to the company in October (2010) to request that repairs be made tD the wastewater settling
basin to grout the walls within the serpentine -settling basin to eliminate short circuiting.

Comment: Two representatives from the Wilminaton Reaional Office went to the site for a aualitative
analyses of -the-discharge from_the settling basins and state stormwater pond. This facility had
been approved for a combined State Stormwater and wastewater treatment system for it's
concrete truck mixer washout wastewater and stormwater runoff. A sample of the discharge
was collected. Turbidity was run in the WiRO lab. Turbidity was 16 NTU, however, the facility
was not in operation at the tirne and the runoff was primarily from the influx of stormwater to the
stormwater pond. A follow up will be conducted when the facility is operating.

Dear Mr. Spence,
A compliance evaluation inspection was conducted at the Ready Mixed Concrete Facility in Leland on October
20, 2010. The purpose of the inspection was to determine compliance with the NPDES General Stormwater
Permit NCG140000. The facility is in significant compliance. A review -of the stormwater pollution prevention plan
and all supporting documents required by the plan as well as the NPDES Permit was conducted. The staff was
able to produce the majority of the documentation requested in a timely manner. An inspection of the facility
grounds was conducted. The settling basin that has been constructed for treatment of the wastewater from the
truck washdown area requires attention, The basin is constructed of large blocks that are not grouted allowing
wastewater to short-circuit the treatment unit. This situation must be remedied as soon as possible.
Please provide this office with a date for which this situation can be remedied. The quality of the discharge
should meet the benchmark cutoff values for total suspended solids (100 milligrams/liter). The characteristics of
the discharges from this settling basin should be able to meet the cutoff' concentrations once the functionality of
this settling basin has been addressed.

Comment: There are two outfalls from the property, one at the northwest corner and
one at the southwest corner. The northwest corner discharges truck washdown
wastewater from the settling basins. The settling basins were recently built (two years
ago). The storage capacity appears to be adequate, but the quality of the discharge is
somewhat turbid because the large concrete blocks used to construct the basin are not
grouted together. The cracks between the blocks allows wastewater to shortcircuit
through the settling basin. This needs to be remedied as soon as possible. The
discharge outfall does have some settled solids in the bottom of the outfall. Repairing
this basin to function properly will likely result in greatly improved discharge
characteristics for TSS.

Dear Mr. Godwin:
,On October 26, 2004, Mr, Ed Beck of this office visited the subject facility in response to a complaint of concrete
truck wash water being discharged to a ditch. Several areas of concern were observed. At the truck exit on the
northeastern side of the site, truck wash water had been directed to discharge along the edge of the concrete drive and
was observed entering the roadside ditch. This wash water should have been entering the settling basin intended for
` treatingitruck wash water. Heavy deposits of concrete solids were settled in the ditch as a result of this discharge. At the
front entrance, water was flowing in the ditch from the discharge of water from the settling basin that treats truck wash
water. The pH of the water was measured and determined to be 11.85 standard units. This is a violation of the permit
limits which state that the pH shall be not less than 6 standard units nor greater than 9 standard units.
y Another area of great concern is the site where waste concrete is being dumped onto the ground. A very large hill
,of concrete has developed from the activity and it extends into the waters of the adjacent quarry and fills the bordering
roadside ditch. This site must be rehabilitated, the concrete must be removed from the ditches, and the activity must be
done in a manner that does not result in concrete solids or contaminated stormwater leaving the site. 'This area should be
addressed in the Stormwater Pollution Prevention Plan with guidance for acceptable operation.
All truck wash water must be directed through a treatment system with sufficient capacity and performance to
produce an effluent quality that meets the limits contained in the permit. The existing settling basin system may not be
capable of consistently providing sufficient treatment for all truck wash water generated on the site. It is strongly
recommended that opportunities for recycling the wash water be explored.
Please submit a plan within ten days of receipt of this notice that contains a schedule for taking necessary actions

Your facility is currently covered for stormwater discharge under General Permit NCG140000. This permit will
expire on July 31, 2004. North Carolina General Statute 143.215.1(c) requires that an application for permit
coverage renewal be filed in a timely manner. A prior notice of renewal was sent for this site on December I,
2003. To date, no response has been received by the Division of Water Quality (DWQ). In order to assure your
continued coverage under the general permit, you must apply to DWQ for renewal of your permit coverage.
Enclosed you will find a general permit renewal application form. The application must be completed and returned
by April 19, 2004 in order to assure continued coverage under the general permit.
Failure to request renewal by April 9, 2004 can result in a civil assessment of at least $250.00. Larger penalties
may he assessed depending on the delinquency of the request. Discharge of stormwater from your facility without
coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and could
result in assessments of civil penalties of up to $10,000 per day.

* Five copies of the Analytical Monitoring form and five copies of the Qualitative Monitoring form
* A copy of a Technical Bulletin for the general permit which outlines changes in the permit, key requirements,

Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require
modification or revocation and reissuanee of the Certificate of Coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by DENR or relieve the permittee from responsibility
for compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order, judgment, or
decree.

forwarding herewith the subject certificate of coverage to discharge under the subject state - NPDES general
permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated
December 6, 1983.
The stormwater permit you are receiving, NCG 140000, does not permit wasting concrete, dumping excess
concrete, directly into storm sewer outfalls or into waters of the state. Any facility which either dumps excess
concrete or washes excess concrete into storm sewers or waters of the state will be operating in direct violation to
both the terms of this permit and the North Carolina General Statutes. Such a discharge shall be considered an
illegal discharge and may subject the owner to enforcement actions in accordance with North Carolina General
Statutes 143-215.6A
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to
you, you have the right to request an individual permit by submitting an individual permit application. Unless
such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division of
Environmental Management. The Division of Environmental Management may require modification or revocation
and reissuance of the certificate of coverage. r
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Environmental Management or permits required by the Division of Land Resources, Coastal Area
Management Act or any other Federal or Local governmental permit that may be required.
