In accordance with the Tier Two monitoring requirements located in Part II(B) of
General NPDES Permit (NCG120000), please find enclosed one original and one
copy of October through December 2016 discharge monitoring reports (DMR) for
stormwater discharge outfalls (SDO-2 and SDO-3). SDO-2 and SDO-3 became
subject to the Tier Two monitoring requirements following two consecutive Total
Suspended Solids (TSS) detections above the benchmark value (100 mg/L) which
occurred on June 30, 2015 and November 2, 2015. Therefore, SDO-2 and SDO-3
are in the process of being monitored monthly for all of the stormwater
parameters until three consecutive monthly sampling results are below the
benchmark values. According to the site manager, no discharge occurred at SDO-
2 and SDO-3 during the months of October, November and December 2016. The
site will attempt to collect a monthly sample during the next discharge producing
rain event.

In accordance with the Tier Two monitoring requirements located in Part II(B) of
General NPDES Permit (NCG 120000), please find enclosed one original and one
copy of the February 2016 discharge monitoring reports (DMRs) for stormwater
discharge outfalls (SDO-2 and SDO-3). The outfalls became subject to the Tier
Two monitoring requirements fallowing two consecutive Total Suspended Solids
(TSS) detections above the benchmark value (100 mg/L) which occurred on June
30, 2015 and November 2, 2015. Since December 2015, a monthly sample has
been collected or no discharge has been reported at SDO-2 and SDO-3. A sample
was collected at SDO-2 and SDO-3 on February 3, 2016 and the results show
TSS, Fecal Coliform and Chemical Oxygen Demand (COD) were below the
benchmark values. Tier Two monitoring will continue at SDO-2 and SDO-3.

Dear Permittee:
In response to your renewal application for continued coverage under stormwater General Permit NCG120000
the Division of Water Quality (DWQ) is forwarding herewith the reissued General Permit. This permit is
reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October
15, 2007 (or as subsequently amended).
The following information is included with your permit package:
• A new Certificate of Coverage (COC)
• A copy of General Permit NCG120000
• A copy of the Technical Bulletin for the General Permit
• Two copies of the Discharge Monitoring Report (DMR) Form
• Two copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater, and it specifies your obligations for discharge
controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself
with all the changes in the reissued permit. Your facility has six (6) months from the time of receipt of the
permit to update your current SPPP to reflect all new permit requirements.
The first sample period of your permit begins January 1, 2013. Your facility must sample a "measureable
storm event" beginning during the periods beginning January 1 and July 1 of every year (or, if applicable,
report "No Flow," as outlined in Part III, Section E). Also, please note that Tier 3 Actions in Part II of your
permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this
permit and do not count prior exceedances.
The more significant changes in the General Permit since your last COC was issued are noted either in the
Draft Permit Fact Sheet that accompanied the public notice (http://portal.ncdenr.org/web/wq/ws/su/current-
notices), or in the Response to Comments / Summary of Changes and Technical Bulletin documents that are
posted on the Stormwater Permitting Unit's website with the new General Permit. Please visit
http://Portal.ncdenr.org/web/wq/ws/su/npdessw (click on 'General Permits' tab) to review that information
for your specific General Permit carefully.

Some of the changes include:
Part II:
• Section A: The Stormwater Pollution Prevention Plan (SPPP) section, if applicable, has been updated
to the most current language of our permits. Additional conditions for specific industry sectors have
been added to the SPPP requirements in some cases.
• Sections B, C: Failure to perform analytical stormwater monitoring may result in the Division requiring
that the permittee begin a monthly sampling scheme.
• Sections B; C: A lower TSS benchmark of SO mg/1 for HQW, ORW, PNA and Tr Waters applies to these
more sensitive waters.
• Sections B, C: The monitoring parameter Oil & Grease (O&G) has been replaced by the parameter
Total Petroleum Hydrocarbons (TPH) for vehicle maintenance areas, and in some cases, other
analytical monitoring requirements.
• Sections B, C, 0: Inability to sample due to adverse weather must be recorded in the SPPP, or in
separate on -site records if your General Permit does not require an SPPP. Adverse weather is defined
in the "Definitions" section of the permit.
• Sections B, C. The term "Representative Storm Event" has been replaced by "Measurable Storm
Event." A measurable storm event is defined in the permit.
• Section D: If the permittee fails to respond effectively to problems identified by qualitative
monitoring, DWQ may require the permittee to perform corrective action.
Please review Parts III and IV to understand the Standard Conditions of your new NPDES General Permit,
including Compliance and Liability, Reporting, Monitoring and Records requirements; Operation and
Maintenance obligations; and Definitions. Please note that all samples analyzed in accordance with the terms
of this permit must be submitted to the Division on Discharge Monitoring Report (DMR) forms available on
the Stormwater Permitting Unit's website above. DMR forms must be delivered to the Division no later than
30 days from the date the facility receives the sampling results from the laboratory. Also note that existing
permittees do not need to submit a renewal request prior to expiration unless directed by the Division.
Your coverage under the General Permit is transferable only through the specific action of DWQ_ This permit
does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it
relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law,
rule, standard, ordinance, order, judgment, or decree, if you have any questions regarding this permit

