hardison, Operations Manager, Mr Johnie
Alexander, Environmental Manager, both employees of Central Carolina Concrete, LLC for an unannounced targeted
compliance inspection This inspection was also part of the multimedia program The National Pollutant Discharge
Elimination System (NPDES), specifically the stormwater and wastewater program is part of the August 29, 2016
inspection, but that information is discussed in a different database and report known as the BasmWide information
Management System or "BIMS " The facility was previously inspected (for DAQ purposes only) on December 14, 2014
by Mr Paul Williams of WSRO and the facility appeared to be operating in compliance at that time This facility is a
concrete hatching and truck -mix facility that currently operates 8-9 hours a day, and 5 days/week, and up to 51
weeks/year Mr Hardison also stated that no equipment has been added to the facility processes since the last inspection
All IBEAM contacts were verified and no changes are needed at this time There appears to be no physical changes with
the plant or the equipment since the last DAQ inspection

Safety Equipment.
Safety shoes, eye and hearing protection, safety vest and a hard hat are required at this facility Inspectors should be aware
of truck traffic
Applicable Regulations•
According to permit Condition A 1 listed in Air Permit No 09230R04, the facility is subject to the following regulations
Title 15A North Carolina Administrative Code (NCAC), Subchapter 2D 0202,21) 0515,21) 0521, 2D 0535, 2D 0540,
2D 0611, 2D I I00, 2D 1806, 2Q 0309 and 2Q 0711 The facility is not subject to the RMP requirements of the 112(r)
program since it does not use or store any of the regulated chemicals in quantities above the threshold in the rule The
facility is only subject to the General Duty clause of the Section I I2(r) program
Discussion and Inspection
Central Carolina Concrete, LLC operates as truck mix concrete batch plant that operates two cement storage silos (ES-1A,
ES-4), fyash storage silo (ES-1 B), weigh hopper (ES-2) and truck loadout (ES-3) Cement storage silo ES-5 has been
removed from the site a few years ago and the facility has no plans of re -constructing this piece of equipment according to
plant personnel Particulate matter from these sources is controlled by dual bagfilter units (CDI, CD-2) Silos ES-lA and
ES- i B are essentially a single silo, which is split into two compartments, on housing gray cement and the other Fyash
The facility also has four aggregate bins that are transferred via a front-end loader fhe materials are then weighed and
dispensed, in measured amounts, via a conveyer that carries them to the weigh hopper Flyash and cement are then
dispensed from their silos into the weigh hopper Designated amounts of water and additives are then administered prior
to the concrete being dropped into a mixing truck via loadout operation ES-3
According to plant personnel, the company receives cement deliveries approximately up to once per day, while Fyash is
delivered one to two times per week There were no deliveries observed during this inspection Several concrete trucks
were observed being loaded with minimal dust being emitted from the truck loadout process (ES-3) of material being
loaded into the trucks There are two suction ports for the Ioadout and both appeared to working properly Also, the shute
and shrouding as part of any dust control for the loadout appeared to be in ok-good condition The facility drive areas are
covered in gravel During dry times the unpaved areas are wetted using one of the company's concrete trucks On the
day of inspection the facility was operating at a throughput rate of-110 cubic yards per hour The magnehelic gauge on
the large bagf her was reading 2 inches of water column The facility does monitor the gauge and they keep daily
readings in a logbook that supplements the internal inspections records
The propane -Fired bolter (I-B1) provides warmed water, to the cement mixture, during cooler periods of weather and it
was not operating during this inspection The three LPG 500-gallon storage tanks are used to provide fuel to the boiler
and the tanks are onsite, near the entrance/exit of the facility The facility currently has six admixture tanks as part of
(IST2) and they are located in a metal storage container adjacent to the permitted equipment Two of the tanks are not
installed, as IST2 currently lists eight tanks
The facility currently does not operate an emergency generator or have any diesel water pumps
PERYHT CONDITIONS -
Condition A 2 contains the 2D 0202 requirements, which include submitting an emissions inventory at least 90 days prior
to the expiration date of the permit The facility's permit expires on July 31, 2022 Central Carolina Concrete has
recently requested exemption under the new DAQ exemption rules. The request for this permitted facility is
currently being processed in the WSRO
Condition A 3 contains the 2D 0515 requirements associated with particulates from miscellaneous industrial processes
