Comment: Employee Training not documented. Plan not updated to show upgrades
to plant. Also need to add property across the street, as it has been used to dump
influent from pump station once during emergency situation. Will not be used on a
regular basis, but the potential for a similar situation is present. All outfalls drain to
common downstream point of stream along property and can be used; however, should
a problem show up, the cause could be attibuted to an off-site/upstream source. This
should be considered when deciding to monitor each outfall separately, or just the one
common downstream point.

Description of Collection System:
The Thomasville sanitary sewer collection system is currently comprised of a system of 209.35 miles, or
1,105,367 feet of pipe, and 28 sewage lift stations spread throughout the area.
Description of Treatment Process:
Hamby Creek WWTP is a 4.OMGD capacity grade 4 wastewater treatment facility using the activated
sludge process with post aeration. Headworks equipment consists of solids shredding and grit collection
and removal. Primary treatment uses two circular clarifiers equipped for removal of raw sludge and
floating solids. Secondary treatment consists of two high rate trickling filters followed by single stage
extended aeration activated sludge. Removal of activated sludge is accomplished in three circular
secondary clarifiers equipped for rapid removal of solids for return to process. Post aerated effluent
receives chlorination for disinfection prior to discharge into Hamby Creek. Raw and waste activated
sludge are belt pressed and landfilled or anaerobically digested to standards for class B sludge as defined by
EPA in CFR Part 503 then applied to farm land as a beneficial reuse.
II. PERFORMANCE
In 2007 Hamby Creek WWTP treated 934.5 Million Gallons of wastewater at an average daily flow of
2.56 MGD. The plant was out of compliance for BOD in January, February, March, April, May, June,
July, August, September, October, and December; for ammonia in February, March, April, May, June,
July, August, September, October; for TSS in April, and June; for pH in November; for chromium in April;

for selenium in May and August; and for fecal coliform in January. The inability to provide sufficient
oxygenation has caused most of the operational problems by resulting in inhibition of the biological process
leading to the BOD and ammonia violations. Additionally, lack of a functional heating and mixing system
in the digesters has limited sludge disposal methods, causing solids to build up beyond normal levels in the
plant process. Construction of the upgraded / expanded plant facility which began in March, 2006 has
continued steadily and benefitted from the dry weather experienced in 2007. Completion of the new
facilities should aid in resolving most operational difficulties in meeting permit parameters.
III. MONTHLY REPORT 2007
January: On the January DMR we reported being noncompliant for BOD all weeks and the month
and noncompliant for Fecal Coliform for one weekly average. NCDENR issued an NOV and the City of
Thomasville paid a fine of $13,598.00.
February: On the February DMR we reported being noncompliant for BOD all weeks and the
month and noncompliance for Ammonia for one weekly average and the monthly average. NCDENR
issued an NOV and the City of Thomasville paid a fine of $6098.00.
March: On the March DMR we reported that BOD limits were exceeded for one week and for the
month -The- monthly, average -effluent ammonia -and -one weekly -average -were -also -slightly exceeded.
NCDENR issued an NOV.
April: On the April DMR we reported exceeding the effluent BOD limits for the month and four
weeks. We also reported exceeding the effluent ammonia weekly average once and the monthly average.
The permit average weekly TSS limit was exceeded once and the monthly average limit was exceeded.
Chromium was also exceeded for one week. NCDENR issued an NOV and the City of Thomasville paid a
fine of $12,098.00.
May: On the May DMR we reported exceeding the effluent BOD and ammonia limits for the
month and four weeks. Selenium was exceeded for one week. NCDENR issued an NOV and the City of
Thomasville paid a fine of $10,598.00.
June: On the June DMR we reported exceeding the effluent BOD limits for the month and five
weeks. Ammonia was exceeded for the month and three weeks. We also reported exceeding the weekly
effluent TSS average limits for one week. NCDENR issued an NOV and the City of Thomasville paid a
fine of $11,351.72.
July: On the July DMR we reported exceeding the effluent BOD limit for the month and three
weeks. The monthly average for ammonia was also exceeded. NCDENR issued an NOV and the City of
Thomasville paid a fine of $6,851.72.
August: On the August DMR the BOD monthly average and four weekly averages were
exceeded. The monthly average and three weekly averages for ammonia were exceeded. Selenium was
exceeded for one week. Chronic Bioassay was failed. NCDENR issued an NOV and the City of
Thomasville paid a fine of $8,351.72.
September: On the September DMR we reported exceeding the BOD weekly average four times and
the monthly average. The ammonia monthly average was also exceeded. Phase II Chronic Bioassay was
failed. 'NCDENR issued an NOV and the City of Thomasville paid a fine of $16,601.72.
October: On the October DMR we reported the BOD effluent weekly averages were exceeded all
four weeks and for the month. The monthly average for Ammonia was also exceeded. We have not yet
received an NOV from NCDENR.

November: On the November DMR we reported the effluent pH range was exceeded once. We have
not yet received an NOV from NCDENR.
December: On the December DMR we reported exceeding the BOD monthly average and the
ammonia average for the month and one week. We have not yet received an NOV from NCDENR.
IV. SANITARY SEWER OVERFLOWS
During 2007 the City of Thomasville experienced 14 sanitary sewer overflows that reached surface waters.
There were 6 spills that did not reach surface waters. These spills are summarized in the table below. The
amount spilled represents only 0.041 % of the total amount collected.

subject state —NPDES general permit. This permit is issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the,US Environmental Protection Agency dated May 9; 1994 (or as subsequently
amended).
Please take notice that this certificate of coverage is not transferable except after notice to
the Division of Water Quality. The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage.
'This permit does not affect the legal requirements to obtain other permits which may be
required by the Division of Water Quality or permits required by the Division of Land

Summary
This facility submitted a Notice of Intent to be covered under General Permit NCG110000
(Treatment Works). The stormwater discharge drains to North Hamby Creek (Class C), in the
Yadkin -Pee Dee Basin. This facility is located in Davidson County, handled by the Winston-
Salem Regional Office.
