The inspection consisted of review of the Stormwater Pollution Prevention Plan (SWPPP) and each of its components, review of the facility outfalls, as well as the review of the overall site conditions. The compliance inspection was conducted to ensure all site practices and facilities are in working order. The inspection was also conducted as assistance to the City of Reidsville should there be any questions about permit NCG110143. , Permit: This facility has General Stormwater Permit NCG110143 to discharge stormwater from industrial activity associated with Wastewater Treatment Device or System with a design flow of 1.0 million gallons per day (MGD) or more under the National Pollutant Discharge Elimination System'(NPDES). The permit was originally issued on February 13, 2009 and this version of the permit was issued on May 31, 2018 and will expire on May 31, 2023. Records/Reiports: This facility is required to develop and maintain a SWPPP in accordance with Part 11, Section A of the permit. All qualitative and analytical monitoring records are required to be maintained with the SWPPP for a minimum of five (5) years. Under this permit, analytical monitoring is not required under current operating procedures. Inspections were conducted for both the NPDES Individual Wastewater Permit and NPDES General Stormwater Permit. The assistance and cooperation of Mr. Mike Burleson, Operator in Responsible Charge (ORC) were greatly appreciated. Inspection findings are summarized below and checklists are attached for your reference. NPDES Individual Wastewater Permit NCO024881 The plant is located at 407 Broad Street in Reidsville, Rockingham County, North Carolina. The NPDES individual wastewater permit authorizes the City of Reidsville to operate this 7.5 MGD plant, which consists of coarse screening, grit removal, influent pump station, aeration basin with mechanical aerators, final clarifiers, single media shallow sand infiltration, sludge thickeners, aerobic digesters, sludge storage basins, disinfection via chlorination, de -chlorination, Parshall flume, and effluent pump station. The permit authorizes the City to discharge treated wastewater into the Haw River, which is currently classified as Class WS-IV (Water Supply IV), Nutrient Sensitive Waters (NSW) in the Cape Fear River Basin. Site Review The facility is very well -operated and maintained. Treatment units were online and functioning properly at the time of inspection. Safety measures and maintenance at the site were excellent. City of Reidsville WWTP 5/9/2013 Page 2 of 3 currently in the design phase. Please keep this office informed as to when construction for those improvements begins. Influent and effluent samples are collected by flow -proportional samplers. The tubing was very clean and samplers were in good condition. Collected samples are kept below the 6° Celsius required by the permit. Both influent and effluent flow meters are calibrated on a quarterly schedule by Carolina Technical Services. The outfall was also inspected and found to be in good condition. The discharge point appeared free of pollutant indicators. Access to the discharge point was well -maintained, despite recent rains raising the water level of the stream. Documentation Review Ms. Taylor -Smith evaluated laboratory data during the inspection. Comparison of lab reports and field monitoring data with discharge monitoring reports (DMRs) showed no concerns or errors. Chain of custody records were available and complete. The laboratory facility on site is currently certified to perform all required testing with the exception of some metals and toxicity. Meritech and Environmental One process the samples not run by the facility. ORC visitation and operation/maintenance logs were reviewed and found to be complete and current. Records of sludge hauling and generator service were also available. All records are very thorough and well -organized. In addition to submitting an annual report, the WWTP also provides a monthly report of plant operations, maintenance, and other data to the City of Reidsville. The NPDES permit NC0024881 expired on 30 April 2011. The NPDES Unit of the DWQ received a renewal application from the City of Reidsville on 19 October 2010. Copies of the draft permit were sent to the City of Reidsville and the Winston-Salem Regional Office of the DWQ on 13 February 2013. Both the expired permit and a copy of the draft renewal permit were made available at the time of inspection. The new permit will go into effect on 1 June 2013. NPDES General Stormwater Permit NCGlIO143 This certificate of coverage became effective in February 2009 and allows the City to discharge Stormwater related to industrial activity from the WWTP to Little Troublesome Creek, which is currently classified as WSW, NSW waters in the Cape Fear River Basin. Site Review The site was very clean and well -maintained. No secondary containment is necessary, as there are no above -ground storage tanks (ASTs) on site that require containment. Outfalls were inspected and found to be in good condition. No evidence of erosion or instability was noted at the outfalls and no pollution indicators were seen in receiving waters. City of Reidsville WWTP 5/9/2013 Page 3 of 3 Documentation Review The Stormwater Pollution and Prevention Plan (SPPP) was reviewed and found to be complete, comprehensive, and current. All required monitoring and inspections have been completed and are well -documented. Employee stormwater training is conducted annually and has been thoroughly documented. The facility is exempt from the vehicle maintenance analytical monitoring requirement. No issues or discrepancies were noted. We appreciate your efforts to effectively operate and maintain this treatment system. The assistance and cooperation of Mr. Mike Burleson, Operator in Responsible Charge (ORC) was greatly appreciated. Inspection findings are summarized below. The facility is located at 407 Broad Street, in Reidsville, in Rockingham County. The permit authorizes