Inspection Summary
The Andrews Plant is compact and neatly kept. Plant personnel have worked out how to operate the three basin wash
water/storm water treatment system. It is a CO2 PH control system that re -circulates waste water back to mix productions
and wash down. There has been no flow documented in the last six-month period.
Last inspection by Roy Davis in 2005 indicated that the secondary containment wall around the diesel storage tank was
badly deteriorated and needed to be replace with a plastic lined earthen dike or a poured concrete wall. This inspection
found that a new wall has been poured around the 10,000-gallon tank since the 2005 inspection.
Concrete Forms on site use Super Strip release agent only. It is composed minerallvegetable oils.
Add mix storage area is good. The cargo storage container needs to be spill proofed and there is one 1,500-gallon
container that needs secondary containment.

1) Location: Southern Concrete Materials, Inc., Andrews Plant is located at 50
Whitaker Lane in Andrews, NC, Cherokee County,
Directions: Take U.S. 74, 19, 129, West of Andrews. Stay on the by Pass. At the first
red light in Andrews turn right onto U.S Business 19 (KFC is on the corner). Turn
right at the next red light onto Whitaker Lane. The plant is on the left
2) Facility Overview: Southern Concrete Materials, Inc., Andrews Plant is a batch
concrete plant producing concrete for commercial use. This facility is permitted under
Air Permit No. 04028R10, effective from January 5, 2012, until December 31, 2016.
Last compliance inspection conducted on August 6, 2015 by Richard Morris. Permit
04028RIO was rescinded July 22, 2016.

Pre -inspection Conference: On August 16, 2016 Richard Morris met with the
compliance contact, Dough Hughes, Plant Manager, Jeff Lamm, Vice -President
Environmental Affairs, and Thomas Ledford, Area Manager. Based on my
conversation with them, we discussed the following:
a) Verified the contacts based on FACFINDER printout. Dough Hughes has
replaced Rob Lindsey as facility contact and plant manager.
b) I was there by appointment to conduct a multi -media inspection for air quality
and storm water permits.
c) This facility has just had their air permit rescinded and a full permit inspection
will not be conducted. I did discuss future visits and that appoints will be
made before visits along with requests of facility production.
Process Description: (AP-42)
Concrete is composed essentially of water, cement, sand (fine aggregate), and coarse
aggregate. Coarse aggregate may consist of gravel, crushed stone, or iron blast furnace
slag. Some specialty aggregate products could be either heavyweight aggregate (of
barite, magnetite, limonite, ilmenite, iron, or steel) or lightweight aggregate (with sintered
clay, shale, slate, diatomaceous shale, perlite, vermiculite, slag, pumice, cinders, or
sintered fly ash). Concrete batching plants store, convey, measure, and discharge these
constituents into trucks for transport to a job site. In some cases, concrete is prepared at a
building construction site or for the manufacture of concrete products such as pipes and
prefabricated construction parts. The raw materials can be delivered to a plant by rail,
truck, or barge. The cement is transferred to elevated storage silos pneumatically or by

bucket elevator. The sand and coarse aggregate are transferred to elevated bins by front
end loader, clam shell crane, belt conveyor, or bucket elevator. From these elevated bins;
the constituents are fed by gravity or screw conveyor to weigh hoppers, which combine
the proper amounts of each material. Truck mixed (transit mixed) concrete involves
approximately 75 percent of U. S. concrete batching plants. At these plants, sand,
aggregate, cement, and water are all gravity fed from the weigh hopper into the mixer
trucks. The concrete is mixed on the way to the site where the concrete is to be poured.
Central mix facilities (including shrink mixed) constitute the other one-fourth of the
industry. With these, concrete is mixed and then transferred to either an open bed dump
truck or an agitator truck for transport to the job site. Shrink mixed concrete is concrete
that is partially mixed at the central mix plant and then completely mixed in a truck mixer
on the way to the job site. Dry batching, with concrete mixed and hauled to the
construction site in dry form, is seldom, if ever, used.
Inspection:
The facility was operating this day, but no truck loadouts were scheduled till later
in the day.
I inspected the bagfilter condition and all ductwork and found the equipment in
good condition with no apparent holes or leaks.

did not observe any fugitive dust emissions during my inspection.
A.8 2D.0611 — Fabric Filter Requirements. In compliance. An annual internal
inspection is required each year. Last internal inspection was performed
on June 1, 2016. Also, a logbook documenting maintenance and repair
activities is required. I observed the logbook and found recent weekly
entries.

