SW Permit #: NCG110005 Dear Mr. Flynt: On February 16, 2012 the City of Greensboro Stormwater Management Division completed an industrial compliance inspection of the TZ Osborne Wastewater Treatment Plant. Local regulations permit an inspection of industrial facilities under authority granted by City of Greensboro Code of Ordinance Chapter 30, Article 12, Section 7.1 (C-5). This inspection consisted of two parts: a review of the facility files/records and an on -site facility inspection. The inspection was performed by Rebecca Wells, and the following deficiencies/observations were noted during the inspection. Stormwater Pollution Prevention Plan/Spill Plan Review Site Maps and SWP3 ♦ No deficiencies observed. Spill Prevention and Response Plan ♦ Document how to properly dispose of materials after a spill. Monitoring ♦ No deficiencies observed. Employee Training ♦ Include employee training records in the Stormwater Pollution Prevention Plan. Site Inspection Preventative Maintenance/Good Housekeeping ♦ No deficiencies observed. Non-Stormwater Discharges ♦ No deficiencies observed. It is recommended that TZ Osborne Wastewater Treatment Plant take appropriate actions to correct the noted deficiencies. Rcci2rd LRej oijs A review of the laboratory reports and Discharge Monitoring Reports (DIv1Rs) for the City of Gteensboro TX Osborne W WTP was conducted on the date of inspection. An amended DMR for October 2008 will be submitted to the Division. A copy of the laboratory data is kept on file at the wastewater platit. Maintenance records were reviewed and deentcd compliant Willi permit Conditions. Operations records iiiLhide all s,irnple analyses and process conhrol tests that arc performed daily. Calibration rccorcls for bold parameters (p77, DO, and T.crnp), as well as flow ineter calibration records, were cvaluated and deemed compliant. Docility site Review The facility site review indicated that the 40 M61) treatment works is cunsisterit with the permitted components. Osborne WWTP COC Number NCG110005 GuilfordCounty Dear Permittee: In response to your renewal application for continued coverage under stormwater General Permit NCG110000 the Division of Water Quality (DWQ) is forwarding herewith the reissued stormwater General Permit. This permit is reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as subsequently amended.) The following information is included with your permit package: • A new Certificate of Coverage • A copy of stormwater General Permit NCG110000 • A copy of a Technical Bulletin for the General Permit • Five copies of the Discharge Monitoring Report (DMR) Form • Five copies of the Qualitative Monitoring Report Form The General Permit authorizes discharges of stormwater only, and it specifies your obligations with respect to stormwater discharge controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself with all the changes in the reissued permit. The more significant changes in the General Permit since the your last Certificate of Coverage include the following: • Permit cover page — Coverage is expanded to include industrial activities similar to the categorically identified industries. • Part I Section A — This part explains that the No Exposure Exclusion from permitting option may be available to the permittee. • Part II Section A — Several minor expansions and clarifications of the required content of the Stormwater Pollution Prevention Plan have been added. Please be sure that your plan is updated to comply with the new requirements. • Part 11 Section B — Qualitative monitoring must now be conducted during a representative storm event. • Part 11 Section B — The permittee is required to keep a written record of his response to problems identified by his qualitative monitoring observations. • Part I I Section C - For facilities with vehicle maintenance activities using more than 55 gallons of new motor oil per month, new provision; require that discharges from the vehicle maintenance areas be sampled twice per year, and that the sampling results be compared to new numerical benchmark values. (The previous_ cutoff concentrations have been eliminated.) • Part 11 Section C — For facilities with vehicle maintenance activities using more than 55 gallons of new motor oil per month, new provisions require the permittee to execute Tier One and Tier Two response actions, based on the first benchmark exceedence ('± ier One) and the second consecutive benchmark exceedence (Tier Two). Tier Two requires that the permittee institute monthly monitoring instead of twice -per -year monitoring, until three consecutive monitoring events show no benchmark exceedences. • Part I I Section C — This part contains a new provision that four exceedences of any particular benchmark will trigger increased DWQ involvement.in the permittee's stormwater management and control actions. DWQ may direct the permittee to apply for an individual permit, or. may direct the implementation or installation of specific stormwater control measures.
