On January 21, 2016 Glen White of the North Carolina Department of Environmental Quality;
Department of Energy, Minerals and Land Resources met with Sandy McDowell, Secretary at
the subject facility located at 1137 Mountain View Church Road, Asheboro, NC to conduct a
compliance inspection for the facilities Industrial Stormwater Permit. Inspection consist of
review of the Stormwater Pollution Prevention Plan, training and monitoring records and
observation of the general condition of the site and ouifall. Her assistance with records review
was greatly appreciated as was Mike Hunt's. He assisted with review of the outfall.
Permit:
This facility holds General Stormwater Permit NCG100039to discharge Stormwater from
industrial activity associated with Used Motor Vehicle Parts and Automobile Wrecking for
Scrap under the National Pollutant Discharge Elimination System (NPDES). The permit
became effective November 1, 2012 and expires on October 31, 2017. A copy of the current
permit is required to be maintained with the Stormwater Pollution Prevention Plan (SPPP)
documents. A copy of the current permit was available for review at the time of inspection.
Records/Reports:
This facility was required to develop and maintain a Stormwater Pollution Prevention Plan
(SPPP) in accordance with Part II, Section A of the permit. The SPPP was was well prepared, up
to date and available for review at the time of inspection. SPPP includes documentation for
employee training per the permit. No deficiencies noted.
Facility Site Review:
Vehicles are brought onto the site to be salvaged. All fluids, mercury switches and batteries are
removed and stored under cover until they can be removed by a licensed hauler. Vehicles are
then inventoried and placed on the yard for salvage.

Effluent Receiving Waters:
The facility has representative outfall status, Two tributaries of Caraway Creek converge near
the property boundary. This location is also the testing site. Caraway Creek is classified as
Class "C" waters of the Yadkin Pee -Dee River Basin.
Self-Monitorin Program:
The facility is required to perform qualitative and analytical monitoring twice annually. Records
show that monitoring has been performed per permit requirements for the past several years. No
discrepancy noted.
The facility was found to be compliant with its stormwater permit requirements. Please include a
copy of this report in your SPPP records. Again, thank you for your time and assistance with

On November 6, 2013, Sue White of this office performed a Stormwater Permit
Inspection at Hunt's Auto Parts, Inc. located at 1137 Mountain View Church Road, Asheboro,
NC in Randolph County. You were present for the inspection. This facility holds General
Stormwater Permit No. NCG100039 to discharge Stormwater from activities associated with
Used Motor Vehicle Parts under the National Pollutant Discharge Elimination System (NPDES).
1. Permit
This facility holds Certificate of Coverage #NCG100039 for the discharge of Stormwater for the
site. The permit became effective November 1, 2012 and expires October 31, 2017.
2. Records/Reports
Part i1, Section A of the permit requires the development of a Stormwater Pollution Prevention
Plan (SPPP). During the inspection, it was determined that your facility has a developed SPPP
prepared by Mid -Atlantic Associates. All required information was included in this plan.
3. Facility Site Review
This site was clean and well managed. When vehicles are brought to the site to be salvaged,
fluids, mercury switches and batteries are removed and stored properly until removed from the
site by a licensed hauler. Vehicles are then inventoried and put in the yard for salvage. This
site is well organized and it is obvious that the owners and employees implement permit
requirements. This facility has two outfalls but is covered under Representative Outfall Status
(ROS).
4. Effluent/Receiving Waters
Stormwater from the property discharges to an un-named tributary to the Caraway Creek (Class
C) in the Yadkin/Pee Dee River Basin,
5. Self -Monitoring Program
It was determined during the inspection that you have been performing the self -monitoring
requirements of the General Permit. Monitoring requirements include semi-annual analytical
sampling and qualitative monitoring at each Stormwater discharge outfail on the property. The
analysis of these samples are being performed at a NC Certified Lab and recorded per permit
requirements. Please be aware that Representative Outfall Status allows you to do the