the City of Reidsville to operate this 7.5 MGD plant which according to the permit, consists of coarse screening, grit removal, influent pump station, aeration basin with mechanical aerators, final clarifiers, single media shallow sand infiltration, sludge thickeners, aerobic digesters, sludge storage basins, disinfection via chlorination, de -chlorination, Parshall flume and effluent pump station. Wastewater produced at the facility is discharged to the Haw River, which is currently classified as C - Nutrient Sensitive Waters in the Cape Fear River Basin. NPDES Permit NCO024881 Site Review The facility is very well operated and maintained. All treatment units were online and functioning properly at the time of inspection. Mr. Burleson informed Mr. Thomas that the City has funded improvements at the plant. The improvements will include increasing -the power connections to the aerators and adding a sludge press. Please keep this office informed as to when construction for these improvements begins. Influent and effluent samples are collected by flow proportional samplers. The tubing was clean and in good physical condition. Collected samples are kept below the 6° Celsius required by the permit. Both flow meters are calibrated on a quarterly schedule. Documentation Review Mr. Thomas evaluated laboratory data during the inspection. Operation logs were reviewed and found to meet all requirements. Laboratory records for both the WWTP lab and the contract lab were reviewed and found to be accurate and complete. The laboratory at the facility is currently certified to perform all required testing with the exception of some of the metals. Merritech and Environment 1 process the samples not run by the facility. NPDES General Stormwater Permit NCG110143 Site Review The site appeared to be very clean and well maintained. Drop inlets were clean with no excessive accumulation of dirt or detritus. Mr. Thomas noted no issues with the stormwater collection system. Documentation Review This Certificate of Coverage became effective in February 2009 and allows the City of Reidsville to discharge stormwater related to industrial activity from the WWTP to Little Troublesome Creek. Little Troublesome Creek is classified as WS-IV (water supply), Nutrient Sensitive Waters, in the Cape Fear River Basin. The Stormwater Pollution and Prevention Plan (SP3) has now been fully implemented. All required components of the SP3 were complete and available for review. Staff training is now being conducted annually as well as when new employees are hired. Records of qualitative monitoring were available for review and were found to be complete. No vehicle maintenance is performed at this facility therefore, no analytical monitoring is required. We appreciate your efforts to effectively operate and maintain this treatment system. The assistance and cooperation of Mr. Mike Burleson,'Operator in Responsible Charge (ORC) was greatly appreciated. Inspection findings are summarized below. The facility is located at 407 Broad.Street, in Reidsville, in Rockingham County. The permit authorizes the City of Reidsville to operate this 7.5 MGD plant which according to the permit, consists of coarse screening, grit removal, influent pump station, aeration basin with mechanical aerators, final clarifiers, single media shallow sand infiltration, sludge thickeners, aerobic digesters, sludge storage basins, disinfection via chlorination, de -chlorination, Parshall flume and effluent pump station. Wastewater produced at the facility is discharged to the Haw River, which is currently classified as C - Nutrient Sensitive Waters in the Cape Fear River Basin. NPDES Permit NCO024881 Site Review The facility is very well operated and maintained. At the time of inspection one aeration basin and one clarifier was offline for maintenance purposes. Carbon continues to be fed into the aeration basin splitter box manually. The addition of carbon combines with surfactants from industrial users to create thick, black foam on the surface of the aeration basin. At the time of inspection this foam did not appear to be hampering the effectiveness of the plant though it does create additional maintenance issues for the staff. Mr. Burleson informed Mr. Thomas that the City is working on funding to make improvements to this system as well as some others at the plant. Influent and effluent samples are collected by flow proportional samplers. The tubing was clean and in good physical condition. Collected samples are kept below the 6° Celsius required by the permit. Both flow meters are calibrated on a quarterly schedule. Due to heavy rains this past winter the facility had to bypass the tertiary filters on a couple of occasions to prevent damaging the filters. On each occasion, Mr. Burleson notified Mr. Thomas of the duration of the bypass. North Carolina Division of Water Qualify. Operation logs were reviewed and found to meet all requirements. Laboratory records for both the WWTP lab and the contract lab were reviewed and found to be accurate and complete. The laboratory at the facility is currently certified to perform all required testing with the exception of some of the metals. Merritech and Environment 1 process the samples not run by the facility. Laboratory chains of custody were complete and well organized. Operation and maintenance records are complete and well organized. In addition, the operator visitation tog was reviewed and found to be complete and current. Mr. Burleson provided Mr. Thomas with a copy of the 2010 annual report for the facility, as required by the permit. Annual reports from previous years were also available at the facility. At the present time, the City of Reidsville has submitted an application for permit renewal for this facility. Until the new permit renewal is received the facility is to continue to operate under the conditions specified in the present permit. The facility operates 24 hours a day 7 days a week. The assigned Operator in Responsible Charge, Mr. Burleson, as well