On February 16, 2012 the City of Greensboro Stormwater Management Division completed an
industrial compliance inspection of the TZ Osborne Wastewater Treatment Plant. Local
regulations permit an inspection of industrial facilities under authority granted by City of
Greensboro Code of Ordinance Chapter 30, Article 12, Section 7.1 (C-5). This inspection
consisted of two parts: a review of the facility files/records and an on -site facility inspection.
The inspection was performed by Rebecca Wells, and the following deficiencies/observations
were noted during the inspection.

A Compliance €;valualion hispection (CHI) was performed on the City of Greensboro 'I'Y. Osborne wastewater treatment
plant on Wednesday, June 24, 2009 by Corey Basinger of the Winston -Sateen Rogiorral Office. Wally K€iiig and A4artie
Croonie represented the City for the inspection. The inspection Consisted of two parts: an on -site ilispcctiorl of the
treatment facility and a file review. The following ate the Inidings front the subject 6q)ection.
I. Permit

Jime 30, 20I 1.
The: perruitted Components of the wastewater Ireatni nt plant include: inllueut now incassrreinent, incclianical bar screens,
aerated grit chambers, How equalization tank, six (6) primary clarifiers, twelvo (12) aeration basins, seven (7) final
elarifiers, six (6) sand filters, chemical addition for phosphorous rcruoval, chlorination, dechlorination, cascade post

II. Rcci2rd LRej oijs
A review of the laboratory reports and Discharge Monitoring Reports (DIv1Rs) for the City of Gteensboro TX Osborne
W WTP was conducted on the date of inspection. An amended DMR for October 2008 will be submitted to the Division. A
copy of the laboratory data is kept on file at the wastewater platit. Maintenance records were reviewed and deentcd
compliant Willi permit Conditions. Operations records iiiLhide all s,irnple analyses and process conhrol tests that arc
performed daily. Calibration rccorcls for bold parameters (p77, DO, and T.crnp), as well as flow ineter calibration records,
were cvaluated and deemed compliant. The records/reports were very cornprehcnsiveand well organized.
l[l. Docility site Review
The facility site review indicated that the 40 M61) treatment works is cunsisterit with the permitted components. All
components were operat'nlg satisfactorily oil the date of the subject inspection. The following were; noted during the
inspection:
• Aeration basins 11 i &_ 1i2 were lint in Ilse on the dtiw of inspection. Basin n 12 was rn itse for the I AS pilot projccL
• One large primary claiibcr was not in use on the dote of inspection.

V. Flow tMeasuremcnt
Flow is measured at rho discharge from the cascade post aeration with a I'arshall flume in conjunction with a A4111tronics 11
Open Channel Monitor ultrasonic flow motor, flow recorder, and totalizer. The flow meter is electronically calibrated at
bast once a year. The most rccent calibration was conducted in March 2009.

All monitoring was performed in compliance with the MIRES Permit No. NC00=17384, A review of the discharge
monitoring reports (DMRs) for the time period of August 2008 through NImcli 2009 was conducted by the inspector. All
effluent violations noted during this review poriod have been previously addresscd via either Notice. of Violation or
Enforcement Action by the Division.
All upstream anti downstream sampling and monitoring is performed by the Upper Cape Fear- River Basin Association,

available at the wastcwatcr plant. The required monitoring had been performed semi-annually as rcquirecl by the permit.

Dear Permittee:
In response to your renewal application for continued coverage under stormwater General Permit NCG110000 the Division of
Water Quality (DWQ) is forwarding herewith the reissued stormwater General Permit. This permit is reissued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North
Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as subsequently amended.)
The following information is included with your permit package:
• A new Certificate of Coverage
• A copy of stormwater General Permit NCG110000
• A copy of a Technical Bulletin for the General Permit
• Five copies of the Discharge Monitoring Report (DMR) Form
• Five copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater only, and it specifies your obligations with respect to stormwater
discharge controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself with all
the changes in the reissued permit.
The more significant changes in the General Permit since the your last Certificate of Coverage include the following:
• Permit cover page — Coverage is expanded to include industrial activities similar to the categorically identified industries.
• Part I Section A — This part explains that the No Exposure Exclusion from permitting option may be available to the
permittee.
• Part II Section A — Several minor expansions and clarifications of the required content of the Stormwater Pollution
Prevention Plan have been added. Please be sure that your plan is updated to comply with the new requirements.
• Part 11 Section B — Qualitative monitoring must now be conducted during a representative storm event.
• Part 11 Section B — The permittee is required to keep a written record of his response to problems identified by his
qualitative monitoring observations.
• Part I I Section C - For facilities with vehicle maintenance activities using more than 55 gallons of new motor oil per month,
new provision; require that discharges from the vehicle maintenance areas be sampled twice per year, and that the
sampling results be compared to new numerical benchmark values. (The previous_ cutoff concentrations have been
eliminated.)
• Part 11 Section C — For facilities with vehicle maintenance activities using more than 55 gallons of new motor oil per month,
new provisions require the permittee to execute Tier One and Tier Two response actions, based on the first benchmark
exceedence ('± ier One) and the second consecutive benchmark exceedence (Tier Two). Tier Two requires that the
permittee institute monthly monitoring instead of twice -per -year monitoring, until three consecutive monitoring events
show no benchmark exceedences.
• Part I I Section C — This part contains a new provision that four exceedences of any particular benchmark will trigger
increased DWQ involvement.in the permittee's stormwater management and control actions. DWQ may direct the
permittee to apply for an individual permit, or. may direct the implementation or installation of specific stormwater control
measures.

Your coverage under the General Permit is transferable only through the specific action of DWQ.
This permit does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it relieve
the permittee from responsibility for compliance with any other applicable federal, state, or local law, rule, standard, ordinance,
order, judgment, or decree.

other applicable federal, state, or local law, rule, standard, ordinance, order, judgment, or decree.

Your facility is currently covered for stormwater discharge under General Permit NCG110000. This permit expires
on March 31, 2003. The Division staff is currently in the process of rewriting this permit and is scheduled to have
the permit reissued by early spring of 2003. Once the permit is reissued, your facility would be eligible for
continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in
advance that your permit coverage will be expiring. Enclosed you will find a Permit Coverage Renewal
Application Form. The application must be completed and returned by October 2, 2002 in order to assure
continued coverage under the general permit. Due to staff and budget constraints, letters confirming our receipt of
the completed application will not be sent.
Failure to request renewal within the time period specified, may result in a civil assessment of at least $250.00.
Larger penalties may be assessed depending on the delinquency of the request- Discharge of stormwater from your
facility without coverage under a valid Stormwater NPDES permit would constitute a violation of NCGS 143-215.1
and could result in assessments of civil penalties of up to $10,000 per day.
Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial
facilities in any of the 11 categories of "storm water discharges associated with industrial activity," (except
construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facility industrial
materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. For additional

If the subject Stormwater discharge to waters of the state has been terminated, please complete the enclosed
