We are in receipt of the Stormwater Pollution Prevention Plan (SWPPP) that you prepared and dated August 18,
2013. Our office received it a month later, and I apologize for the time that it has taken to respond.
The plan as submitted does not meet the requirements of an SWPPP. The band drawn sketch does not provide
adequate detail and does not provide a scale. There is also nothing documented about Employee Training or records
of Preventative Maintenance and Good Housekeeping, just to highlight a couple of other SWPPP components.
In order to provide some guidance, the following is the outline in the NCG100000 General Permit:
The Site Overview shall provide a description of the physical facility and the potential pollutant sources that
may be expected to contribute to contamination of stormwater discharges. The Site Overview shall contain
the following:
(a) A general location map (USGS quadrangle map or appropriately drafted equivalent map),
showing the facility's location in relation to transportation routes and surface waters; the
name of the receiving waters to which the stormwater outfalls discharge, or if the discharge
is to a municipal separate storm sewer system, the name of the municipality and the
ultimate receiving waters; and accurate latitude and longitude of the points of stormwater
discharge associated with industrial activity. The general location map (or alternatively the
site map) shall identify whether any receiving waters are impaired (on the state's 303(d)
list of impaired waters) or if the site is located in a watershed for which a TMDL has been
established, and what the parameters of concern are.
(b) A narrative description of storage practices, loading and unloading activities, outdoor
process areas, dust or particulate generating or control processes, and waste disposal
practices. A narrative description of the potential pollutants that could be expected to be
present in the stormwater discharge from each outfall.
(c) A site map drawn at a scale sufficient to clearly depict: the site property boundary; the
stormwater discharge outfalls; all on -site and adjacent surface waters and wetlands;
industrial activity areas (including storage of materials, disposal areas, process areas,
loading and unloading areas, and haul roads); site topography and finished grade; all
drainage features and structures; drainage area boundaries and total contributing area for
each outfall; direction of flow in each drainage area; industrial activities occurring in each
drainage area; buildings; stormwater Best Management Practices (BMPs); and impervious
surfaces. The site map must indicate the percentage of each drainage area that is
impervious, and the site map must include a graphic scale indication and north arrow.

(d) A list of significant spills or leaks of pollutants during the previous three (3) years and any corrective
actions taken to mitigate spill impacts.
(e) Certification that the stormwater outfalls have been evaluated for the presence of nonstormwater
discharges. The pernuttee shall recertify annually that the stormwater outfalls have been evaluated
for the presence of nonstormwater discharges. The certification statement will be signed in
accordance with the requirements found in Part ill, Standard Conditions, Section B, Paragraph 3.
We advise you to consider hiring an environmental consultant to step you through the process. If that's not an
alternative, you need to develop a more detailed plan, incorporate inspection records (or at least templates where
you're going to record them), and create more appropriate maps.

Materials used in the maintenace area is Oil Drain Pans, Parts Washer,Maintained by Safety Kleen

We use seperate containers for Waste oil and Solvents..
Antifreeze, gasoline, used oil and Solvents are held in Barrells and picked up by Noble oil Services.
Greasy rags, and uniforms are picked up weekly by Cintas Corp.
Invoices are given for picked up Waste Materials.
Battery's are picked up bi-weekly by Interstate Batteries.
Oil filters are drained and returned to cars for crushing, which is done offsite.
Used fluids are held in container's until picked up.
Maintenace area is inspected through out the day.
Used parts remain on vehicle until sold.
New parts are stored in parts Dept.
Parts washing is conducted inside building, in Parts Washer.
Vehicles loaded and unloaded in the drive way, beside the shop..Vehicle are brought inside shop for
draining..
and fluids put into containers, nothing is spilled on the ground..
No spills in the last 3 years, or at anytime..
Feasibility = We are doing every thing we are suppose to do and more..
Secondary Containment Requirements, We use 250 Gallon drums, for Noble Oil co.

