On February 5, 2013, Mike Mickey of this office met with Billy Wyrick to perform a NPDES
General Stormwater Permit Inspection at the Triad Auto Parts operation located off Alamance Church
Road and Hacketts Lake Road. The facility was found to be non -compliant with the recordkeeping
requirements of the permit as outlined in Items 2 and 5 below. An inspection checklist is also attached for
your records.
1. Permit
Triad Auto Parts holds NPDES Stormwater Permit No. NCGl00203 for the discharge of stormwater from
the facility. The permit was recently reissued on November 1, 2012.. Please note that the Hacketts Lake
Road operation also falls under the Stormwater Permit. The permit requirements for analytical sampling,
qualitative monitoring and Stormwater Pollution Prevention Plan apply to both sites.
2. Records/Reports
Part I1, Section A of the permit requires the development -of a Stormwater Pollution Prevention Plan
(SPPP). To date, a SPPP has not been prepared for the Triad Auto Parts operation. A Plan needs to be
prepared that is. specific for the Triad Auto Parts facility and the Plan should follow the outline provided in
Section A mentioned above. The SPPP may be prepared in-house or you may retain the services of a
consultant to assist you.
3. Facility Site Review
A tour of the Alamance Church Road location found the area to be clean and well managed. All fluids
were stored properly (oil is burned for heat and anti -freeze is recycled) and most parts are stored indoors.
The parts washing unit that recycles wastewater was out of service for refurbishing. All interior and
exterior concrete surfaces were free of any oil stains.

The Hacketts Lake Road location is used for auto storage and crushing for scrap. The storage lots had
been covered with, gravel and used batteries, etc., are stored under roof. There were some minor oil stains
on the soil behind the roll off dumpster.
4. Effluent/Receiving Waters
Both operations have a stormwater basin that receives runoff from each site. Stormwater from the
Alamance Church Road site (Outfall 91) and the Hasketts Lake Road site (Outfall 42) eventually reach
Little Alamance Creek, class "WS-IV; NSW" waters in the Cape Fear Basin. Observations of both
outfalls found them to be clean and free of any accumulated debris, stains or sheens.
5. Monitoring Program
Part II, Section B of the permit requires the collection of semi-annual analytical samples of the stormwater
discharge during a measurable storm event. The file review showed that this monitoring was performed as
required for the Alamance Church Road site. The last three monitoring events for which data was
available occurred on March 10, 2011, September 23, 2011 and June 6, 2012. The December 2012 results
have yet to be received from James Environmental. All values were below the benchmark levels specified
in Table 3 of the permit. No analytical monitoring has been performed for the Hasketts Lake Road
operation. Please make sure this monitoring begins in 2013. The monitoring results should be reported to
the Division of Water Quality (see form SWU-25 I -Revised 10/25/12 attached) within 30 days of
receiving the results from your certified laboratory.
Part II, Section D of the permit requires that qualitative monitoring be performed and documented semi-
annually. This monitoring was not being performed for either site. This visual monitoring should occur at
the same time of the analytical sampling events. The qualitative records do not need to be .submitted to
the Division, however, the records must be kept on file at the facility for a period of five years.
In summary, Triad Auto Parts needs to prepare a SPPP and begin qualitative monitoring for both
operations, as well as to begin analytical monitoring at the-Hasketts Lake Road location. You are
requested to respond in writing to this office on or before March 15, 2013, with a proposed timetable for
completion of the above items. Please be aware that non-compliance with the stormwater conditions
contained in your permit could subject Triad Auto Parts to civil penalties of up to $25,000 per day per
violation.

Comment: Part 11, Section B of the permit requires the collection of semi-annual
analytical samples during a measurable storm event. This monitoring was performed
for the Alamance Church Road site on 3/10111, 9/23/11 and 6/6112. Sampling occurs at
a single outfall (#1) located behind the building. All results were below the benchmark
levels established in the permit.
Analytical monitoring for the Hasketts Lake Road site has not been conducted.
Samples should be collected from the large sediment basin discharge pipe (Outfall #2)
per permit requirements.
Please note that all analytical sample results must be reported to the Division on the
proper forms no later than 30 days after receiving the sample results from the laboratory.

