This inspection consists of review of the facility outfalls and possible exposures. Your assistance with inspection was greatly appreciated. Permit: This facility holds General Stormwater Permit NCG100131 to discharge stormwater from industrial activity associated with Used Motor Vehicle Parts and Automobile Wrecking for Scrap under the National Pollutant Discharge Elimination. System (NPDES). The permit became effective November 01, 2012 and expires on October 31, 2017. The current permit was available for review and was included in the SPPP. Records/Revorts: This facility was required to develop and maintain a Stormwater Pollution Prevention Plan (SPPP) in accordance with Part II, Section A of the permit. All qualitative and analytical monitoring records are required to be maintained with the SPPP for a minimum of five (5) years. Records appear to be current and complete. Facilily Site Review: This facility is a full service scrap metal and used auto parts dealer. When vehicles are brought onto the site, fluids are removed under roof. Used oil and transmission fluid are used for heat. Good gasolineds used in service vehicles. Bad gasoline is picked up for recycling. Tire go to recycling as well. Box 7 Pfafftown, NC 27040 Subject: Compliance Evaluation Inspection & Notice of Deficiency NPDES General Stormwater Permit NCG100000 Certificate of Coverage NCG100131 67 Motors, Pfafftown Forsyth County, Dear Mr. White: Ron Boone of the Winston-Salem Regional Office of the NC Division of Water Quality (DWQ or the Division) conducted a compliance evaluation inspection (CEI) at the subject facility on March 29, 2012. Your assistance and cooperation was greatly appreciated. An inspection checklist is attached for your records and the inspection findings are summarized below. The facility is located at 6775 Reynolda Road, in Pfafftown, Forsyth County, North Carolina, at approximate coordinates 36.1861790,-80.3680340.. According to you, stormwater related to industrial activity is not typically discharged from the site; point source discharges from the site are averted through a combination of employing sheet flow through heavily vegetated areas, diversion berms, and sediment traps. This is discussed in further detail below under "Other Issues". If and when there is a discharge from the site it would go to Mill Creek No 3, which is currently classified as a Class C water located in the Yadkin Pee -Dee River Basin. This contrasts with what is currently listed on the Certificate of Coverage (COC) as the receiving stream. The COC lists Bashavia Creek as the receiving stream but Mr. Boone found this to be incorrect. Action has already been initiated to change the receiving stream to Mill Creek No 3 in DWQ's database and on your COC. Documentation & Monitoring Review The Stormwater Pollution Prevention Plan (SP3) and monitoring activities were reviewed. The following deficiencies were identified: The facility does not have a general location map (GLM) as required by Part II, Section A,. Paragraph 1(a),of the general permit. You do have an aerial photograph of the facility. 2. The facility does not have a site map (SM) as required by Part II, Section A, Paragraph 1(c) of the general permit. 3. . The facility does not have a list of significant spills that have occurred over the past three years and according to you and your records, none have occurred. As long as accurate records are kept of all spills, then this is considered to meet the requirements of Part II, Section A, Paragraphs 1(d) and 7, as they pertain to spills. Since Part II, Section A, Paragraph 7 requires you to update the spill list annually, you should, at least annually, use the spill form to annotate that there have not been any spills over the previous year. 4. Stormwater discharge outfall (SDO) certification, as required by Part II, Section A, Paragraphs 1(e) and 7, of the general permit, has not been accomplished. 5. The SP3 does not contain a feasibility study as required by Part 11, Section A, Paragraph 2(a), of the general permit. 6. The Spill Prevention and Response Plan (SPRP), as required in Part II, Section A, Paragraph 3, of the general permit, was found to be satisfactory except that Mr. Boone strongly suggests you add the following list of emergency contacts within your SPRP: a. NC Division of Water Quality, Surface Water Protection Section, Winston-Salem Regional Office (this office): 336-771-5000, 8 AM to 5 PM, Monday through Friday. After hours and weekends: 1- 800-858-0368. b. Environmental contractors you would hire to aid in spill clean-up in the event of a large spill. c. City and County Environmental and Stormwater officials. d. Emergency services personnel, such as the fire department. e. The EPA National Response Center, 1-800-424-8802. 7. SP3 annual reviews, as required in Part II, Section A, Paragraph 7 of the general permit, are not being conducted. 