This Stormwater Pollution Prevention Plan is prepared in accordance with and follows
the format of Part 11 Monitoring, Controls, and Limitations for Permitted Discharges,
Section A: Stortnuuter Pollution Prevention Plan found in Part 11 on pages 1 through 5 of
the Town of Spindale's General Stormwater Permit No. NCG1 10000 issued by the NC
Division of Water Quality on ,tune 1, 2008.
1. SITE PLAN
(a.) A general location map of the wastewater treatment plant (WWTP) existing
facilities is provided in Figures i and 2 which show the location of the
W wastewater treatment plant in relationship to transportation routes and the
receiving water to which most of the stonnwater eventually discharges via
sheet flow, Holland Creek. The latitude and longitude of the discharge point,
which is a drainage ditch exiting the WWTP, is 35" 22' 32.50"N and 81' 54'
14.93"W respectively,
(b.) The Town of Spindale stores chemicals at the wastewater treatment plant that
are necessary for the operation and maintenance of the facility.

Small amounts of other chemicals are stored on site such as reagents in the
laboratory, cleaning supplies, etc. that will not be included in this report.
Ferric Chloride is delivered to the WWTP by the vendor and stored near the
Operations Building. The chlorine and sulfur dioxide cylinders are brought to
the plant site by a commercial truck and offloaded to the Chemical Feed
Facility. Only quantities necessary for immediate use are stored on site.
There is no long term storage of these or other chemicals on site.
There are no chemicals stored outside exposed and unprotected. There are no
outside process areas involving chemicals. Chemical use is contained within
dedicated structures. No dust or particulates are generated at this facility.

Waste sludge is stored in an above -ground concrete sludge holding basin.
When sludge removal is necessary a licensed sludge disposal company pumps
the sludge from the holding basin into trucks for land application off site.
(c.) Figure 3 shows the existing stormwater system to scale as well as the existing
wastewater treatment plant facilities. All stormwater within the wastewater
treatment plant is transported off site by means of sheet flow over gentle grade
with one exception. There is an area where stormwater is captured in a
drainage ditch and transported off site and that is the southern end of the
aeration basin, the location is shown on Figures 2 and 3.
There is no piped discharge for stonmwater from this site.
(d) There have been no spills or leaks at the wastewater treatment plant during the
3 previous years.
(e.) Certification that the stormwater outfall has been evaluated for the presence of
non-stormwater discharges is found in Exhibit A. Non-stonmwater discharges
have not been found at the outfall.

2. STORMWATER MANAGEMENT PLAN
(a.) Feasibility Study
An onsite evaluation was conducted of the chemicals utilized at the
wastewater treatment plan, the method of operations and storage practices
employed on site. The Town of Spindale's operation staff is properly
managing the chemicals stored on site. There is no direct potential
contamination of stormwater by the chemicals utilized on site. No changes to
the existing operations and storage practices are recommended.
(b.) Secondary Containment Schedule
M There is no requirement for a secondary containment schedule for the bulk
storage of liquid materials at the Spindale wastewater treatment plant.
(c.) Best Management Practices (BMP) Summary
` No non-stormwater discharges were identified anywhere at the wastewater
treatment plant. All chemicals used at the wastewater treatment plant are
properly managed and stored by the operations stafff No spills or leaks have
occurred within the last three (3) years at the wastewater treatment plant. No
structural BMPs are necessary at the wastewater treatment plant at this time.
The following non-structural BMPs are recommended as good practice to
follow by the operations staff on a continuous basis.

l . Eliminate exposure of materials and equipment wherever possible by
moving materials and equipment to indoor locations.
2. Practice good housekeeping on -site. Keep materials handled and
stored at the facility in an orderly fashion.
3. Examine process and exchange hazardous materials for non -hazardous
ones wherever possible.
4. Establish routine leak and maintenance checks to minimize the chance
of spills before they occur. When spills do occur, clean them up
immediately.
5. Establish bulk storage tank protocols that minimize the risk of spills
during loading and unloading procedures.
6. Store used pallets and process waste dumpsters inside or under a roof
where water cannot flow on or around them.
3. SPILL PREVENTION AND RESPONSE PLAN
(a.) Conduct an Annual Inspection of the Plant Facilities.
(b) Update the SPPP based on the Annual Review.
(c.) The Wastewater Treatment Plant Superintendent shall have overall
■
responsibility for implementing the Spill Prevention and Response Plan.
(d.) The Wastewater Treatment Plant Superintendent shall also have day-to-day
M responsibility for operating procedures and controlling potential contamination
of stormwater.
(e.) If an accidental chemical spill results in potential stormwater contamination the
Wastewater Treatment Plant Superintendent shall take immediate steps to
protect the spill from entering the catch basins. This shall initially consist of
placing impervious protection devices around the catch basins affected by the
spill. These devices shall be on hand at the wastewater treatment plant at all
times.

