Introduction
On June 12, 2017, Sue Homewood, Environmental Specialist of the DWR WSRO, contacted Mr Jacob Callahan of
Callahan Concrete Co, for an announced targeted multi -media compliance inspection The facility was previously
inspected on July 25, 2016 by Paul Williams, Environmental Specialist DAQ-WSRO, and was found to be operating in
compliance at that time The facility is a concrete batching and truck -mix facility that currently operates Monday through
Friday, 9 hours/day and 50 weeks/year Mr Callahan stated that production has increased since the prior inspection Mr
Callahan also confirmed the IBEAM contacts were accurate
Safety Equipment
Safety shoes, hardhat and a safety vest are required on site
Applicable Regulations
Applicable regulations listed in the air permit include Title 15A NCAC, Subchapter 2D 0202, 2D 0515, 2D 0521, 2D
0535, 2D 0540, 2D 0611, 2D 1806 and 2Q 0309 This facility is not required to implement a risk management plan
under the federal regulation 40 CFR 68, Section 1 12(r) because they do not produce, use or store any of the regulated
chemicals in quantities above the threshold limits in the rule The facility is also subject to the General Duty requirements
contained in the General Duty Clause

Discussion
This facility is a concrete batching and truck mix operation The facility was operating during this inspection The
processes begin as cement and flyash are pumped into the pneumatically loaded cement storage silo (ES-1) and
pneumatically loaded flyash storage silo (ES-3) respectively Mr Callahan stated that the facility receives cement
deliveries approximately 5 times per week and flyash approximately 1 5 — 2 times per month Emissions from ES-1 and
ES-2 are controlled by a single bagfilter (CD-3), which was operating when Ms Homewood arrived but not operating
during the inspection Cement is gravity fed to the weigh hopper (ES-4), which is located beneath cement silo ES-1
Flyash is auger fed, from the flyash silo (ES-3), to the weigh hopper The facility conveys aggregate from storage piles to
a three -compartment storage bin and is then loaded into the weigh hopper From the weigh hopper, the aggregate is
conveyed to the truck loadout operation (ES-5) Cement and flyash are gravity fed from the weigh hopper into the truck
loadout operation
The facility is permitted for a second pneumatically loaded cement storage silo (ES-2) This cement silo was not in
operation during this inspection Mr Callahan confirmed what was noted in the previous inspection, that this silo has not
been utilized in several years and there are no plans to operate this device in the foreseeable future
The following exempt items are listed on the permit and were observed during this inspection one 7,500 aboveground
diesel fuel storage tank (I ES-6), one 500-gallon motor oil storage tank (IES-7), and four concrete admixture storage tanks
(IES-8 — IES-1 1) It should be noted (as noted in the 2016 inspection) that the facility actually operates a total of 6
concrete admixture tanks and this should be updated during the next permit opening Also the facility is permitted for an
exempt natural gas -fired hot water heater (IES-12) The facility has removed this device because it became inoperable
and has installed a new natural gas -fired water heater as a replacement This new water heater was installed in January
2016 This new unit is an 840 000 BTU/hr unit manufactured by Pearson Systems The unit s information is as follows
Model 9 P-5-6W Burner # J091273-0 and Serial 901600-15 This unit should be added during the next permit opening
Permit Conditions/ Applicable Regulations
Condition A 2 contains the 2D 0202 requirements, which include submitting an emissions inventory at least 90 days
prior to the expiration date of the permit The facility's permit expires on March 31, 2023 and must include an emissions
inventory for the 2021 calendar year Ms Homewood discussed the renewal process and emissions inventory with Mr
Callahan and compliance with this condition is anticipated
Condition A 3 contains the 2D 0515 requirement associated with particulates from miscellaneous industrial processes
According to the facilities emissions calculations within Permit Review R08, emissions, with controls, are below the
allowable rates Also, the emission sources, baghouses, and associated ductwork appeared to be in good condition during
this inspection Compliance with this condition is indicated
Condition A 4 contains the 15A NCAC 2D 0521 visible emissions control requirement This facility is limited to 20%
opacity visible emissions, for sources manufactured after July 1, 1971, when averaged over a six -minute period No
visible emissions were observed during this inspection Compliance with this condition is anticipated
Condition A 5 contains the requirements for 2D 0535 which requires the facility to notify the DAQ of any malfunctions
associated with a source causing an excess of emissions lasting more than four hours Mr Callahan stated there had been
no such incidents since the prior inspection Compliance with this condition is indicated
Condition A 6 contains the 15A NCAC 21) 0540 fugitive dust control requirement No fugitive dust was observed
traveling beyond the property boundaries during this inspection The facility operates a Bobcat with an attached water
tank to wet the unpaved areas during periods of dry conditions Compliance with this condition is indicated
Condition A 7 contains the requirements for 2D 0611 bagfilter maintenance and recordkeeping for particulate matter
emissions control The facility is required to perform, at a minimum, an annual (for each 12-month period following the