There are no reporting requirements in the current permit.

There have been no violations at this facility in the previous 5 years.
5) Stack Test Review:
Stack tests are not required at this facility.
6) 112R Status: Based on the facility's inventory, it was decided that they are not
subject to 112R reporting requirements
7) Comments and Compliance Statement:
Based on review of records and visual observations, this facility "appeared to be
operating in compliance" with the Air Quality standards and regulations at the time of
this inspection.

Dear Permittee:
In response to your renewal application for continued coverage under stormwater General Permit NCG140000
the Division of Water Quality (DWQ) is forwarding herewith the reissued General Permit. This permit is reissued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or
as subsequently amended).
The following information is included with your permit package:
• A new Certificate of Coverage
• A copy of General Permit NCG140000
• A copy of the Technical Bulletin for the General Permit
• Two copies of the Discharge Monitoring Report (DMR) Form for Stormwater
• Two copies of the DMR Form far Wastewater
• Two copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater and/or wastewater, and it specifies your obligations with
respect to discharge controls, management, monitoring, and record keeping. Please review the new permit to
familiarize yourself with all the changes in the reissued permit. tf your Certificate of Coverage (COC) restricts
authorization to discharge stormwater only (or other limited terms), you must contact the Stormwater Permitting
Unit if you would like to request a modification to your COC.
The first sample period of your permit begins July 1, 2011. However, because of the delayed mail -out of permit
packages, your facility is expected to attempt to sample during the first sampling period, and must comply with
sampling requirements beginning during the second sampling period. Your facility must sample a representative
storm event between January 1 and July 30, 2012 (for stormwafersampling), and/or sample wastewater between
October 1, 2011 and December 31, 2011 (or, if applicable, report "No Flow", as outlined in Part V, Section E).
Additionally, your facility has 6 months from the time of receipt of the permit to update SPPP to current standards.
The more significant changes in the General Permit since your last Certificate of Coverage include the following
(please note the names and numbers of sections have been changed from the previous permit revision):
Part II:
• Authorization to Construct (ATC) language was added to the permit and adapted to fit NCG140000.
Part III:
• The Stormwater Pollution Prevention Plan (SPPP) section has been updated to the most current
language of our permits. Some additional conditions specific to this industry have been added to the
SPPP language.
Part IV:
• Sections A & B: Revised provisions require stormwater discharges be sampled twice per year (instead of
annually), and sampling results shall be compared to new benchmark values. (The previous cut-off
concentrations have been removed).

• Sections A & 8: Revised provisions require the permittee to execute Tier One, Two & Three response
actions: Tier One requirements for a first benchmark exceedence, and/or Tier Two response for a second
consecutive benchmark exceedence, if applicable. Tier Two requires that the permittee institute monthly
monitoring (reported to the appropriate Regional Offices) instead of twice -per -year monitoring. Monthly
monitoring shall be done until three consecutive monitoring events show no benchmark exceedences.
• Sections A & 8: This permit now contains a provision that four exceedences of any benchmark will trigger
increased DWQ involvement in the permittee's stormwater management and control actions (Tier Three).
DWQ may direct the permittee to apply for an individual permit, institute specific monitoring, or may direct
the implementation or installation of specific stormwater control measures.
• Sections A & B: Failure to perform analytical stormwater monitoring requires that the permittee begin a
monthly sampling scheme for both industrial stormwater discharges and/or stormwater discharges from
VMA areas (where applicable). After 6 months of monthly sampling, the permittee may return to semi-
annual sampling, unless otherwise specified by DWQ.
• Sections A & B: A lower TSS benchmark of 50 mg/I for HQW, ORW, PNA and Tr Waters was added to
this permit for these more sensitive waters.
• Section B: The monitoring parameter Oil & Grease has been replaced by the parameter Total Petroleum
Hydrocarbons.
• Sections A, B & C: Inability to sample due to adverse weather must be reported in the SPPP.
• Sections A, B & C: The terminology "Representative Storm Event" has been replaced by "Measurable
Storm Event" in response to comments from industry groups. A measurable storm event is a storm event
that results in an actual discharge from the permitted site outfall. The previous measurable storm event
must have been at least 72 hours prior, unless the permittee is able to document that a shorter interval is
representative for local storm events during the sampling period, and obtains written approval from the
local DWQ Regional Office.
• Section C: Qualitative monitoring must now be conducted semi-annually and is tied to the same
measurable storm event as the analytical monitoring.
• Section C: If the permittee fails to respond effectively to problems identified by qualitative monitoring, the
permittee may be required by DWQ to perform corrective action.
• Section D: This section has now been consolidated to one section with one set of combined tables for all
types of authorized wastewater discharges.
• Section D: The wastewater analytical monitoring schedule has been set to quarterly.
• Section D: TSS limits have been revised for PNA, Trout and HQW waters per rule requirements in 15A
NCAC 02B .0224.
• Section D: 7Q10-based flow limits have been set for PNA waters per regulations in 15A NCAC 02B.
• Section D: TPH monitoring requirements have been added to wastewater monitoring when stormwater
runoff from VMA areas commingles with wastewater.
• Section D: This wording in the permit has been removed: "For facilities which route stormwater and all
process wastewater to a recycle system, no analytical monitoring of process wastewater discharges is
required if that recycle system discharges as a result of a 10-year 24-hour (or larger) precipitation event."
Part V:
• Section A: For existing facilities previously permitted and applying for renewal under this General Permit:
New elements of the Stormwater Pollution Prevention Plan for this permit renewal shall be developed and
implemented within 6 months of the effective date of the initial Certificate of Coverage issued pursuant to
this General Permit and updated thereafter on an annual basis.
Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit does
not affect the legal requirements to obtain other permits which may be required by DENR, nor does it relieve the
permittee from responsibility for compliance with any other applicable federal, state, or local law, rule, standard,
ordinance, order, judgment, or decree. If you have any questions regarding this permit package please contact

