Jim Gregson and I conducted an inspection of the Wilmington Northside WWTP Facility on May 9, 2013. This
inspection was conducted to verify that the facility is operating in compliance with the conditions and limitations
specified in NPDES General Permit No. NCG110063. The findings and comments noted during this inspection are
provided in the enclosed copy of Compliance Inspection Reports.

Inspection Summary:
While the facility is not technically in compliance with this permit,. the Regional Office staff believes that the facility does
not require coverage under this permit any other longer 9 the system is added as a component of their individual NPDES
wastewater treatment permit (NC0023965). DWQ staff also believes that SW8040311 would no longer be necessary
either and could be rescinded by the City of Wilmington rather than transferred to the CFPUA which never occurred in
2008.
The CFPUA should request a minor permit modification of NCO023965 from DWQ's NPDES permitting unit and once
issued, then initate recission of both stormwater permits mentioned above.
A channel has been either cut or eroded around the forebay opposite of the inlet within the larger Pond #1. This is not
shown to be a design feature per the approved plans, thus it needs to be repaired. If it eroded, then it is likely that velocity
across the forebay is too high from the inlet and may require installation of something to dissipate the incoming
energy/velocity.

Comment: The Plan has not been updated since it was developed in 2004, thus in
theory the facility is not in compliance with the requirements of this permit. However, all
stormwater is directed to one of two on -site ponds approved under SW8040311 issued
to the City where it is pumped into the wastewater plant and treated -- each pump
station is connected to the facility's back-up power system too. The facility is in the
process of developing/revising its Spill Response Plan and does need a simpler
stormwater collection system map that is color -coded by drainage basin ideally.

Comment: It was being done into 2011 until all outfalls were plugged so all stormwater

then it is likely that velocity across the forebay is too high from the inlet and may require

forwarding herewith the subject certificate of coverage to discharge under the subject state — NPDES general
permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated May
9, 1994 (or as subsequently amended).
Please take notice that this certificate of coverage is not transferable except after notice to the Division
of Water Quality. The Division of Water Quality may require modification or revocation and reissuance of the
certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Water Quality or permits required by the Division of Land Resources, Coastal Area Management