8. No analytical or qualitative monitoring has been accomplished because, as discussed above, you have stated that there is no discharge from the. site. This is discussed in further detail below. Site Review The site was fairly clean and well maintained. The BMPs used at your site appear to be effective. Mr. Boone noted no deficiencies. Other Issues As mentioned above, according to you,- stormwater related to industrial activity is not typically discharged from the site; point source discharges from the site are averted through a combination of employing sheet flow through heavily vegetated areas, diversion berms, and sediment traps. After inspecting the entire site, Mr. Boone did find one potential point source, located in the very bottom of the ravine at approximate coordinates 36.1880050,-80.3672740'. All other runoff appears to be entrapped within diversion berms and sediment traps that are strategically located throughout the site, as well as sheet flow through heavily vegetated and fairly flat areas. Please monitor the point identified above, at the bottom of the ravine, during the next heavy rain event to observe whether a point source discharge actually develops. Regardless of whether or not point sources are identified on your site through our combined evaluations, you should be performing qualitative monitoring (QM) semi-annually during representative storm events as defined in Part II, Section C, of the general permit. During these QM events you should monitor the sediment traps to ensure no -discharge develops and overtops. the berms. The QM monitoring forms should be used to annotate your observations during these events: Your notes should indicate whether a discharge develops, and if it does you should evaluate it for the presence of the nine indicators listed in QM table 4, in 67 Motors Attn: Greg White, Manager Compliance Evaluation Inspection, NCG100131 67 Motors, Pfafftown 3/29/2012, Page 3 of 3 Part II, Section C of the permit. If point source discharges develop at these locations then you will need to begin the analytical monitoring as required in Part II, Section B, of the general permit immediately. Please correct all deficiencies noted above as soon as practical. Please reply in writing to this letter within 30 business days of receiving it. Your written response should include proposed corrective actions along with estimated completion dates for each of the deficiencies noted above. Please note that violations of the NCG100000 general permit are subject to civil penalty assessments not to exceed $25,000 per day, per violation. Your written reply will be used, in part, to determine whether further enforcement actions are necessary. Secondary Containment - spill containment for the contents of the single largest tank within the containment structure plus sufficient freeboard to allow for 25 yr, 24 hr storm event 5-6 in . Accumulated stormwater shall be released if found to be uncontaminated by the material stored within the containment area. Records documenting the individual making the observation, the description of the accumulated stormwater and the date and time of the release shall be kept fora period of five years. c. BM P Summary. A narrative description shall be provided of BMP's to be considered such as, but not limited to, oil and grease separation, debris control, vegetative filter strips, infiltration and stormwater detention or retention, where necessary. The need for structural BMP's shall be based on the assessment of potential sources to contribute significant quantities of pollutants to stormwater discharges and data collected through monitoring of stormwater discharges. d. InsneCtiOR Schedule Schedules for inspection of stormwater conveyances, controls, and measures to be taken to limit or prevent erosion associated with the stormwater systems. 3. Spill Prevention and Response Plan. The Spill Prevention and Response Plan (SPRP) shall incorporate an assessment of potential pollutant sources based on a materials inventory of the facili(y. Materials inventory Facility personnel (or team) responsible for implementing the SPRP identified. A responsible person — shall be on site at all times during facility operations that might have the potential to contaminate stormwater runoff through s ills or exposure of material associated with the facility operations. SPRP must be site stormwater specific. SPCC may be a component of SPRP. Areas where spills can occur and their discharge points. Spill response team Spill response equipment and locations. Regular cleaning schedules of material handling areas. 5. Employee Training. Training schedules shall be developed and training provided at a minimum of an annual basis on proper spill response and cleanup procedures and preventative maintenance activities for all personnel involved in any of the facility's o )erations that have the. otential to contaminate stormwater runoff. The SPPP shall identify a specific position(s) responsible for the overall coordination, development, implementation, and revision to the Plan. The permittee shall amend the plan whenever there is a change in design, construction, operation, or maintenance which has a significant effect on the potential for the discharge of pollutants to surface waters.
