Please note that no samples were collected at the facility during the 2015 second semi-annual monitoring period. A sample was collected during the first qualifying storm event discharge in 2016 in lieu of the sample for the 21d semi-annual monitoring period of 2015. As such, this submittal is intended to satisfy the monitoring requirements for the second semiannual monitoring period for 2015. A sample for the first semi-annual monitoring period of 2016 will be collected later in the second quarter and the results will be submitted to the Division of Water Quality later in the year. You are required by the permit to submit your SDO Monitoring Report to the Division of Water Quality (DWQ). A cover letter to the DWQ for transmittal of the SDO Monitoring Report is also attached here for your convenience. Please print two copies of the cover letter and two copies of the SDO Monitoring Report, and sign and date inside the gray boxes of the cover letter. Please also sign and date the certification sections on the second pie of the SDO Monitoring Report (note: you will need to sign both the Solvent Management Plan certification on the top of the second page, and perilittee certification the bottom on the second page of the SDO Monitoring Report).
