On January 20, 2016 Glen White of the North Carolina Department of Environmental Quality;
Department of Energy, Minerals and Land Resources met with you at the subject facility located
at 9316 US Hwy Bus 220 N, Randleman, NC to conduct a compliance inspection for the
facilities Industrial Stormwater Permit. Inspection consist of review of the Stormwater Pollution
Prevention Plan, training and monitoring records and observation of the general condition of the
site and outfall. Your assistance with the inspection was greatly appreciated,
Permit:
This facility holds General Stormwater Permit NCG100023 to discharge stormwater from
industrial activity associated with Used Motor Vehicle Part & Automobile Wrecking for
Scrap under the National Pollutant Discharge Elimination System (NPDES). The permit
became effective November' 1; 2012 and expires on October 31, 2017. A copy of the current
permit is required to be maintained with the Stormwater Pollution Prevention Plan (SPPP)
documents. A copy of the current permit was available for review at the time of inspection.
Records/Deports:
This facility was required to develop and maintain a Stormwater Pollution Prevention Plan
(SPPP) in accordance with Part I1, Section A of the permit. The SPPP was available was well
prepared, complete, updated annually and was available for review at the time of inspection.
SPPP includes documentation for employee training per the permit. No deficiencies noted.
Facility Site Review:
The facility is a salvage yard for motor vehicle parts. Many clients are insurance companies and
automotive repair facilities. When vehicles are brought onto the site, fluids are removed and
stored under cover until removed by a licensed hauler. Vehicles are inventoried and placed on
the yard. Site appears to be clean and well maintained.

Effluent Receiving Waters:
Stormwater from the NE corner of the property discharges from Outfall 1. Stormwater from
Outfall 2 is discharged at the fence on the south side of the property. Both discharge to an
unnamed tributary of Polecat Creek. "Class WS-III" waters of the Cape Fear River Basin.
Outfall 3, located at Bus 220 flows to an unnamed tributary of Deep River "Class WS-IV, also
waters of the Cape Fear River Basin
Self -Monitoring Program:
The .facility is required to perform qualitative and analytical monitoring twice annually. Records
show that monitoring has been performed per permit requirements for the past several years.
Results appear to be within parameters and are consistent throughout. Suggest applying for
"Representative Outfall" Status. This would allow the facility to only provide bi-annual
analytical monitoring at one outfall rather than all three. Suggested outfall would be SD02,
located along the fence at the rear of the property. Qualitative monitoring would still be required
at all outfalls on a bi-annual basis. A copy of the application is included with this report for your
use should you choose to apply.
The facility was found to be compliant with its stormwater permit requirements. Please include a
copy of this report, in your SPPP records. Again, thank you for your time and assistance with

Comment: Site has 3 outfalls. Monitorina results are within reauired Darameters on each. Mr. McCaskill
walked the site with the inspector for observation of each outfall. This site appears to quality for
"Representative Outfall" Status. The outfall at the rear of the property recieves majority of
possible exposures and is the appropriate testing point if Representative Outfall status is to be
applied.

If a facility is required to sample multiple discharge locations with very similar stormwater discharges, the
permittee may petition the Director for Representative Outfall Status (ROS). DWQ may grant Representative
Outfall Status if stormwater discharges from a single outfall are representative of discharges from multiple
outfalls. Approved ROS will reduce the number of outfalls where analytical sampling requirements apply.
If Representative Outfall Status is granted, ALL out
foils are still subject to the g_ualitatiye monitoring
requirements of the facility's permit ---unless otherwise allowed by the permit (such as NC6020000) and DWQ
approval. The approval letter from D WQ must be kept on site with the facility's Storm water Pollution
Prevention Plan. The facility must notify DWQ in writing if any changes affect representative status.

North Carolina General Statute 143-215.6 B(i) provides that:
Any person who knowingly makes any false statement, representation, or certification in any application, record,
report, plan, or other document filed or required to be maintained under this Article or a rule implementing this
Article; or who knowingly makes a false statement of a material fact in a rulemaking proceeding or contested case
under this Article; or who falsifies, tampers with, or knowingly renders inaccurate any recording or monitoring device
or method required to be operated or maintained under this Article or rules of the [Environmental Management]
Commission implementing this Article shall be guilty of a Class 2 misdemeanor which may include a fine not to exceed
ten thousand dollars ($10,000),
I hereby request Representative Outfall Status for my NPDES Permit. I understand that ALL outfalls are still
subject to the qualitative monitoring requirements of the permit, unless otherwise allowed by the permit
and regional office approval_ I must notify DWQ in writing if any changes to the facility or its operations
take place after ROS is granted that may affect this status. If ROS no longer applies, I understand I must
resume monitoring of all outfalls as specified in my NPDES permit.