initial inspection) internal inspection of the bagfilter system and perform periodic inspections and maintenance as
recommended by the equipment manufacturer The facility is required to record the results of all inspection and
maintenance activities in a logbook Mr Callahan keeps this information in a spreadsheet The spreadsheet was found to
be complete with records indicating that the most recent internal inspection was performed and filters were replaced on
bagfilter CD-3, on January 16, 2017 Compliance with this condition is indicated
Condition A 8 contains the requirements for 2D 1806 which requires the facility to control and prohibition of odorous
emissions No objectionable odors were present during this inspection Compliance with this condition is indicated
NSPSINESHAP
The facility does not appear to be subject to any NSPS/NESHAP regulations at this time
Facility Wide Emissions
Facility wide actual emissions, as reported by this facility for CY 2009 and CY 2014, are located in the report header
Permit Issues
The facility operates 6 cement admixture tanks, while the current permit list only 4 in the exempt activities section
As discussed previously, the facility has removed the exempt 0 0017 million BTUIhr natural gas -fired hot water heater
(IES-12) and has replaced it with an 840,000 BTUIhr natural gas -fired hot water heater manufactured by Pearson
Systems The new unit is model # P-5-6W, burner # J091273-0, and serial # 01600-15
Stack Testing
No stack testing has ever been conducted at this facility

No violations or deficiencies have been noted within the last five years
Conclusion
Based on visual observations and review of records, Callahan Concrete Co appears to be operating in compliance with all

I have spoken with Glen White regardingthe changes you need to make to your outfall location He was in agreement and he
indicated that you do not need approval to modify the Stormwater Pollutson Prevention Plan (SPPP) Outfall #2 should be
relocated to the outlet into the roadside ditch along the driveway You should choose a location in the ditch that captures both
sides of the driveway, so the lower side in the direction that water flows You should update all your documents accordingly and
start sampling at this location in the future, both for your qualitative and quantitative sampling Please let me know if you have

This letter is a response to our notice of deficiency NOD-2016-SP-0003 received July 28`h 2016 regarding
our noncompliance status of Permit #NCG140229 First our facility would like to apologize for our
oversight of these 2 violations of our permit Steps have been taken to bring our permit back to
compliance These violations have been taken very seriously by our company and will be abated
immediately to bring the facility back into compliance All these issues have been evaluated and
resolved
In reference to noncompliance of Qualitative Monitoring of storm water permit NCG140229 this facility
was unaware the monitoring should be done semiannually Our facility has records of monitoring of
outfalls once per year, nonetheless with the help of Paul Williams,) am aware these outfalls should be
monitored at least twice per year and will be done sequentially to resolve this noncompliance status
In reference to noncompliance of Analytical Monitoring twice per year this facility has recently taken
steps to bring Storm water Permit NCG 140229 to compliance Our facility had some difficulties finding
a testing facility The previously used facility in Clemmons had closed and we had not done our due
diligence in finding anew testing facility We have found anew facility in Kernersville and taken steps to
have our first Analytical test performed From this date forward these test will be performed
semiannually
Please accept Callahan Concrete Companies apology for our noncompliance to our permit It was
certainly an oversight and will be resolved immediately I appreciate the professional manner in which
Paul Williams brought these issues to my attention The observations and violations that were noted on
his visit have been brought into compliance of Storm water permit #NCG 140229

On July, 25, 2016 Paul Williams, Multi -Media Inspector with the North Carolina Department of
Environmental Quality visited the facility located at 3545 W US Hwy 64 in Lexington, to
conduct a compliance inspection as required by your industrial stormwater permit
This facility is covered under General Permit No NCG 140229 which allows the discharge of
stormwater point source discharges associated with a primarily engaged in production of Ready -
Mixed Concrete [SIC 3273] and like activities to the surface waters of North Carolina
The following observations and violations were noted during NCDEQ's Multi -Media inspection
and subsequent file review
Qualitative Monitoring)
General Permit NCG 140000, Part IV Section C requires the facility to conduct
qualitative monitoring twice per year following the established guidelines The facility
has not been monitoring per requirements Monitoring must be completed twice annually
and records must be kept with the SPPP for a minimum of five (5) years
2 Analytical Monitoring)
General Permit NCG 140000, Part IV Section A requires the facility to conduct analytical
monitoring twice annually at each of the facilities stormwater outfalls No analytical
monitoring has been performed in accordance with the permit since July 12, 2010

2 Begin Analytical Monitoring per permit requirements defined in NCG060000, Part IV,
Section A Keep records in SPPP and maintain for five (5) year minimum
Thank you for your attention to this matter 1 his Office is considering sending a
recommendation for enforcement to the Director of the DEMLR (Division of Energy, Minerals,
& Land Resources) regarding these issues and any future/continued violations that may be
encountered This office requires that the violations, as detailed above, be abated
immediately and properly resolved Environmental damage and/or failure to secure proper
authorizations have been documented on the subject tract as stated above Your efforts to
undertake activities to bring the subject site back into compliance is not an admission,, rather it is
an action that must be taken in order to begin to resolve ongoing environmental issues
Pursuant to G S 143-215 6A, these violations and any future violations are subject to a civil
penalty assessment of up to a maximum of $25,000 00 per day for each violation Your

