This certificate of coverage shall become effective April 14, 2014.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

A Compliance Evaluation Inspection was performed at the Cemex plants in Thomasville and
Lexington, NC on June 30, 2010 by Jenifer Carter and Mike Thomas of the Winston-Salem Regional
Office. You, Mr. Johnny Tappia, Regional Environmental Manger and Mr. Tony Hardison, Area
Manager were present for the inspection. The purpose of the inspections was to review compliance with
the National Pollutant Discharge Elimination System (NPDES) General Stormwater Permits. The
following are the findings from the subject inspections:
NCG140066 -- Thomasville
• A Storrriwater Pollution Prevention Plan (SPPP) has been developed, implemented, and updated as
required.
• There are two outfalls. Stockpile rinsing and vehicle/equipment/drum cleaning areas all drain to a
holding pond with an outlet designated as outfall 001. Oufall 002 is a stormwater only outfall. There
is no recycle system in place, and no vehicle maintenance is performed on site.
• Qualitative monitoring has been conducted as required and recorded in accordance with permit at both
outfalls.
• Analytical monitoring has been conducted, with the exception of SS, which will be added to the SPPP
to prevent that from happening again. A pH limit exceedences in November 2008 at outfall 002 was
noted, and corrective actions were taken and properly documented.
• There were no residuals from spills or leaks observed on the ground, nor in or near surface waters.
NCG140067 -- Lexington
• A Stormwater Pollution Prevention Plan (SPPP) has been developed, implemented, and updated as
required.
• There are two outfalls required to be monitored. Oufall 001 drains the stockpile area, though there
were no stockpiles during the visit, as the facility has been closed since 2008. The recycling system
and vehicle/equipment/drum cleaning areas all drain to a bolding pond with an outlet designated as
outfall 003. No vehicle maintenance is performed on site. Outfall 002 actually drains to outfall 003,
so is not required to be monitored separately.

The stormwater permit you are receiving, NCG140000, does not permit wasting concrete, dumping excess
concrete, directly into storm sewer outfalls or into waters of the state. Any facility which either dumps excess
concrete or washes excess concrete into storm sewers or waters of the state will be operating in direct violation to
both the terms of this permit and the North Carolina General Statutes. Such a discharge shall be considered an
illegal discharge and may subject the owner to enforcement actions in accordance with North Carolina General
Statutes 143-215.6A
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to
you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt
of this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North
Carolina General Statutes, and filed with the Office of Administrative Hearings Post Office Drawer 27447,
Raleigh, North Carolina 27611-7447. Unless such demand is made, this decision shall be final and binding.
Please take notice that this certificate of coverage is not transferable. Part II, B.2. addresses the
requirements to be followed in case of change in ownership or control of this discharge.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Environmental Management or permits required by the Division of Land Resources, Coastal Area
Management Act or any other Federal or Local governmental permit that may be required.
