Attached is the report resulting from the Compliance Evaluation Inspection
conducted at Southern Concrete Materials' Hayesville Plant on January 12, 2006 by Ms.
Starr Silvis and 1. It was our determination that the storm water and wash water was
being managed in such a way so as to comply with the limits as contained in
NCG140249.
I call to your attention the following items:
(1)Some minor regradeing of the yard in the area of the slump rack needs to occur in
order to insure that all wash water drains to the bermed area. Rob Lindsey and Tom
Strickland will attend to this matter.
(2)Plant records to not show evidence of annual employee training relating to storm water
pollution prevention. Rob Lindsey says such training has and will occur. Plant records
should reflect this activity.
(3)The Stormwater Pollution Prevention Plan did not contain a site plan.
(4) The diesel fuel storage tank could be better protected from truck traffic by the strategic
location of a couple of SCM's precast concrete blocks. Rob and Tom will take care of this
item.

In order to reduce the volume of drum wash water SCM uses the stabilizer DELVO and 25 gallons of water
at the end of each day. In the morning the drum is emptied into an earthen basin. Water in the earthen basin
was found to have a pH of 12.45.
All of the plant site receiving wash water or silo air pollution control water drains to a second bermed area
which can discharge to an unnamed tributary to Blair Fork. The earthen basin described in the first
paragraph also seeps into the bermed area. PH of water contained in the bermed area was 9.45_ No
evidence of a discharge was observed.
During the inspection it was suggested that minor regradeing of the site, in the area of the slump rack, be
done in order to insure that all wash water drain to the bermed area. Additionally, it was suggested that a
couple additional precast concrete blocks be placed near the diesel fuel tank to offer added protection from
fueling trucks. No site plan, such as I am accustomed to seeing as a part of SCM's Stormwater Pollution
Prevention Plans, was available. Plant records did not indicate staff training relating to storm water and
wash water management.

permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated
December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division
of Water Quality. The Division of Water Quality may require modification or revocation and reissuance of the
certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the

Act or any other Federal or Local governmental permit that may be required.