On April 21, 2020, Jenifer Carter, from the Winston-Salem Regional Office of the Division of Water
Quality, performed a stormwater inspection at Gobble & Callahan, located on US Highway 64 in
Lexington, North Carolina The purpose of the inspections was to review compliance NN ith the National
Pollutant Discharge Elimination System (NPDES) General Stormwater Permit The following are the
findings from the subject inspection
• Ownership of the company changed recently, in March 2010
• A Stormwater Pollution Prevention Plan (SPPP) had not been developed, though was almost
completed at the tune this letter was written
• Qualitative monitoring has not been conducted and recorded in ac,c.ordance with permit acquirements
at two (2) discharge outfalls
• Analytical monitoring has not been conducted and recorded in accordance with the permit
requirements at either of two (2) discharge outfalls Outfall 41 drains an area where vehicle cleaning
takes place Outfall 42 is the overflow from a pit that receives process wastewater from the wetting of
stockpiles and mixing drum cleanout A list of labor atoi tes was pi ovided dui ing the inspection and a
lab has been contacted
• There were no residuals from spills or leaks observed on the ground, nor in or near surface waters
• Secondary containment around an above -ground diesel fuel tank had an open di ain at the bottom
This has since been fitted with a i elease valve and lock
• The pit receiving wastewater from the wetting of stockpiles and mixing drum clean out was almost
full It has since been clean out
• It was recommended that secondary containment be provided around an above -ground used oil tank

To complete the task, bN June 30, 2010, please
• Provide a copy of the completed SPPP to this office
• Provide a copy of the lab results for the first analytical monitoring events at each outfall This is
required annually Since it has never been done before, please have it done this year prior to June
30, 2010 If there is no discharge from the pit during this period, please submit a report anyway,
with the words `no discharge' written on it
• Provide a copy of the first semi-annual Qualitative Monitoring Report to this office by June 30,
2010 According to the permit, it must be conducted at the same time as the Analytical monitoring
once during the first year of the permit (each time it is renewed), and at anytime thereafter during
each semi-annual period It does not have to be performed during a rain event Since the pit does
not discharge regularly, please make sure the Qualitative Monitoring is performed at the same
time the Analytical Monitoring is done, if and Ahen a discharge does occur
• This letter does not supersede the requirement to sent copies of all analytical discharge monitoring
reports to address noted on the ieport form, so be sure your are doing that as well
Due to N our immediate correctn a actions taken, this office does not intend to pursue further
enforcement action at this time That being said, please be aNiare that any future "lolations are

On September 10, 2009, Sue White of this office conducted a compliance evaluation inspection at the
Gobble and Callahan Ready Mix Concrete site at 3545 West US Hwy 64 in Lexington During the inspection the
permit requirements were discussed There was not a copy of the current permit on site This permit was issued on
August I, 2004 and had expired on July 31, 2009 It was noted that the new permit had been applied for The
following permit violations were discovered during the interview
l To date no analytical monitoring has been performed This permit requires that analytical monitoring be
performed the first year that the permit is issued Parameters to be analyzed outlined in the tables listed in part I[
sections b and c of the permit If samples are within the cut-off concentrations specified in the appropriate tables,
then no sampling is needed until the last year before the permit expires If cut-off concentrations are exceeded, then
analytical monitoring must be performed annually during a representative storm event
2 Qualitative monitoring had not been done at the time of the inspection Qualitative monitoring is required
semi-annually with the initial one to occur during the first analytical monitoring event This is a visual inspection of
the outfall, which is outlined in Part 11, section f A good practice when performing the qualitative monitoring is to
photograph each outfall with a digital camera, utilizing the date stamp 'I his is easily stored in binder with any notes
on odor
3 There was no Stormwater Pollution Prevention Plan on -site during the interview Also required by the
permit iS a Stormwater Pollution Prevention Plan Contents of a Stormwater Pollution Prevention Plan are outlined
in detail in Part 11, section A, pages 1 3 of i I in the permit This information should be contained in a binder and
kept on -site at all times

forwarding herewith the subject certificate of coverage to discharge under the subject state - NPDES general
permit This permit is issued pursuant to the requirements of North Carolina General Statute 143-215 1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated
December 6, 1983
The stormwater permit you are receiving, NCG140000, does not permit wasting concrete, dumping excess
concrete, directly into storm sewer outfalls or into waters of the state Any facility which either dumps excess
concrete or washes excess concrete into storm sewers or waters of the state will be operating in direct violation to
both the terms of this permit and the North Carolina General Statutes Such a discharge shall be considered an
illegal discharge and may subject the owner to enforcement actions in accordance with North Carolina General
Statutes 143-215 6A
if any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to
you, you have the right to request an individual permit by submitting an individual permit application Unless
such demand is made, this certificate of coverage shall be final and binding
Please take notice that this certificate of coverage is not transferable except after notice to the Division of
Environmental Management The Division of Environmental Management may require modification or revocation
and reissuance of the certificate of coverage