(f) Contact the Division of Water Quality if the accidental spill results in any water
quality violation.
(g.) A post evaluation shall be conducted atter the spill cleanup has been completed
to detennine the cause of the spill and to take necessary measures to adjust
operation and maintenance procedures to eliminate as much as possible the
potential for contamination to stonnwater.
4. PREVENTATIVE MAINTENANCE AND GOOD HOUSEKEEPING
PROGRAM
The Town of Spindale's operation and maintenance staff at the wastewater treatment
plant consist of the Wastewater Treatment Plant Superintendent and two operators.
Both operators at the wastewater treatment plant are responsible for conducting an
assessment of potential pollutant sources quarterly. The assessment shall consist of a
walk through of the wastewater treatment plant facility documenting any changes in
the use and storage of chemicals at the wastewater treatment plant. Any changes
shall be noted ?nd identified to the %�.Iastcv,,at►.r T roatment i lain Supel'111teIll'lent who
shall perform the following actions upon notification of any changes.
1. Visually inspect the change to verify if they are the result of approved operational
procedures.
2. Identify if the changes will become permanent.
3. Review the non-structural BMPs to take immediate action as necessary to prevent
potential contamination of stormwater.
4. If needed consult the Town Engineer to modify the Stormwater Pollution
Prevention Plan as necessary to incorporate measures to prevent potential
contamination of stormwater.

5. EMPLOYEE TRAINING
The Wastewater Treatment Plant Superintendent shall conduct annual training on proper ,
spill response, cleanup procedures and preventative maintenance activities.
6. RESPONSIBLE PARTY
The Wastewater Treatment Plant Superintendent shall be responsible for the overall
coordination, development, implementation, and revision to the SPPP. The Wastewater
Treatment Plant Superintendent shall utilize the Town Engineer as necessary for review
and updating components of the SPPP.
7. PLAN AMENDMENT
The Town of Spindale shall amend the SPPP whenever there is a change in design,
construction, operation, or maintenance at the wastewater treatment plant which has a
significant effect on the potential for the discharge of pollutants from the wastewater
treatment plant site. The SPPP shall be reviewed by the Town Engineer on an annual
basis.
8. FACILITY INSPECTION PROGRAM
The Wastewater Treatment Plant Superintendent shall conduct semi annual inspections of
all stormwater systems on the following schedule:

The inspections will be conducted either by the Wastewater Treatment Plant
Superintendent or the Town Engineer. The inspections shall document the date and time
of the inspection, individual(s) perfonning the inspection and provide a narrative
description of the facility's stonnwater control systems, plant equipment and systems.
Records of these inspections shall be incorporated into the SPPP and shall be kept on file
for five (5) years.
The stormwater discharge characteristic monitoring as required in Part II: B, Table I of
. the General Stormwater Permit shall be performed in addition to the facility inspections,
a form is provided in Exhibit B for this purpose.
9. IMPLEMENTATION
The Town of Spindale shall document all monitoring, measurements, inspections and

maintenance activities and training provided to employees, including the log of the
sampling data and of activities taken to implement BMPs. Such documentation shall be
kept on -site for a period of five (5) years and made available to the Division of Water
Quality upon request.

The Town of Spindale owns and operates a wastewater treatment plant (WWTP) located
on Ecology Drive, Spindale, NC 28160 under NPDES Permit NC0020664. On June 1,
2008 this plant was certified by the Division of Water Quality to operate under General
Storm Water Permit No. N C G l 10000.
This Certification is provided by Kurt D. Wright & Assoc., Inc. pursuant to Part II,
Section A part I (e) on behalf of the Town of Spindale. The plant was inspected by Kurt
Wright, PE along with Bill Hodge, Superintendent and ORC ofthe WWTP.

