the requirements of North Carolina General Statutes 143-215.1 and the Memorandum of Agreement
between North Carolina and the U.S. Environmental Protection Agency.

This certificate of coverage shall become effective August 3, 2010.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

Form for the transfer of the Town of Elkin Stormwater Permit (Town of Elkin Waste Water Treatment
Plant Permit # NCG 1 10000; Certificate of Coverage NCG 1 10139) to the Yadkin Valley Sewer
Authority. Please note that since we are a `new' entity, we are still working on establishing the
operations for the YVSA and are still determining items that may still need to be addressed. However,
please note that operations for the YVSA became effective on JULY i", 2010. At 12:01am, transfer
of ownership for the entire collections systems for the Towns of Elkin, Jonesville and Ronda were
roiled over to the YVSA. Please note that legal documentation of the transfer of ownership is attached
to this application.

Being a certain lot, tract or parcel of land situated within the municipal
boundaries of the Town of Elkin, Elkin Township, Surry County, State of North
Carolina; bounded by natural boundaries and/or lands now or formerly owned by
or in possession of the following persons: on the north by the Town of Elkin

east by the western controlled access right of way boundary of Interstate 77

"assignor") to THE YADKIN VALLEY SEWER AUTHORITY, INC., a duly
organized "sewer authority" (hereinafter "assignee").
FOR AND IN CONSIDERATION OF THE SUM OF TEN DOLLARS AND
OTHER GOOD AND VALUABLE CONSIDERATIONS, the receipt of which
are hereby acknowledged, the assignor does hereby set over, transfer,
assign and convey all of its rights of way and easements for sewer
lines, locations of pumping stations and transfer stations, and any and
ali other uses in conjunction with the Town of Elkin's former sewer
treatment operations, including all recorded and unrecorded rights of
way and easements, including those acquired by prescriptive use,

located within any of said rights of way and easements.
The assignor represents that it is the owner of these rights of way and
easements and that by a resolution, duly adopted by the Board of
Commissioners, is authorized to transfer and assign these rights of
way and easements to the assignee for the purpose of the Yadkin
Valley Sewer Authority assuming ownership and operation of the entire
sewer processing operation formerly owned and conducted by the

THIS BLANKET ASSIGNMENT OF RIGHTS OF WAY AND EASEMENTS

TOWN OF JONESVILLE, a North Carolina municipal corporation
(hereinafter "assignor") to THE YADKIN VALLEY SEWER AUTHORITY,
INC., a duly organized "sewer authority" (hereinafter "assignee").
FOR AND IN CONSIDERATION OF THE SUM OF TEN DOLLARS AND
OTHER GOOD AND VALUABLE CONSIDERATIONS, the receipt of which
are hereby acknowledged, the assignor does hereby set over, transfer,
assign and convey all of its rights of way and easements for sewer
lines, locations of pumping stations and transfer stations, Jonesville
Operations Station and waste water treatment plant as recorded in
Book 89, page 274, YCR, and any and all other uses in conjunction
with the Town of Jonesville's former sewer treatment operations,
including all recorded and unrecorded rights of way and easements,
including those acquired by prescriptive use, together with all sewer
lines, pumping stations and transfer stations located within any of said
rights of way and easements.
The assignor represents that it is the owner of these rights of way and
easements and that by a resolution, duly adopted by the Town
Council, is authorized to transfer and assign these rights of way and
easements to the assignee for the purpose of the Yadkin Valley Sewer
Authority assuming ownership and operation of the entire sewer
processing operation formerly owned and conducted by the Town of
Jonesville.

Lake District Properties, LLC, (hereinafter "Party of the First Part"); and The Yadkin Valley
Sewer Authority (hereinafter "Party of the Second Part");
WITNESSETH:
That whereas said Party of the First Part owns a certain tract of land more fully described
in Record Book 1051, Page 225 of the Surry County Registry, located South of N. C. Highway
268 and North of the Yadkin River, and further bounded on the East by the lands of Harold and
Barbara Adams, and on the South and West by the lands of CMC Holding Corp.; and whereas,
the Party of the Second Part desires to construct a sewer line main across the said lands of the
Party of the First Part;
NOW THEREFORE, said Party of the First fart for and in consideration of the sum of
S 10.00, and other valuable consideration, to it in hand paid, the receipt of which is hereby
acknowledged, does hereby give, grant and convey unto said Party of the Second Part a perpetual
right and easement to construct and maintain across and upon said land of the Party of the First
Part an underground sewer line and necessary manholes upon the thirty foot sewer line easement
hereinafter described and to go upon said lands hereinafter described whenever the same is
reasonably necessary for the purpose of inspecting, maintaining and repairing said sewer line and
necessary manholes; provided, that in constructing and repairing said sewer line and necessary
manholes, Party of the Second Part will remove all surplus earth, make level the surface of the
ground above said sewer line, and interfere as little as is reasonably possible with any plants,
fences, or other improvements upon the land of the Party of the First Part.
