Dear Mr. McDonald:
On October 26, 2012 the City of Greensboro Stormwater Management Division completed an
industrial compliance inspection of the Green Day Waste and Recycling. Local regulations permit an
inspection of industrial facilities under authority granted by City of Greensboro Code of Ordinance
Chapter 30, Article 12, Section 7.1 (C-5). This inspection consisted of two parts: a review of the facility
files/records and an on -site facility inspection.
The inspection was performed by Debbie Shoffner, and the following deficiencies/observations were
noted during the inspection.
Stormwater Pollution Prevention Plan/Spill Plan Review
Site Maps and SWP3
• Include the following in the SWP3:
• Signed copy of permit
• General location map
• Site map showing drainage patterns, drains, outfalls, and receiving waters
• Routine inspection information: frequency, locations to observe, etc.
Spill Prevention and Response Plan
♦ Include the following in the Spill Response Plan.
• Contact information (names, phone numbers, and other pertinent information)
• Procedures for clean-up in case of a spill
• Proper disposal of materials after a spill
• Major spills that occur or have occurred
Monitoring
♦ Conduct and document water quality sampling as required by your NPDES permit. Include
observation reports and/or qualitative results in the plan.
Employee Training
♦ Regularly train all employees on stormwater issues and practices and spill response
procedures. Maintain specific,. up-to-date training records to include in the plan.

Site Inspection
Preventative Maintenance/Good Housekeeping
♦ Provide spiff containment/clean-up materials, placing them in convenient and easily accessible
locations.
♦ Implement a solution to prevent debris and/or other contaminents from entering the storm
drainage system.
Non-Stormwater Discharges
♦ No deficiencies observed.
Based on the above deficiencies, the City of Greensboro will notify the North Carolina Department of
Environmental and Natural Resources (NCDENR) and request a follow-up inspection. Green Day
Waste and Recycling may or may not be notified by NCDENR in advance of this inspection.
Furthermore, any penalties assessed against the City by the State as a result of this non-compliance
will be assessed against Green Day Waste and Recycling.

Subject: Notice of Violations/Compliance Evaluation Inspection, General Permit No. NCG130000, CDC
NCG130051, Guilford County
Dear Mr. Lowdertnilk:
On May 11, 2010 Sue White of this office conducted a compliance evaluation inspection at Piedmont Paper
Stock site located at the listed address above. Mr. Mark Piotrowski was present for this meeting. During the.
inspection the permit requirements were discussed. There was not a copy of the current permit on site. This permit
was issued on January 30, 2009 and replaced permit number NIC00130002. The folloxving permit violations were
discovered during the interview.
1. Qualitative monitoring had not been done at the time of the inspection. Qualitative monitoring is required
semi-annually with the initial one to occur during the first year that the permit is issued. This is a visual inspection
of the outfall, which is outlined in Part 1I, section B (table 1). A good practice when performing the qualitative
monitoring is to photograph each outfall with a digital camera, utilizing the date stamp. This is easily stored in
binder with any notes on odor.
2. There -was no Stormwater Pollution Prevention Plan on -site during the interview. Also required by the
permit is a Stormwater Pollution Prevention Plan. Contents of a Stormwater Pollution Prevention Plan are outlined
in detail in Part 11, section A, pages 1-4 of 8 in the permit. This information should be contained in a binder and
kept on -site at all times.
3. There were barrels and other containers with potentially hazardous product in them that were not under
cover and had no secondary- containment around them. This is a violation of your StoiTnwater Pollution
Prevention Plan, Part 2, Section (b) on page 2 0f 5, "Secondary Containment Requirements and Records" and could
result in additional violations, from spills if these containers leaked and caused a product spill. (see enclosed photos
1,2,4,6, 10& 12)
Please be aware that the above violations could result in judgments being levied against Piedmont Paper
Stock. You should respond to this office within 30 days of receipt of this letter as to how these violations are to be
addressed and corrected. Should you have any questions about this letter or how to reach compliance, you may

In accordance with your application for discharge permit received on October 9,.1992, we are
forwarding herewith the subject certificate of coverage to discharge under the subject state - NPDES general
permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated
December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division
of Environmental Management. The Division of Environmental Management may require modification or
revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Environmental Management or permits required by the Division of Land Resources, Coastal Area
Management Act or any other Federal or Local governmental permit that may be required.