This certifies that the stormwater discharge (drainage ditch exiting the WWTP) located at
the Spindale Wastewater Treatment Plant with Lat: 35' 22' 32.50"N and Long: 81' 54'

Dear Permittee:
In response to your renewal application for continued coverage under stormwater General Permit NCG110000 the Division of
Water Quality (DWQ) is forwarding herewith the reissued stormwater General Permit. This permit is reissued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North
Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as subsequently amended.)
The following information is included with your permit package:
• A new Certificate of Coverage
• A copy of stormwater General Permit NCG110000
• A copy of a Technical Bulletin for the General Permit
Five copies of the Discharge Monitoring Report (DMR) Form
+ Five copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater only, and it specifies your obligations with respect to stormwater
discharge controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself with all
the changes in the reissued permit.
The more significant changes in the General Permit since the your last Certificate of Coverage include the following:
• Permit cover page — Coverage is expanded to include industrial activities similar to the categorically identified industries.
• Part I Section A — This part explains that the No Exposure Exclusion from permitting option may be available to the
permittee.
• Part II Section A — Several minor expansions and clarifications of the required content of the Stormwaler Pollution
Prevention Plan have been added. Please be sure that your plan is updated to comply with the new requirements.
• Part II Section B — Qualitative monitoring must now be conducted during a representative storm event.
• Part II Section B — The permittee is required to keep a written record of his response to problems identified by his
qualitative monitoring observations.
• Part II Section C - For facilities with vehicle maintenance activities using more than 55 gallons of new motor oil per month,
new provisions require that discharges from the vehicle maintenance areas be sampled twice per year, and that the
sampling results be compared to new numerical benchmark values. (The previous cutoff concentrations have been
eliminated.)
• Part It Section C — For facilities with vehicle maintenance activities using more than 55 gallons of new motor oil per month,
new provisions require the permittee to execute Tier One and Tier Two,response actions, based on the first benchmark
exceedence (Tier One) and the second consecutive benchmark exceedenct; (Tier Two). Tier Two requires that the
permittee institute monthly monitoring instead of twice -per -year monitoring, until three consecutive monitoring events
show no benchmark exceedences.
• Part 11 Section C — This part contains a new provision that four exceedences of any particular benchmark will trigger
increased DWQ involvement in the permittee's stormwater management and control actions. DWQ may direct the

The Division several years ago developed General Stormwater Permit No. NCG110000, it is
applicable to owners or operators of stormwater point source discharges associated with activities
classified as Treatment Works. Treatment Works are defined as those facilities treating municipal or
domestic sewage with a design flow of 1.0 million gallons per day or more, or facilities, which are
required to have an approved pretreatment program.
A review of the Division's records indicates that the Town does not have a permit, has not
submitted a Notice of Intent (NOI) or has not submitted a No Exposure Cerfification for the Spindale
WWTP, in accordance with North Carolina General Statutes 143-214.7 and 143-215.1. 1 have
enclosed a copy of the NOI and a copy of the Technical Bulletin for Stormwater General Permits,

Enclosed please find a copy of the Compliance Evaluation Inspection form from the
inspection conducted on April 25, 2006 by Wanda Frazier of the Asheville Regional Office.
The facility was found, to 'be deficient with i regard to. permit NCG 100107,- because there
was no site specific Stormwater Pollution Prevention Plan (SPPP).
Based on our discussion and a review of the site it appears that stormwater does not
leave the. site. Due to your ownership of the large acreage of land surrounding your business
and the lay of the land, I discussed this with the Stormwater Permitting Unit in Raleigh. They
indicated that your business may not be required to maintain a stormwater permit.
l understand that you are in the process of crushing, cleaning -up and going out of
business by the end of the summer.. Please contact me at that time, so that I may perform a
final site inspection.
Or if you prefer, you may instead submit a site map of your business detailing the lay of
the land and stormwater flow documenting that stormwater does not leave your property, Along
with this information, you may send a letter requesting that stormwater permit NCG100107 be
rescinded.
