A NPDES General Stormwater Compliance Evaluation inspection of the Chadbourn Wastewater Treatment
Facility was conducted by Dean Hunkele from this office on August 11, 2011. This inspection was to verify
that the facility is operating in compliance with the conditions and limitations specified in its NPDES
General Stormwater Permit No. NCG110109. The findings and comments noted during this inspection are
provided in the enclosed copy of the inspection report entitled "Compliance Inspection Report".
The following violations were noted during the inspection:
Compliance Issues
Facility had a single page document with a few paragraphs written on it and was titled as the
Stormwater Pollution Prevention Plan (SPPP). However, it is missing most of the required elements
identified in the General Permit (NCG110000). Facility did not have a copy of the Certificate -of -
Coverage.
Facility and SPPP does not have a site map with drainages & outfalls shown. At least two known outfall
locations exist on the site.
No documented semi-annual qualitative inspections of the site and outfalls.

Town parks/stores its sludge hauling truck very close to the one outfall near the head of the newer aeration basin. Need
to ensure this vehicle is cleaned after each use before parking it here and ensure that no mechanical leaks occur to
stormwater system.

state — NPDES general permit. This permit is issued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina
and the US Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).
Please take notice that this certificate of coverage is not transferable except after notice to
the Division of Water Quality. The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be

Summary
This facility submitted a Notice of Intent to be covered under General Permit NCGI10000
(Treatment Works). The stormwater discharge drains to Soules Swamp (Class C-Sw), in the Lumber
River Basin. This facility is located in Columbus County, handled by the Wilmington Regional
Office.
Application review
a) The permit application has been signed and sufficiently completed. Mailing address of
owner/operator left blank, but cover letter with Town Manager's information was
included with the NOI.
b) The SIC code accurately describes this facility's activities and is covered by this general

No concerns, this is a facility that should have been permitted under the NCGll a long
time ago. You should get another NOI from City of Whitevilie. They
need the permit as well.

Does the Wilmington Regional Office have any concerns about issuing this facility a
COC for this general permit? If we don't receive any objections, we'll issue the
COC in 3C days.