Dear Mr. Thomas:
On March 24, 2010, ,fenny Graznak of this office performed a General Stormwater Permit
Inspection at the Alamance County Landfill located at 2701 Austin Quarter Road in Graham,
North Carolina. This facility holds General Stormwater Permit No. NCG120043 under the
National Pollutant Discharge Elimination System (NPDES) to discharge Stormwater from
Landfills. You were present for the inspection. This type of inspection consists of two basic
parts: a review of facility files and self -monitoring data, and an on -site inspection of the facility's
outfalls.
1. Permit
The permit became effective November 1, 2007 and expires October 31, 2012. Please make
sure you are familiar.with the new permit and changes to the monitoring frequency. Analytical
monitoring is now required twice per year and qualitative monitoring remains on a weekly basis.
2. Records/Reports
As required by the permit, the landfill has implemented an Erosion and Sedimentation Control
Plan, which was approved by NC Division of Land Resources. In accordance with the
Stormwater permit, the facility also holds a permit for a sanitary landfill from NC Division of Solid
Waste Management, Permit # 01-04.
3. Facility Site Review
The landfill site is approximately 536 acres. The landfill consists of an approved trash disposal
area of 90 acres divided into 7 cells, although less than half are currently in use. Leachate is
pumped into two 340,000 gallons holding tanks and then transported by Synagro to the East
Burlington Wastewater treatment plant. The facility also has a recycling program and collects
old batteries and electronics, which are both stored under cover. Typical recyclables such as
plastics, glass, and newspapers are also collected in dumpsters on site.
4. Effluent/Receiving Waters
Stormwater discharges from this site drain to an unnamed tributary to the Haw River in the
Cape Fear River Basin. There are three outfalls on ,the property. However, Outfall 002 was
converted to a fire pond in 2007, at the suggestion of the local fire marshal. The pipe was
capped in order to permanently maintain the storm water in the basin. Therefore, the only storm

water outfalls that discharge from the property are Outfall 001 and Outfall 003. Outfall 001
discharges through a ditch from a small basin at the edge of the woods. At the time of Ms.
Graznak's inspection, Outfall 001 was holding some water and looked a little muddy. Outfall
003 also discharges (through rip -rap) from a small basin at the edge of the woods. Ms. Graznak
did not observe water in Outfall 003 during her inspection. Outfall 002 (now defunct) is now a
large fire pond, which was bright orange and very muddy on the day of the inspection.
5. Self -Monitoring Program
The stormwater permit requires semi-annual analytical and weekly qualitative monitoring at
each of the site's stormwater outfalls. Qualitative monitoring takes place on a weekly basis after
a rain event of 0.5 inches of rain in less than 24 hours. There is a rain gauge on site. Pace
Analytical Services analyzes the samples for the analytical sampling. An on -site review of the
analytical monitoring data during the inspection showed that you have been following the
requirements of the previous permit and only taking samples once per year. Your results for
2006-09 are summarized in the attachment. The new permit was issued in November 2007
and requires a semi-annual monitoring frequency.
Required Action
According to the data, this facility had several exceedances of the benchmark values for Total
Suspended Solids and Fecal Coliform from 2006 to' 2009. Therefore, you are required to do the
following:
(1) Complete a permit -required analytical sampling event prior to June 30, 2010. If the results
exceed the benchmark values listed in your permit (Chemical Oxygen Demand: 120 mgll, Fecal
Coliform: 1000 colonies per 100 ml, and Total Suspended Solids: 100 mg/L), follow the Tier One
response as described in your permit in Part II, Section B. Tier One involves conducting an
inspection of the site to identify the possible causes of the exceedances. Also, you will need to
identify and implement improvements that could be made to bring the results into compliance.
(2) Complete the next analytical sampling event between July 1 and December 31, 2010, If the
results once again exceed the benchmark values listed in your permit, repeat the Tier One
response as described above and Tier Two requirement of monthly monitoring at every outfall
where the sampling result exceeded the benchmark value. Monthly monitoring shall continue
until three consecutive sample results are below benchmark values OR until four sample results
exceed the benchmark value. Please contact our office when you reach that point.

Compliance Schedule Correction

It recently came to the Division's attention that the compliance schedule in the NCG120000 permit is
incorrect. The compliance refers to requiring a Stormwater Pollution Prevention Plan which is not a
requirement of this permit. The corrected compliance language that should go under Part III, Section A: 1. is
shown below. We have also attached a new page reflecting this change for your permit file (Part III page 1 of
11). This new page should replace the current page in your permit.
SECTION A: COMPLIANCE AND LIABILITY
1, Compliance Schedule
The permittee shall comply with the permit in accordance with the following schedule:
Existing Facilities: Upon the effective date of the Certificate of Coverage the permittee shall comply
with all of the conditions detailed in Part 11, Section A: Final Limitations and Controls for Stormwater
Discharges.
New Facilities: Upon the beginning of discharges from the operation of the industrial activity the
permittee shall comply with all conditions detailed in Part H, Section A: Final Limitations and Controls
for Stormwater Discharges.

Water Quality (DWQ) is forwarding herewith the reissued stormwater general permit. This permit is reissued
pursuant to the requirements of North Carolina General Statute 143-215. t and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.
The following information is included with your permit package:

* A copy of General Stormwater Permit NCG120000
* A copy of the Analytical Monitoring Form (DMR)
* A copy of a Technical Bulletin for the general permit
Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require
modification or revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by DENR or relieve the permittee from responsibility
for compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order, judgment, or
decree.
