In -,response to a complaint about stormwater runoff from your facility, and an inspection
of noncompliance on March 2, 2015, NCDENR-Land Quality Section staff conducted a
follow-up si#e inspection on April 8, 2015. The primary goal of this inspection was to
evaluate compliance with the conditions of the NCG140000 General Permit, and to
discuss any actions that have been or could -be taken to assure compliance with your
permit NCG140309.
At the time of this inspection the facility was found to be in compliance with your permit.
Please refer to the enclosed Compliance Inspection Report for additional comments,
observations, and recommendations made during the inspection. There are some
matters that require your attention to keep the site in compliance.
1. You were provided an Authorization to Construct a wastewater collection system
and we encourage you to proceed with these plans. You indicated that a closed -
loop recycle system is planned for this facility in the near future. Note that to be
defined as a truly closed -loop system with no monitoring requirements; the
system will have to meet the requirements of 15A NCAC 02T .1003 "Permitting
by Regulation." This requires that there is sufficient storage capacity to contain
the runoff from a 24-hour, 25-year storm event plus one foot freeboard; and that
the facility develops and maintains a spill control plan in the event of a

wastewater release. Please contact Ms. Beverly Price of the Division of Water
Resources, Asheville Region at (828) 296-4500 if you have any questions or
need more. information regarding the "27' requirements for this system. Please
provide our office a schedule for installation of this system.
2. Develop a routine maintenance schedule for keeping accumulated silt off of
Pond Road and out of the drainage ditch in front of your facility.
During this follow-up compliance inspection it was determined that the settling basins
and its outflow channel had been maintained, seeded and matted; a secondary berm
had been provided near the road to direct stormwater flows to the settling basins; and
monthly sampling of the stormwater and wastewater outfall has been initiated.
Please retain and append this letter to your Stormwater Pollution Prevention Plan
(SPPP) or permit. If you have any questions or comments concerning this letter, please

Inspection Summary:
This inspection was conducted as a follow-up inspection to the March 2, 2015 stormwater permit compliance Inspection.
The site was found to be in noncompliance during that inspection primarily due to the lack of maintenance of the settling
basins, the need for additional bean protection near the front of the site to direct stormwater flow to the settling basins, and
failure to provide quarterly monitoring of the wastewater discharge componenet of the site. During this follow-up inspection,
it was noted that these Issues were addressed.
The facil€ty was asked to provide a maintenance plan for keeping silt off of Pond Road by scraping current deposition from
the road and ditch, and then providing regular maintenance with a vacuum sweeper. It was noted that both SCM and other
entities contribute to the deposited silt along Pond Road and that when the road is repaired by DOT the ditchline needs to
be reworked to provide for better positive flow and drainage.

In response to a complaint about sormwater runoff from your facility, NCDENR-Land
Quality Section staff conducted a site Inspection on March 2, 2015., The primary goal of
this inspection was to evaluate compliance with the conditions of the NCG140000
General Permit, and to discuss any actions that have been or could be taken to assure
compliance with your permit NCG140309.
At the time of this inspection the facility was found to be in non-compliance with your
permit. There are several issues that need to be addressed. Please refer to the
enclosed Compliance Inspection Report for additional comments, observations, and
recommendations made during the inspection. In addition, there are some matters that
require your immediate attention to bring the site into compliance.
1. You must immediately clean out and maintain your stormwatertwastewater
settling basins to make them fully functional, as well as providing maintenance of
the ditched channel outlet from these basins. Current lack of maintenance has
contributed to stream bank erosion due to excessive outflows from these basins.

2. There is an area at the front comer of the property near the road that you are
directed to provide a berm or some other means of redirecting stormwater flows
to the settling basins. Currently this area is flowing to the outer berm and is
contributing to stream bank erosion.
3. You were provided an Authorization to Construct a wastewater collection system
and we encourage you to proceed with these plans. You indicated that a closed -
loop recycle system is planned for this facility in the' near future. Note that to be
defined as a truly closed -loop system with no monitoring requirements, the

4. Given that you currently have co -mingled wastewater and stormwater, you are
required to conduct quarterly wastewater monitoring of the outfall(s) of these
materials per your permit. Since your analytical sampling has not been
conducted at this frequency, your permit triggers you to now conduct monthly
sampling for a period of at least six months and report these results to this office
pursuant to Part IV, Section D of Permit No. NCG140000.
A follow-up compliance inspection will be conducted in approximately 30 days to assure
the settling basins and its outflow channel have been maintained; a secondary berm
has been provided near the road; and monthly sampling of the stormwater and
wastewater outfall has been initiated.