On November 6, 2013, Sue White of this office performed a Stormwater Permit
Inspection at M&M Motor Company, Inc. located at 9316 US Hwy Bus, 220 N., Randleman, NC
in Randolph County. You were present for the inspection. This facility holds General
Stormwater Permit No. NCG100023 to discharge Stormwater from activities associated with
Used Motor Vehicle Parts under the National Pollutant Discharge Elimination System (NPDES).
1. Permit
This facility holds Certificate of Coverage #NCG 100023 for the discharge of stormwater for the
site. The permit became effective November 1, 2012 and expires October 31, 2017.
2. Records/Reports
Part Il, Section A of the permit requires the development of a Stormwater Pollution Prevention
Plan (SPPP). During the inspection, it was determined that your facility has a developed SPPP
prepared by Mid -Atlantic Associates. All required information was included in this plan.
3. Facility Site Review
This site was clean and well managed. When vehicles are brought t&the site to be salvaged,
fluids, mercury switches and batteries are removed and stored properly until removed from the
site by a licensed hauler. Vehicles are then inventoried and put in the yard for salvage. This
site is well organized and it is obvious that the owners and employees implement permit
requirements. There are three identified Stormwater Discharge Outfall (SDO) associated with
this property.
4. Effluent/Receiving Waters
Stormwater from SDO #1 (NE property corner) and SDO #2 (along fence on south property)
discharge to an un-named tributary to Polecat Creek (Class WS-III) in the Cape Fear River
Basin. SDO #3 (along business 220) flows to an un-named tributary tot eh Deep River (Class
WS-IV), also in the Cape Fear River Basin,
5. Self -Monitoring Program
It was determined during the inspection that you have been performing the self -monitoring
requirements of the General Permit. Monitoring requirements include semi-annual analytical
sampling and qualitative monitoring at each Stormwater discharge outfall on the property. The

In response to your renewal application for continued coverage under stormwater General Permit NCG100000
the Division of Water Quality (DWQ) is forwarding herewith the reissued General Permit. This permit is
reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October
15, 2007 (or as subsequently amended).
The following information is included with your permit package:
• A new Certificate of Coverage (COC)
• A copy of General Permit NCG100000
• A copy of the Technical Bulletin for the General Permit
• Two copies of the Discharge Monitoring Report (DMR) Form
• Two copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater, and it specifies your obligations for discharge
controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself
with all the changes in the reissued permit. Your facility has six (6) months from the time of receipt of the
permit to update your current SPPP to reflect all new permit requirements.
The first sample period of your permit begins January 1, 2013. Your facility must sample a "measureable
storm event" beginning during the periods beginning January 1 and July 1 of every year (or, if applicable,
report "No Flow," as outlined in Part III, Section E). Also, please note that Tier 3 Actions in Part II of your
permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this
permit and do not count prior exceedances.
The more significant changes in the General Permit since your last COC was issued are noted either in the
Draft Permit Fact Sheet that accompanied the public notice (http://portal.ncdenr.org/web/wq/ws/su/current-
notices), or in the Response to Comments / Summary of Changes and Technical Bulletin documents that are
posted on the Stormwater Permitting Unit's website with the new General Permit. Please visit

for your specific General Permit carefully.

Some of the changes include:
• Part II:
• Section A: The Stormwater Pollution Prevention Plan (SPPP) section, if applicable, has been updated
to the most current language of our permits. Additional conditions for specific industry sectors have
been added to the SPPP requirements in some cases.
• Sections B, C: Failure to perform analytical stormwater monitoring may result in the Division requiring
that the permittee begin a monthly sampling scheme.
• Sections B, C: A lower TS5 benchmark of 50 mg/I for HQW, ORW, PNA and Tr Waters applies to these
more sensitive waters.
• Sections B, C. The monitoring parameter Oil & Grease (O&G) has been replaced by the parameter
Total Petroleum Hydrocarbons (TPH) for vehicle maintenance areas, and in some cases, other
analytical monitoring requirements.
• Sections B, C, D: Inability to sample due to adverse weather must be recorded in the SPPP, or in
separate on -site records if your General Permit does not require an SPPP. Adverse weather is defined
in the "Definitions" section of the permit.
• Sections B, C: The term "Representative Storm Event" has been replaced by "Measurable Storm
Event." A measurable storm event is defined in the permit.
• Section D: If the permittee Fails to respond effectively to problems identified by qualitative
monitoring, DWQ may require the permittee to perform corrective action.
Please review Parts III and 1V to understand the Standard Conditions of your new NPDES General Permit,
including Compliance and Liability, Reporting, Monitoring and Records requirements; Operation and
Maintenance obligations; and Definitions. Please note that all samples analyzed in accordance with the terms
of this permit must be submitted to the Division on Discharge Monitoring Report (DMR) forms available on
the Stormwater Permitting Unit's website above. DMR forms must be delivered to the Division no later than
30 days from the date the facility receives the sampling results from the laboratory. Also note that existing
permittees do not need to submit a renewal request prior to expiration unless directed by the Division.
Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit
does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it
relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law,
rule, standard, ordinance, order, judgment, or decree. If you have any questions regarding this permit