In response to your renewal application for continued coverage under stormwater General Permit NCGlO0000
the Division of Water Quality (DWQ) is forwarding herewith the reissued General Permit. This permit is
reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October
15, 2007 (or as subsequently amended).
The following information is included with your permit package:
• A new Certificate of Coverage (COC)
• A copy of General Permit NCG100000
• A copy of the Technical Bulletin for the General Permit
• Two copies of the Discharge Monitoring Report (DMR) Form
• Two copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater, and it specifies your obligations for discharge
controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself
with all the changes in the reissued permit. Your facility has six (6) months from the time of receipt of the
permit to update your current SPPP to reflect all new permit requirements.
The first sample period of your permit begins January 1, 2013. Your facility must sample a "measureable
storm event" beginning during the periods beginning January 1 and July 1 of every year (or, if applicable,
report "No Flow," as outlined in Part III, Section E). Also, please note that Tier 3 Actions in Part II of your
permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this
permit and do not count prior exceedances.
The more significant changes in the General Permit since your last COC was issued are noted either in the
Draft Permit Fact Sheet that accompanied the public notice (http://portal.ncdenr.org/web/wq/ws/su/current-
notices), or in the Response to Comments / Summary of Changes and Technical Bulletin documents that are

Some of the changes include:
• Part II:
• Section A: The Stormwater Pollution Prevention Plan (SPPP) section, if applicable, has been updated
to the most current language of our permits. Additional conditions for specific industry sectors have
been added to the SPPP requirements in some cases.
• Sections B, C: Failure to perform analytical stormwater monitoring may result in the Division requiring
that the permittee begin a monthly sampling scheme.
• Sections B, C: A lower TSS benchmark of SO mg/I for HQW, ORW, PNA and Tr Waters applies to these
more sensitive waters.
• Sections B, C: The monitoring parameter Oil & Grease (O&G) has been replaced by the parameter
Total Petroleum Hydrocarbons (TPH) for vehicle maintenance areas, and in some cases, other
analytical monitoring requirements.
• Sections B. C, D: Inability to sample due to adverse weather must be recorded in the SPPP, or in
separate on -site records if your General Permit does not require an SPPP. Adverse weather is defined
in the "Definitions" section of the permit.
• Sections B, C: The term "Representative Storm Event" has been replaced by "Measurable Storm
Event." A measurable storm event is defined in the permit.
• Section D: If the permittee fails to respond effectively to problems identified by qualitative
monitoring, DWQ may require the permittee to perform corrective action.
Please review Parts III and IV to understand the Standard Conditions of your new NPDES General Permit,
including Compliance and Liability, Reporting, Monitoring and Records requirements; Operation and
Maintenance obligations; and Definitions. Please note that all samples analyzed in accordance with the terms
of this permit must be submitted to the Division on Discharge Monitoring Report (DMR) forms available on
the Stormwater Permitting Unit's website above. DIVIR forms must be delivered to the Division no later than
30 days from the date the facility receives the sampling results from the laboratory. Also note that existing
permittees do not need to submit a renewal request prior to expiration unless directed by the Division.
Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit
does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it
relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law,

On behalf of Hunt's Auto Parts, Inc., Mid -Atlantic Associates, Inc. requests representative outfall
status for the above referenced site. The facility is permitted to discharge stormwater under the
North Carolina Department of Environment and Natural Resources, Division of Water Quality,
General -Permit NCG100000. Stormwater currently discharges off site at two outfails (See
Attached Drawing 2.1). Potential pollutants associated with stormwater discharges at each outfall
include oils, greases, petroleum, vehicle fluids, metals and suspended solids. Based on our recent
evaluation of the site and its stormwater outfalls, it appears that the stormwater discharge from
Outfall 1 is representative of discharges from each outfall at the site and would be the "worst -case"
discharge of the two. It also appears that discharge leaving the site at Outfall 2 eventually
converges with other site discharges at Outfall 1 after moving through an off site wooded area.

We received an ROS request from Hunt's Auto Parts in Randolph County
(NCG100039) that I'm forwarding to the WSRO. Our network connection to
the scanner is down this morning, so I'm not attaching a copy of the
letter, but,I'll get in the mail to the WSRO today. .I've updated BIMS
with the 'ROS Requested' event date.
I know Jenny used to be the contact for Randolph, but I think that might
have changed --- I've copied her on this note just in case but am sending

By the way, I'm about to finalize all the ROS Guidance -and get it out to
the ROs, so that's coming soon. There's also an event in RIMS now for