Inspection Summary:
1. The SWPPP is reasonably good, however more consistent updates and organization might be helpful.
2. The settling basins are clearly In need of malntenance and repairs. Because they no longer function at full holding
capacity, overflows are more frequent than they need to be and overflows are contributing to streambank erosion.
3. The area currently storing some sandy materials near the front of the property needs to have a berm or some other
measure installed to force stormwater flows from this area to the settling basins. Currently, flows from this area are
contributing to the streambank erosion because they bypass the settling basin.
4. An Authority to Construct a wastewater collection system was provided in July 2009. This system has not been built.
Therefore, the facility currently has co -mingled stormwater and wastewater going to the earthen pits and detention basins.
The co -mingled flows of truck wash water plus rainfall are regutated under the wastewater portion of the permit (see Part IV,
Section D). Therefore, quarterly monitoring is required. Quarterly monitoring has not been conducted. Under Part IV,
Section D, page 10, the lack of quarterly monitoring triggers monthly monitoring for a period of six months.
5. pH buffers were expired; pH buffers need to be kept current
7. There is no vehicle maintenance on site; however there is one 10,000 gallon doub"alled diesel tank on site. We
recommend providing a spill kit at the diesel tank in case of any leaks or spills.

By this letter DWQ is issuing an Authorization to Construct (ATC) wastewater
collection, treatment, and recycle facilities at your plant located at 80 Pond Road in
Asheville, North Carolina.
You are authorized to construct and install the facilities as described in your narrative
and calculations submitted, and as shown on the approved drawings attached to this letter;
including the specific elements:
A truck rinse and drum and chute washout pad;
Three, 20'x 56'x 6' deep settling chambers to receive truck rinse water, washout
waters, any incidental aggregate storage drainage, pit sludge drainage, and
stormwater generated from the small contributing drainage area;
1 • A pit sludge drying area;
0� • A carbon dioxide pH correction system;
• A pump and necessary piping to both return treated wastewater to the mix, and to
�9 reuse treated wastewater for truck rinse and drum and chute washout;
4 • A runoff diversion structure in front of pits #2 and #3;
• And all incidental mechanical, electrical, and structural construction and equipment
installation necessary to accomplish a wastewater collection, treatment, and recycle
system functioning as implied by the approved submittal package.
We are issuing this ATC based on your specific, stated design basis that the facilities
will not discharge wastewaters at the design storm (i.e. the 25-year, 24-hour rainfall). We
note that your current COC #NCG140309 authorizes only the discharge of stormwaters, not
wastewaters.
This ATC does not affect the legal requirements to obtain other permits which may
be required by the Division of Water Quality or permits required by the Division of Land
Resources, Coastal Area Management Act or any other federal or local governmental permit
that may be required,
Please notify DWQ's Asheville Regional Office, telephone number (828) 296-4500, at
least forty-eight (48) hours in advance of operation of the installed facilities so that an in -
place inspection can be made. Such notification to the regional supervisor shall be made

excluding State Holidays.
Upon completion of construction and prior to operation of this authorized facility, a
certification must be received from the designing professional engineer certifying
that the wastewater collection, treatment, and recycle facility has been installed in
accordance with the NPDES Permit, this Authorization to Construct, and the
approved plans and specifications. Please mail the certification (attached) to the
Stormwater Permitting Unit, 1617 Mail Service Center, Raleigh, NC 27699-1617.
Please note that any future construction, installation, or modification of wastewater
treatment facilities (including process wastewater recycle systems) will require an additional
Authorization to Construct (ATC) .prior to construction per 15A NCAC 2H .0138 & .0139. You
must submit, in triplicate, plans/specifications and design calculations, stamped and sealed
by a professional engineer, with a letter requesting an ATC to the Stormwater Permitting
Unit, 1617 Mail Service Center, Raleigh, NC .27699-1617.]
Please note that we are returning your check for $100, since the permit processing
and annual administering and compliance monitoring fees are strictly associated with
coverage under the General Permit, NCG140000 which you already have. There is no
additional fee for the ATC.
One (1) set of approved plans and specifications is being forwarded to you. Please
maintain a copy of the approved plans and specifications on file for the life of the facility. if
