Inspection conducted in conjuction with multimedia inspection program. The inspection of the facility revealed two areas of
concern. The stockpile area should be rerouted to the wastewater pond, or alternative solution The wastewater pond
should be reevaluated for effectiveness. Sampling limitations are }cwer for TSS than with pure stormwater. Improvements
to the pond should be made such as raising the normal pool level.

Comment: Drainage area on the southeast side of property behind the stockpiles should be named as an
outfall or diverted to another outfall.
The center outfall recieves wastewater from the process area. Recommend improvements to
the settling basin by raising the outlet elevation and installing weirs. The stockpile area should
drain to this pond. Recommend consulting with engineer for pond capacity.

Inspection Summary:
Overall, the facility looked to be doing a pretty good job considering it is a very tight site. Would be better if the recycle
could be relocated to different facility or location to allow more flexibiity in managing this site and maintaining vegetative
buffers around entrances to outfalls & around the settling basin.
While not completely compliant, giving facility a marginally compliant status -- would expect all noted issues to be
addressed appropriately and when Plan is updated to meet new permit requirements. Please Note: Permit expires at
end of June 2011, thus need to apply for renewal before expiration to continue coverage.

Comment: All outfalls must be sampled within the window or at least attempted, thus
unlikely that off -site staff can visit site and sample since not all outfalls may discharge
during any given rainfall event or start discharging at relatively the same time. Off -site
staff has been doing the sampling which poses some concern to DWQ staff, but not
sure that all 3 identified outfalls have been properly monitored. Also, samples need to
be collected at end of pipe on backside of retaining wall so visual notes can be taken of
actual discharge appearance & outfall condition can be inspected/noted.

Comment. Only observed one as it was raining and access is difficult; not everyone
was prepared to do so. May require some periodic vegetation maintenance to aid in
doing so. Do not recall finding the current C-of-C, but GP was present.

pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.
The following information is included with your permit package:
* A copy of general stormwater permit NCG140000
* A Stormwater Pollution Prevention Plan (SPPP) Certification Form. Completion of this form is required to
certify that you have developed and implemented the SPPP as per the conditions of the permit. This form
must be completed and returned to the Division within 30 days of receipt of this letter.
DO NOT SEND the SPPP with the signed form.
* Five copies of the Analytical Monitoring form and five copies of the Qualitative Monitoring form
* A copy of a Technical Bulletin for the general permit which outlines changes in the permit, key requirements,

decree.

In accordance with your application for a stormwater discharge permit received on
November 18, 1994, we are forwarding herewith the subject state - NPDES permit. This permit
is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection
agency dated December 6, 1983.
The stormwater permit you are receiving, NCG140000, does not permit wasting or
dumping excess concrete directly into storm sewer outfalls or waters of the state. Any facility
which either dumps excess concrete or washes excess concrete into storm sewers or waters of
the state will be operating in direct violation to both the terms of this permit and the North
Carolina General Statutes. Such a discharge shall be considered an illegal discharge and may
subject the owner to enforcement actions in accordance with North Carolina General Statutes
143-215.6A.
If any parts, measurement frequencies, or sampling requirements contained in this permit
are unacceptable to you, you have the right to an adjudicatory hearing upon written request
within thirty (30) 'days following receipt of this letter. This request must be in' the form -of a
written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed
with the Office of Administrative Hearings, Post Office Drawer 27447, Raleigh, North Carolina
27611 -7447. Unless such demand is made, this decision shall be final and binding.
Please take notice this permit is not transferable. Part II, B.2. addresses the requirements
to be followed in case of change in ownership or control of this discharge.
This permit does not affect the legal requirements to obtain other permits which may be
required by the Division of Environmental Management or permits required by the Division of
Land Resources, Coastal Area Management Act or any other Federal or Local governmental
permit that may be required.
