The site is well maintained The ponds slopes should be maintained with a good grass cover and not bushes or weeds.
Maintain interior of ponds as well to reduce long term maintenance.
The previous inspection documented that the drum washout was conducted across the street at Adams (permit number
070204) This is a violation of thier permit. Please inform this office of the status of this practice and measures taken for

This is a well designed and constructed facility with basins for recycling truck wash water as makeup
water for product. The site is grassed and stormwater from most of the site is directed to a
stormwater treatment system. The exception is the area adjacent to the waste concrete storage pile.
A vegetated swale needs to be maintained to protect the adjacent wetlands. This site does not have a
site specific Pollution Prevention Plan and monitoring is not being done. A NOV will be sent.

between the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.

* A Stormwater Pollution Prevention Plan (SPPP) Certification Form. Completion of this form is required to
certify that you have developed and implemented the SPPP as per the conditions of the permit. This form
must be completed and returned to the Division within 30 days of receipt of this letter.

In accordance with your application for a stormwater discharge permit received on January 16,1997,
we are forwarding herewith the subject state - NPDFS permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215 .1 and the Memorandum of Agreement between
North Carolina and the US Environmental Protection agency dated December 6, 1983.
The stormwater permit you are receiving, NCG140000, does not permit wasting concrete, dumping
excess concrete, directly into storm sewer outfalls or into waters of the state. Any facility which either
dumps excess concrete or washes excess concrete into storm sewers or waters of the state will be operating
in direct violation to both the terms of this permit and the North Carolina General Statutes. Such a discharge
shall be considered an illec,al discharge and may subject the owner to enforcement actions in accordance with
North Carolina General Statutes 143-215.6A.
If any parts, measurement frequencies, or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings,
Post Office Drawer 27447, Raleigh, North Carolina 27611 -7447, Unless such demand is made, this
decision shall be final and binding.
.Please take notice this permit is not transferable. Part 11, B.2. addresses the requirements to be
followed in case of change in ownership or control of this discharge.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Environmental Management or permits required by the Division of Land Resources, Coastal
Area Management Act or any other Federal or Local governmental permit that may be required.