This certificate of coverage shall become effective August 1, 2011.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

Select the third and fourth tabs in the center of the page to get the permit text for NCG14, and the monitoring forms for
NCG14.
Our database doesn't show any difference between the permits you received, and this one that you didn't, so I'm not
sure what happened since the address looks the same. As far as I know, all the NCG14 renewals went out of here on the
same day in one large batch.

Dear Mr. Lamm:
Attached you will find the report resulting from my November 9, 2005 inspection of
Southern Concrete Materials (SCM) Andrews Plant. The plant is compact and neatly kept.
At the time of my inspection several inches of freeboard were observed in the settling
basin. It was my impression that the basin would overflow only during storm events.
The cinder block wall providing secondary containment around the diesel fuel tank
is badly deteriorated and needs immediate replacement. It seems logical that you would
want to build a poured concrete wall rather that a plastic lined earthen dike. Achieving a
liquid tight wall using blocks is very difficult. You will have to decide whether you wish to
remove storm water from inside the containment structure by pumping or opening a
capped drain pipe.
I appreciate the fine job being done at the Andrews Plant. Should you need to

Comment: At the time of the inspection the settling basins had several inches of

treated water is used for hatching and end of day drum washing. it is my impression that

be doing a good job of operating the basins so as to minimize overflows.

Water Quality (DWQ) is forwarding herewith the reissued stormwater general permit. This permit is reissued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.
The following information is included with your permit package:
* A copy of general stormwater permit NCG140000
* A Stormwater Pollution Prevention Plan (SPPP) Certification Form. Completion of this form is required to
certify that you have developed and implemented the SPPP as per the conditions of the permit. This form
must be completed and returned to the Division within 30 days of receipt of this letter.
DO NOT SEND the SPPP with the signed form.
* Five copies of the Analytical Monitoring form and five copies of the Qualitative Monitoring form
* A copy of a Technical Bulletin for the general permit which outlines changes in the permit, key requirements,
and addresses frequently asked questions
* A Certificate of Coverage
Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require
modification or revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by DENR or relieve the permittee From responsibility
for compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order, judgment, or
decree.

.permit. This permit is issued pursuant to the requirements of North Carolina. General Statute 143-215 .1 and the

December 6, 1983.
The stormwater permit you are receiving, NCG140000, does not permit wasting concrete, dumping excess
,,oncrete, directly into storm sewer outfalls or into waters of the state' Any facility which either dumps excess
concrete or washes excess concrete into storm'sewers or waters of the state will be operating in direct violation to
both the terms of this permit and the North Carolina General Statutes. Such a discharge shall be considered an
illegal discharge and may subject the owner to enforcement actions in accordance with North Caro I'm a'General

If any parts. measurement frequencies or sampling requirements contained in this permit are unacceptable to
you, you have the right to request an individual permit by submitting an individual permit application. Unless
such -demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division of
Environmental Management. The Division of Environmental Management may require modification or revocation
and reissuance of the certificate of coverage.