On October 9, 2008, Mike Mickey and Corey Basinger of this office met with you to
perform a General Stormwater Permit Inspection at Hunt's Auto Parts. This site visit was
prompted by the representative outfall status request submitted by Mid -Atlantic Associates,
Inc. The inspection evaluated the five (5) areas as designated on the attached inspection
form. Observations from each area are addressed below:
1. Permit
Hunt's Auto. Parts holds Certificate of Coverage # NCG100039 for the discharge of
stormwater from the site. The General Permit was reissued effective November 1, 2007 and
expires next on October 31, 2012. - `
2. Records/Reports
Part 11, Section, A of the permit requires the development of a Stormwater Pollution
Prevention Plan (SPPP)- A copy of the facility's SPPP was reviewed during the inspection.
This plan, prepared by Mid -Atlantic Associates, was thorough and complete.
3. Facility Site Review
The, site was clean and well managed. Fluids (motor oil, gasoline, antifreeze, etc) are
removed upon receipt of the vehicle and are stored properly for pick-up by Noble Oil.
Additionally, mercury switches and batteries are also removed from the vehicle.
4. EffluenUReceiving Waters
Stormwater from the Hunt's Auto Parts site is discharged from two separate outfalls that
ultimately reach an unnamed tributary to Caraway Creek, Class "C" Waters, in the Yadkin -
Pee Dee River Basin. The stormwater conveyances were free of any obvious contamination,

5. Monitoring Program
Part 11, Section B of the permit requires collection of semi-annual analytical samples at both
outfalls during a representative storm event. The first semi-annual sample for 2008 was
collected on June 30, 2008. The results were below the benchmark values established in
Table 3 of the permit. Please be aware that the permit establishes benchmark values that, if
exceeded, requires tiered responses that may involve increased best management practices,
inspections, record keeping and/or sampling. All analytical monitoring results must be
submitted to the Division on form SWU-251-031308 no later than 30 days from the date the
facility receives the sampling data from the laboratory.
Part 11, Section C of the permit requires that qualitative (visual) monitoring be performed and
documented semi-annually at the same time the analytical samples are collected. The file
review found that these observations are not being recorded as required. Attached is a copy
of the monitoring form. The qualitative records do not need to be submitted to the Division,
however, the records must be kept on file at the facility for a period of five years.
In regards to the representative outfall status request, our office will recommend approval in
a separate letter. Once granted, only outfall #001 will have to be sampled semi-annually.
Please note that the representative outfall status designation does not apply to the qualitative.
(visual) monitoring. You will still have, to perform visual monitoring at both outfalls semi-
annually.

Comment: This site visit was the result of a request for representative outfall status
determination for out -fall # 001. The facility has two outfalls. Outfall #001 receives .
stormwater from the majority of the site (vehicle storage areas, dismantling garage,
crushing area, diesel AST, 55-gallon drum shed and office), Outfall #002 receives
stormwater from a small corner of the property near Jarvis Miller Road and Mt. View
Church Road. This area contains rims, the new transmission storage building and some
empty fuel tanks. Observations of both outfalls did not reveal any concerns or problems
with the ditches that receive stormwater from the site. Our office has no problem with
granting representative outfafl status so that tyhe analytical monitoring is only required
to be performed on outfafl #001,

Dear Sir:
On behalf of Hunt's Auto Parts, Inc., Mid -Atlantic Associates, Inc. requests representative outfall
status for the above referenced site. The facility is permitted to discharge stormwater under the
North Carolina Department of Environment and Natural Resources, Division of Water Quality,
General Permit NCG100000. Stormwiater currently discharges off site at two outfalls (See
Attached Drawing 2.1). Potential pollutants associated with stormwater discharges at each outfall
include.oils, greases, petroleum, vehicle fluids, metals and suspended solids. Based on our recent
evaluation of the site and its stormwater outfails, it appears that the stormwater discharge from
Outfall 1 is representative of discharges from each outfall at the site and would be the "worst -case"
discharge of the two. It also appears that discharge leaving the site at Outfall 2. eventually
converges with other site discharges at Outfall 1 after moving through an off site wooded area.

This letter is to inform you that, as of the date of this letter, the Division of Water Quality has not received a
renewal request for the subject permit certificate of coverage. This is a violation of NCGS § 143.215.1 (c)(1) which
states "All applications shall be filed with the commission at least 180 days in advance of the date on which it is
desired to commence the discharge of wastes or the date on which an existing permit expires, as the case may be".
Any permittee that has not requested renewal at least 180 days prior to expiration or permittee that does not have a
permit after the expiration and has not requested renewal at least 180 days prior to expiration, will be subjected to
enforcement procedures as provided in NCGS § 143-215.6 and 33 USC t251 et. seq.
In order to prevent continued, escalated action, including the assessment of civil penalties you must submit a
completed permit coverage renewal application to the attention of the "Stormwater and General Permits Unit" at
the letterhead address within ten (10) days of your receipt of this letter (renewal application enclosed). If the
subject discharge has been terminated, please complete the enclosed rescission request form. Mailing instructions
are listed on the bottom of the form. You will be notified when the rescission process has been completed.