On July 31, 2013, Joanne Steenhuis of the Division of Water Quality (Division) from the Wilmington
Regional Office conducted a compliance inspection of your facility located off Gum Branch Road in
Jacksonville, Onslow County. The Division appreciates the assistance you provided during the inspection
procedures.
This site was issued an NPDES Stormwater General Permit (No. 100205) on June 11, 2010. The permit
requires a written Stormwater Pollution Prevention Plan within 12 months of receiving coverage under
the General Permit. The permit also requires quantitative monitoring of stormwater discharges from
the site. Failure to follow your permit requirements is a violation of North Carolina General Statute 143-
215.1(a). Accordingly, the following observations and permit condition violations were noted during the
Division of Water Quality inspection and subsequent file review:
1) Stormwater Pollution Prevention Plan (SPPP)
A Stormwater Pollution Prevention Plan (SPPP) has not been developed, recorded, and properly
implemented. The failure to develop this plan represents a violation of the requirements specified in
Part II. Section A. 1-10 of the NPDES General Stormwater Permit 100000.
2) Analytical Monitoring
Analytical monitoring has not been conducted and recorded in accordance with permit requirements.
The failure to conduct analytical monitoring represents a violation of the requirements specified in
Part II. Section B of the NPDES General Stormwater Permit 100000.

31 Qualitative Monitoring
Qualitative monitoring has not been conducted and recorded in accordance with permit
requirements. The failure to conduct qualitative monitoring represents a violation of the
requirements specified in Part II. Section C of the NPDES General Stormwater Permit 100000.
Requested Response
1. This office requests your immediate attention to the development of a Stormwater Pollution
Prevention Plan (SWPPP) for this facility. The SWPPP should be developed in no less than 60
days from receipt of this notice.
2. Start collecting, documenting and maintaining the required data as stipulated in your permit
(See Part II, Section B, Table 1 and 2 and Section D).
Thank you for your attention to this matter. This office requires that the violations, as detailed above,
be properly resolved. Your efforts to undertake activities to bring the subject site back into compliance
is not an admission, rather it is an action that must be taken in order to begin to resolve ongoing
environmental issues.
Pursuant to G.S. 143-215.6A, these violations and any future violations are subject to a civil penalty
assessment of up to a maximum of $25,000.00 per day for each violation. Your above -mentioned
response to this correspondence, the degree and extent of harm to the environment and the duration
and gravity of the violation(s) will be considered in any civil penalty assessment process that may t

Comment: The facility does not have a written plan. This was discussed with the
owner. He was told that a written plan is required and that he would need to work on
writing on up and start implementing it.
The site is well vegetated and all drainage goes to a pond at the back of the lot which
drains to a ditch and then to surface waters. No stained soils or sheens were noted
during the inspection.

Comment: The owner showed the inspector where water was draining onto the site
from an adjacent property. The site has been bermed to separate the water coming off

On July 25, 2013 staff from the Wilmington Regional Office of the Division of Water Quality
conducted a site inspection of Bogue Sound Septic & Grading, Inc. / Howard Pit located on
Highway 24 in Onslow County, NC. The purpose of the inspection was to follow up on the
Notice of Violation you were issued on May 2, 2013 for your NPDES General
Stormwater/Wastewater Permit NCG020537 for discharges from your mine site in Onslow
County.
'As a result of the site inspection and a file review,
The following items are no Ionger under Notice of Violation.
Item I The sediment that was discharged into the wetlands from mine dewatering activity has
been satisfactorily removed.
Item II The issues regarding the haul road within the 50 foot buffer designated as "undisturbed"
on the DLR Mine Permit is in the process of being resolved through the Division of Land
Quality.
The follow items are Continuing NCG020000 Permit Condition Violations:
Item I Stormwater Pollution Prevention Plan
Part III Section A of Stormwater General Permit Number NCG02000 states that the
permittee shall develop and implement a Stormwater Pollution Prevention Plan (hereby
known as SPPP). The DWQ site visit and required response revealed that no SPPP is
available for this site.
Item II Analytical Monitoring Part III Section E of Stormwater General Permit Number
NCG02000 requires that Analytica! Monitoring be conducted for mine dewatering
wastewater discharges. The DWQ site visit and file review revealed that the required
Analytical Monitoring has not occurred.
Enclosed please find a copy of the NPDES GENERAL PERMIT NO. NCG020000 TO
DISCHARGE STORMWATER, MINE DEWATERING, AND PROCESS WASTEWATER. The
sections of the permit pertaining to the SPPP and Analytical Monitoring requirements have been
highlighted in yellow for your attention. Please developed and implement a Stormwater
Pollution Prevention Plan for the site and begin analytical monitoring of the dewatering
discharge according to the permit requirements in Parts III and IV Section E.