Comment: The facility has two outfalls that require sampling, one at the Alamance
Church Road site and one at the Hasketts Lake Road site. Observations of both
outfalls did not reveal any concerns.
Stormwater from both sites drains to Little Alamance Creek, class VS -IV; NSW" waters
in the Cape Fear River Basin.

In response to your renewal application for continued coverage under stormwater General Permit NCG100000
the Division of Water Quality (DWQ) is forwarding herewith the reissued General Permit. This permit is
reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October
15, 2007 (or as subsequently amended).
The following information is included with your permit package:
• A new Certificate of Coverage (COC)
• A copy of General Permit NCG1o0000
• A copy of the Technical Bulletin for the General Permit
• Two copies of the Discharge Monitoring Report (DMR) Form
• Two copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater, and it specifies your obligations for discharge
controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself
with all the changes in the reissued permit. Your facility has six (6) months from the time of receipt of the
permit to update your current SPPP to reflect all new permit requirements.
The first sample period of your permit begins January 1, 2013. Your facility must sample a "measureable
storm event" beginning during the periods beginning January 1 and July 1 of every year (or, if applicable,
report "No Flow," as outlined in Part III, Section E). Also, please note that Tier 3 Actions in Part II of your
permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this
permit and do not count prior exceedances.
The more significant changes in the General Permit since your last COC was issued are noted either in the
Draft Permit Fact Sheet that accompanied the public notice (http://portal.ncdenr.orp/web/wq/ws/su/current-
notices), or in the Response to Comments / Summary of Changes and Technical Bulletin documents that are
posted on the Stormwater Permitting Unit's website with the new General Permit. Please visit

for your specific General Permit carefully.

Some of the changes include:
• Part II:
• Section A: The Stormwater Pollution Prevention Plan (SPPP) section, if applicable, has been updated
to the most current language of our permits. Additional conditions for specific industry sectors have
been added to the SPPP requirements in some cases.
• Sections B, C: Failure to perform analytical stormwater monitoring may result in the Division requiring
that the permittee begin a monthly sampling scheme.
• Sections B, C: A lower TSS benchmark of 50 mg/I for HQW, ORW, PNA and Tr Waters applies to these
more sensitive waters.
• Sections 8, C. The monitoring parameter Oil & Grease (O&G) has been replaced by the parameter
Total Petroleum Hydrocarbons (TPH) for vehicle maintenance areas, and in some cases, other
analytical monitoring requirements.
• Sections B, C, D: Inability to sample due to adverse weather must be recorded in the SPPP, or in
separate on -site records if your General Permit does not require an SPPP. Adverse weather is defined
in the "Definitions" section of the permit.
• Sections B, C. The term "Representative Storm Event" has been replaced by "Measurable Storm
Event." A measurable storm event is defined in the permit.
• Section D: If the permittee fails to respond effectively to problems identified by qualitative
monitoring, DWQ may require the permittee to perform corrective action.
Please review Parts III and IV to understand the Standard Conditions of your new NPDES General Permit,
including Compliance and Liability, Reporting, Monitoring and Records requirements; Operation and

of this permit must be submitted to the Division on Discharge Monitoring Report (DMR) forms available on
the Stormwater Permitting Unit's website above. DMR forms must be delivered to the Division no later than
30 days from the date the facility receives the sampling results from the laboratory. Also note that existing
permittees do not need to submit a renewal request prior to expiration unless directed by the Division.
Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit
does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it
relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law,

Hi Corey,
We've received an NOI from Triad Auto Parts, LLC. for their facility in Greensboro (Guilford County) for coverage of their Automobile Wrecking for Scrap Yard
under NCG10 (Automobile Scrap Yard permit). The site discharges stormwater into Little Alamance Creek (WS-IV, NSW). The NOI is attached.
A couple of things of note:
• They seem like they have a lot of used oils and fluids on site. They also need to have secondary containment.
• They state in their application that they have no hazard oug .I believe used motor of as up by a Haz. Waste company?
A site visit is rPcommended befoCaj-&sake of this permit.
Does the Winston-Salem Regional Office have any concerns about issuing this facility a COC for this general permit? If we don't receive any obje' tions, we'll
issue the COC in 30 days.
