Inspection Summary:
The inspection was conducted by Beverly Price of the Asheville Regional Office. Mr. Allan Langston was present and
assisted with the inspection.
The permit was renewed and became effective August 1, 2017.
There is no vehicle maintenance conducted at this site.
The records appeared to be complete.
Areas of concern:
The design/operation of the system at this facility was described during the last inspection but was not noted as an area of
concern. After further discussion/investigation, the Asheville Regional Office feels the current mode of operation is an area of
concern. All three types of wastewater (ww) are generated at this site: vehicle/equipment cleaning — external surfaces,
wetting of raw material stockpiles and mixing drum cleanout. This ww flows to either of two pits. The vehicle wash pit is
located immediately adjacent (less than 20 feet) to Scott's Creek. The raw materiallmixing drum ww pit is located
approximately 55 feet from Scott's Creek. The pits are not lined and essentially operate as infiltration basins with some
evaporation. The concern is due to the proximity of the unlined pits to Scott's Creek. The ARO would recommend that this
mode of operation be re-evaluated. We would also recommend pH monitoring in Scott's Creek up and downstream of the
facility in the interim. In addition, Section 6.2 Recycle Pit System Description in the Stormwater Pollution Prevention Plan
(SWPP) says that all process water generated at the site is directed to Flow into the recycle water system. Recycling is not
being conducted at this site. The description indicates multiple pits but also a multiple -bay concrete weir structure. The
SWPP should be updated to reflect the actual operation at this site.
There was an area next to the creek and below the truck wash pit that appeared to have an abnormal amount of concrete
residual, possibly from a truck wash event that did not go to the pit. Drivers should be reminded that truck washing should
be done in a manner to ensure the ww goes to one of the pits.
Compliance Issue:
Admixture Storage Tanks — There are 7 Admixture tanks totaling more than 1320 gallons that should have secondary
containment or double -walled tanks. The following was noted in the 2016 inspection... Inspection Summary Page 2: SCM
should begin working toward appropriate containment for this area.
No adverse impacts to Scott's Creek were noted at the time of inspection.

The inspection was conducted by Beverly Price and Jeff Menzel. Jeff Lamm, Jason Green, Darren Watson and Andy Elliott
with SCM were present and assisted with the inspection.
The permit was renewed 6130/15.
There is no vehicle maintenance conducted at this site.
The records appeared to be complete.
There are two pit areas on site, one (closest to Scott's Creek) for stormwater and one for truck wash. The stormwater that
collects in the pit closest to the creek, infiltrates into the ground. Most of the water flows to either of those two pits.
Stormwater from the entrance area of the plant flows to Scotts Creek. This is the area for Outfall 001 (stormwater
monitoring point).
One area of concern is related to the Admixture storage tanks. There are B tanks on site that do not have secondary
containment. SCM should begin working toward appropriate containment for this area.

Inspection Summary:
Susan Wilson met with Mike Hill and Ken Ewart (SCM) on -site, June 21, 2012. A brief site visit was conducted on May
30, 2012; this site visit was to follow-up and review documents/record keeping.
Some aspects of the SWPPP need to be updated (Jeff Lamm is aware of this). One of the pits nearest Scott's Creek
needs to be cleaned out regularly (and likely bermed on the side nearest Scott's Creek to provide more capacity); much of
the drainage flows to this area (exterior truck washing, loading area). Per Mike Hill, no discharge occurs from the other pit
(the truck wash pit).
Although located next to Scott's Creek - the majority of the site drains to the two pit areas. There is some runoff from the
entrance that sheet flows to Scott's Creek (where samples are taken).
SCM has a small water intake along Scott's Creek (with a concrete support structure located above ordinary high water
mark).
The site appeared to be well maintained and no adverse impacts to Scott's Creek were noted.

Water Quality (DWQ) is forwarding herewith the reissued stormwater general permit. This permit is reissued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.
The following information is included with your permit package:
* A copy of general stormwater permit NCG140000
* A Stormwater Pollution Prevention Plan (SPPP) Certification Form. Completion of this form is required to
certify that you have developed and implemented the SPPP as per the conditions of the permit. This form
must be completed and returned to the Division within 30 days of receipt of this letter.
DO NOT SEND the SPPP with the signed form.
* Five copies of the Analytical Monitoring form and five copies of the Qualitative Monitoring form
* A copy of a Technical Bulletin for the general permit which outlines changes in the permit, key requirements,
and addresses frequently asked questions
* A Certificate of Coverage
Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require
modification or revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by DENR or relieve the permittee from responsibility
for compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order, judgment, or
decree.

In accordance with your application for a stormwater discharge permit received on December 2, 1994, we
are forwarding herewith the subject certificate of coverage to discharge under the subject state - NPDES general
permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated
December 6, 1983.
The stormwater permit you are receiving, NCG 140000, does not permit wasting concrete, dumping excess
concrete, directly into storm sewer outfalls or into waters of the state. Any facility which either dumps excess
concrete or washes excess concrete into storm sewers or waters of the state will be operating in direct violation to
both the terms of this permit and the North Carolina General Statutes. Such a discharge shall be considered an
illegal discharge and may subject the owner to enforcement actions in accordance with North Carolina General
Statutes 143-215.6A
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to
you, you have the right to request an individual permit by submitting an individual permit application. Unless
such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division of
Environmental Management. The Division of Environmental Management may require modification or revocation
and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Environmental Management or permits required by the Division of Land Resources, Coastal Area
Management Act or any other Federal or Local governmental permit that may be required.