to receiving waters designated as UT to the New River, a class C, NSW water in the White Oak
River Basin, in accordance with the effluent limitations, monitoring requirements, and other
conditions set forth in Parts I, 11, 111, IV, V, and VI of General Pen -nit No. NCG100000 as
attached.
This certificate of coverage shall become effective June 11, 2010.
This Certificate of Coverage shall remain in effect for the duration of the General Pen -nit.

On January 7, 2010, Linda Willis and Jean Conway of the DWQ Wilmington Regional Office conducted
a complaint investigation at East Coast Imports and observed that East Coast Imports was operating a
used motor vehicle parts business without approved General Stormwater Permit NCG100000. This site
is within the New River watershed and located in the White Oak River Basin.
A letter was sent to you on January 25, 2010 requesting you apply for the NPDES General Permit within
30 business days of receipt of the notification. To date, no response or application has been made by
East Coast Imports.
As a result of the site visit and file review, the following violation is detailed below:
(1) Operating this facility without a stormwater permit
Item 1. Operating a facility without a stormwater permit
It is illegal to operate certain industrial plants without a permit under North Carolina General Statute
(NCGS) 143-215.1(a). Illegal operation is subject to civil penalties of up to $25,000 per day according
to NCGS 143-215.6A(a)(2). NPDES general permit NCG100000 has been established pursuant to
NCGS 143-215.1(b) for discharges of stormwater to surface waters. This general permit is applicable to
owners or operators of stormwater point source discharges associated with activities classified as
establishments primarily engaged in used motor vehicle parts. We believe this plant was operating at
the time of inspection.

'rhe Division of Water Quality, Wilmington Regional Office, Surface Water Protection
Section, received a complaint from an anonymous caller on January 7, 2011 concerning a
discharge of oil from an oil storage tank at your place of business, East Coast Imports,
located at 6315 Gum Branch Road in Jacksonville. Division of Water Quality
Representatives, Linda Willis and Jean Conway, with the Wilmington Regional Office,
conducted the complaint investigation on January 12, 2010. According to my staff, no oil
sheens were seen on any stormwater accumulation on site or in the peripheral ditches
around the facility. Staff indicate that the facility appeared to maintain better
housekeeping than the majority of the salvage yard facilities in our region. You are
commended for your efforts in maintaining good housekeeping practices. This complaint
will be documented in the Division's database as unsubstantiated. A review of the.
Division's database did reveal that this facility does not have an NPDES Permit for
stormwater discharges.
Enclosed is an application form called the "Notice of Intent". Should you have any
questions concerning this application form, please contact Linda Willis at 910-796-7343
for assistance. Your application for the permit should be received by the Division within
30 business days of receipt of this notification.

Hi Linda,
We've received an NOI from E. Coast Imports, Inc. for their facility in Jacksonville (Onslow
County) for coverage of their used car vehicle parts place under NCG10. The site discharges
stormwater into a UT to New River ( C, NSW). The NOI is attached.
Does the Wilmington Regional Office have any concerns about issuing this facility a COC for
this general permit? If we don't receive any objections, we'll issue the COC in 30 days (June
12, 2010).