The allowable PM emission rate for silos ES -IA, ES-113, and ES-4, are each 36 4 lb PM/hr The allowable PM emission
rate for the weigh hopper, ES-2, and truck loadout, ES-3 are 412 lb PM/hr and 610 lb PM/hr, respectively With the
bagfilter having a control efficiency of 99+%, calculated emissions from ES -IA, and ES-4 are each 0 03 lb PM/hr, with
ES-113 at 0 23 lb PMJ1-Ir, ES-2 being 0 96 lb PM/hr, and ES-3 also at 0 96 lb PM/hr All of the ductwork (metal and/or
flexible plastic) associated with the emission sources appeared to be in fair -good condition Compliance with 2D 0515 is
indicated
Condition A 5 pertains to regulation 2D 0535 "Notification Requirement" The facility is to notify the DAQ director of
any source with excess emissions that last for more than four hours and that result from a malfunction, a breakdown of
process, or any other abnormal conditions No notifications have been received and plant personnel was unaware of any
issues Compliance is indicated
Condition A 6 (2D 0540) concerns the control of fugitive dust emissions DAQ personnel observed no fugitive dust
emissions near the facility boundary and no complaints have been received, indicating likely compliance Facility
personnel stated that the facility will use a trick to water the haul roads during summer months or dry conditions No dust
was observed off the plant's property No watering of the haul roads was necessary during the inspection due to the
recent rains The facility uses sprinkler for the aggregate to help with any possible dust, but it is mainly used so that the
aggregate is not to dry going into the concrete mix
Condition A 7 pertains to 2Q 1100, "Toxic Air Pollutant Emissions Limitation and Reporting Requirement," which
requires the facility to limit the toxic air pollutants (TAPS) released in accordance with the Toxic Permit Emission Rates
(TPERs) Central Carolina Concrete, LLC is limited in facility -wide of arsenic emissions to 0 0434 pounds/year The
facility is required to submit annual concrete production reports by January 30`h of each year and keep monthly records of
concrete production The facility is limited annually to 90,000 cubic yards of concrete production The facility last report
was received in the WSRO on January 29, 2016 (on -tune) and the facility reported a total 37,866 cubic yards of concrete
The facility was able to produce up to date records (including monthly) and production thus far in 2016 is a total of 28,286
cubic yards up thru August 29, 2016
Condition A 7 (per 2D 0611) also requires this facility to perform, at minimum, an annual internal inspection of the
bagfilter(s) and record all maintenance activities in a logbook to be made available to Division personnel Mr Alexander
provided the inspector with the bagfilter logbook showing the most recent internal inspection of both bagfilters occurred
December 31, 2015, indicating compliance No problems were noted in the logbook for either bagf[ter There area total
52 bags in CD-1 and two cartridge style filters in CD-2
Condition A 8 (2D 1806) concerns the control of odorous emissions Mr Palmer detected no objectionable odors during
the inspection nor have any complaints been received, indicating likely compliance
Condition A 9 pertains to North Carolina General Statute (NCGS) 143-215 108(e), which allows the Director to modify
and reissue the permit with additional emission controls and/or additional operational restrictions upon becoming aware of
any credible air emissions data not previously considered by the DAQ This is in order for the facility to demonstrate
compliance with any applicable regulation Currently, there is no cause for the DAQ to reissue the permit The factlity
appears to be in compliance with NCGS 143-215 108(c)
Condition A 10 pertains to 15A NCAC 2Q 0711, Toxic Air Pollutant Emissions Limitation Requirement This condition
requires the facility to obtain a permit prior to exceeding any of the listed TPER's In calendar year 2015, the facility
reported an annual production of 37,866 cubic yards which falls well below the maximum limit of 90,000 cubic yards
Compliance with this condition is expected Below is a table taken from the November 17, 2011 permit review

PERMIT DISCUSSION:
The facility's permit exemption request is currently being processed by Mr Chengging Xiao
COMPLIANCE HISTORY (LAST FIVE YEARS).
The facility was issued a Notice of Deficiency (NOD) on January 21, 2014 for failing to keep updated (over 12
months since last records) inspection records for the weigh hopper as a result of a January 9, 2014 DAQ
inspection
• No dust complaints have been logged against this facility based on file and [BEAM review
CONCLUSION:
Based upon observations of its emission sources, controls, and review of records, Central Carolina Concrete, LLC appears
to be in compliance with all applicable Air Quality Rules at the time of the August 29, 2016 inspection

inspection stated "no flow " Mr Palmer recommended that the facility add a statement that there
is no reall outfall and that all water is pumped back onto the higher portions of the haul roads