as, the other operators has the appropriate, current certifications to operate the facility. NPDES General Stormwater Permit NCG110143 Site Review The site appeared to be very. clean and well maintained. Drop inlets were clean with no excessive accumulation of dirt or detritus. Mr. Thomas noted no issues with the stormwater collection system. Documentation Review This Certificate of Coverage became effective in February 2009 and allows the City of Reidsville to discharge stormwater related to industrial activity from the WWTP to Little Troublesome Creek. Little Troublesome Creek is classified as WS-IV (water supply), Nutrient Sensitive Waters, in the Cape Fear River Basin. The Stormwater Pollution and Prevention Plan (SP3) has now been fully implemented. All required components of the SP3 were complete and available for review. Staff training is now being conducted annually as well as when new employees are hired. Records of qualitative monitoring were available for review and were found to be complete. No vehicle maintenance is performed at this facility therefore, no analytical monitoring is required. - We appreciate your efforts to effectively operate and maintain this treatment system. This letter is in response to your request for action regarding Item 7 (SP3 Plan) and 3d (Reporting). The SP3 plan was not inquired about at the plant visit, but instead was inquired about after the visit by a phone call from Mr. Boone to Mr. Burleson. Although I'm unsure where the confusion was made, the City does, in fact, have a SP3 plan that was developed for us by Jewell Engineering Consultants and became effective April 30, 2009. I've attached the cover sheet and the certification page as proof. We are due some training as part of the program and that is scheduled for March 17th. The entire plan is available for inspection at the plant if more information is needed. Due to our concern about erroneous information being in the plant's record, we respectfully request a retraction of the Compliance Report indicating that the plant was non -compliant for stormwater. Also, the checklist should be revised to indicate that the documentation was "not evaluated" (NE) versus indicating that it was 'not compliant". It is my understanding that in regards to 3d, all reporting was made as required. The plant is located at 407 Broad Street, Reidsville, Rockingham County, North Carolina. The permit authorizes the city to operate this 7.5 MGD plant, which, according to the permit, consists of coarse screening, grit removal,. influent pump station, aeration basin with mechanical aerators, final clarifiers, single media shallow sand filtration, sludge thickeners, aerobic digesters, sludge storage basin, disinfection by chlorination, dechlorination, parshall flume and effluent pump station. The permit authorizes the city to discharge treated effluent from the plant to the Haw River, this section of which is currently classified as Class C, nutrient -sensitive waters (NSW) in the Cape Fear River Basin. Site Review 3. The plant is well operated and managed; it was very clean and well maintained at the time of the inspection. All components were well maintained and operational at the time of the inspection. No discrepancies were noted. b. Carbon is still being fed manually into the aeration basin splitter box. C. Mr. Burleson discussed that the city is currently considering several plant improvements, to include installation of a bulk carbon feeding system, improvements to the solids handling systems, electrical system upgrades and upgrades to the aeration basins. d. The plant is currently experiencing flow slugs of an unknown source that caused high total suspended solids in February and a bypass of the tertiary filters in early March. Mr. Burleson has informed us that he and his staff are currently trying to pinpoint the source of the slugs but so far have not been able to. The city and the operators are reminded that the permit requires a 24 hour report and written 5 day report for treatment plant bypasses. Please review that part of the permit and provide the necessary reports as soon as possible. Please also keep us informed of the city's progress in locating the source of the slugs. e. The plant effluent was fairly clear but had a slightly tannic color. Despite the color it still appeared to be high quality, with very little solids. The effluent flow is used for reporting. The meter is calibrated at least annually and it matched the recorder at the time of the inspection. Effluent samples are flow proportional. f. The plant has a generator that can run the entire plant. According to Mr. Burleson, it is tested regularly under load. Documentation Review 4. The following documentation was reviewed: a. Laboratory records, both in-house and contract, were reviewed. All records were complete, accurate, well organized and available for review. At least five years worth of records are kept on file. Laboratory records matched discharge monitoring reports and no transcription or other errors were noted. The plant laboratory is currently certified to perform all required testing except for a few of the metals. A contract lab is used for these tests. All laboratory chains of custody were also complete, well organized and available for review. b. Operation and maintenance records were also complete, current well organized and available for review. C. An operator visitation log is kept and is complete and current. d. An annual compliance report for 2008 was received by the Division on 16 February 2009 and is on file. Mr. Burleson had previous reports on file. e. Mr. Burleson and Ms. Blackwell have current appropriate certifications to enable them to operate the plant. f. The plant has not had any limit violations over the previous twelve months. N CG'I 10142 General Information 5. This permit/certificate of coverage became effective in February 2009. This Phase I permitting program was established in 1990 for stormwater discharges from various types of industrial facilities.