determination that stormwater discharge outfall (SDO) 002 be granted representative outfall status for
stormwater outfall 003. Based on the information and maps provided and the site inspection on February
16, 2011, we are approving this request. In accordance with 40 CFR §122.21(g)(7), you are authorized
to sample outfall number 002 as a representative outfall. This approval is effective with the next
sampling event. We also want to remind you that the permit still requires Qualitative Monitoring be
performed at all SDOs, regardless of representative status.
Please remember that any actions you initiate in response to benchmark exceedances as directed in the
tiered response provisions of your permit must address all drainage areas represented by SDO 002,
where appropriate.

On February 26, 2009, Mike Mickey of this office met with you to perform a General
Stormwatef Permit Inspection at the M & M Motor Company facility. The inspection found the
facility to be in compliance with the stormwater permit. This inspection evaluated the five (5) areas
designated on the attached inspection form: -Observations from each area are addressed below;
1. Permit
M & M Motor Company holds Certificate of Coverage # NCG100023 for the discharge of stormwater
from the site. Please note that the above Certificate of Coverage expired 8/31/07, It is our
understanding that you submitted the renewal information to our Raleigh office the week of February
i G`h.
2. Records/Reports
Part I1, Section A of the permit requires the development of a Stormwater Pollution Prevention Plan
(SPPP). A copy of the M & M Motor Company SPPP was reviewed during the inspection. The plan
was found to be thorough and complete.
3. Faciiity Site Review
The site was clean and well managed. The motor oil and antifreeze removed from vehicles is stored
inside a building in totes for pump -out by Noble Oil. Gasoline.is reclaimed using a Gas Buggy. Used
car batteries are picked up by Battery Master.
4. Effluent/Receiving, Wafters
Stormwater from the M & M Motor Company site is discharged from three separate outfalls. Outfall
#'s 001 (NE property corner) and 002 (along fence on south property line) flow to an unnamed
tributary to Polecat Creek, Class "WS-111" Waters in the Cape Fear River Basin. Outfall #003 (along
Business 220) flows to an unnamed tributary to the Deep River, Class "WSAV CA*" waters. All
three stormwater conveyances were free of any obvious contamination.

5. Monitoring Program
Part II, Section B of the permit requires collection of semi-annual analytical samples at all three
outfalls during a representative storm event. A review of facility records revealed that semi-annual
samples were only collected from outfall 001 in 2008. The other two outfalls should also be sampled
per the permit requirements. Please be aware that the permit establishes benchmark values that, if
exceeded, require tiered responses that may involve increased best management practices, inspections,
record keeping and/or sampling. You should familiarize yourself with the two tier levels and the
responses required in the permit if benchmark values are exceeded. The analytical monitoring results
must be submitted to the Division on form SWU-25I-031308 no later than 30 days from the date the
facility receives the sampling data from the laboratory.
Part 1I, Section C of the permit requires that qualitative (visual) monitoring be performed and
documented semi-annually at all three outfalls at the same time that the analytical samples are
collected. The file review found that these observations were performed only once in 2008 on one
outfall..Please make sure that this inoniforing'is performed semi-annually at all three outfalls. The
qualitative records do not need to be submitted to the Division, however, the records must be kept on
file at the facility for a period of five years.

Comment: A copy of the Stormwater Pollution Prevention Plan (SPPP) was available
for review during the inspection. The plan, prepared by Mid -Atlantic Associates, Inc.,
was found to be thorough, complete and up to date, A detailed site map locating the
three outfalls was included, no significant spills were noted over the last three years and
employee training was conducted in December 2008. As fog fluids and materials
handling, waste oil and antifreeze are stored inside a building in —300 gallon totes for
pick-up by Noble Oil Services, used batteries are picked up by Battery Master, and fuel
is reclaimed by using a Gas Buggy.

In accordance -with your application for discharge, permit received on September 13, 1994, we are
forwarding herewith the subject certificate of coverage to discharge under the subject state - NPDES general
permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated
December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division
of Environmental Management. The Division of Environmental Management may require modification or
revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Environmental Management or permits required by the Division of Land Resources, Coastal Area
Management Act or any other Federal or Local governmental permit that may be required.