On March 13, 2012 a Compliance Evaluation Inspection was performed by George Smith, of this office Mr
Johnie Alexander, Environmental Manager, Mr Robert Crews, Stormwater Plan Maintenance, and Mr Jimmy
Chavis, Plant Manager were all present for the inspection The facility located at 3905 Liberty Road,
Greensboro
Records & Reports
The Stormwater Pollution Prevention Plan (SPPP) is kept in a 3-ring binder and is being implemented
• The Plan has a section containing good housekeeping and chemical storage forms that are used
to evaluate the site as defined in the SPPP

The analytical records show quarterly monitoring for pH, TSS, Settleable Solids, and Total Petroleum
Hydrocarbons (TPH) for the outfall
Qualitative semi -monitoring is performed at the outfall Rainfall amounts are recorded
Total Petroleum Hydrocarbons (TPH), otherwise known as Oil & Grease do not need to be sampled at this
facility because there is no Vehicle Maintenance Area
Please keep in mmd that all quarter sampling that is non -compliant twice in a row with Table 7 will institute
monthly monitoring
The facility is well maintained, monitoring is performed, and the SPPP is being implemented The staff is doing

On April 15, 2008, .fenny Graznak of this office performed a General Stormwater Permit
Inspection at the Central Carolina Concrete Plant #2 located at 3905 Liberty Road in
Greensboro, North Carolina This facility holds General Stormwater Permit No NCG160124
under the National Pollutant Discharge Elimination System (NPDES) to discharge Stormwater
from activities associated with Asphalt Paving Mixtures and Blocks Plant Manager Mike Davis
was present for the inspection This type of inspection consists of two basic parts a review of
facility files and self -monitoring data and an inspection of the facility's discharge points This
particular inspection evaluated five (5) areas, and observations from each area are addressed
below
1 Permit
The permit became effective August 1, 2004 and expires July 31, 2009
2 Records/Reports
As of the date of this letter, Stormwater records for this facility have not been made
available to the Division of Water Quality Your permit requires that the facility keep a
current Stormwater Pollution Prevention Plan including verification of annual employee
training in Stormwater pollution prevention, and retain analytical and visual monitoring
records Mr Davis, the Plant Manager does not have copies of this information at the
plant and therefore, could not present them to Ms Graznak during the inspection
3 Facility Site Review
The site includes an asphalt production area and an aggregate storage area
4 Effluent/Receivinq Waters
Stormwater from this site drains to Little Alamance Creek (Class WS-IV, NSW) The site has a
centrally located stormwater retention pond that, with the rest of the stormwater from the site,
drains to a ditch at the edge of the property From there, the ditch empties to a pond on the
adjoining property of Martin Marietta, where it is recycled with their process wastewater The
stormwater from your site must remain permitted by your current permit NCG160124 This
stormwater would not be lawfully covered under Martin Marietta's permit for Mining, since your
site is an asphalt plant and is involved in different activities with unique limits and requirements
from mining activities

5 Self -Monitoring Program
Your permit requires semi-annual qualitative and annual analytical monitoring at each
stormwater discharge outfall On -site vehicle maintenance monitoring requirements at the site
are waived, since these activities are performed off -site As of the date of this letter, records
of this monitoring have not been made available to the Division of Water Quality
You should note that this inspection report is being sent as a Notice of Deficiency for
failure to make records and reports available to Division of Water Quality upon request In order
to avoid a Notice of Violation, you should submit this information to our office for review before
April 30, 2008 In addition, it was noted that ownership of this plant was recently transferred to
Central Carolina Concrete from APAC However, our office's records do not show any evidence
of this change Please complete the enclosed Permit Name/Ownership Change Form and
submit it to the address specified on the form

concrete, directly into storm sewer outfalls or into waters of the state Any facility which either dumps excess concrete or
washes excess concrete into storm sewers or waters of the state will be operating in direct violation to both the terms of
this permit and the North Carolina General Statutes Such a discharge shall be considered an illegal discharge and may
subject the owner to enforcement actions in accordance with North Carolina General Statutes 143-215 6A
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to
you you have the right to request an individual permit by submitting an individual permit application Unless such
demand is made this certificate of coverage shall be final and binding
