Please find enclosed a copy of our Stormwater Discharge Outfall Monitoring
Report for the two outfalls identified in the above captioned permit. The report
covers the annual permitted period of January 1, 2011 — June 30, 2011. Copies
of this report have also been submitted to the Central Files as required in the
permit.
This report is being sent in response to your request that was made during your

Safety rules and policies
Welcome to Akio Nobel Coatings, Inc. High Point facility. The health and safety of our visitors, employees
and neighbors Is an integral part of our business policy. As a visitor to our location, please tamd€ar€ze
yourself with the following rules and procedure$.
All visitors must Sign in before entering the facility and out upon departure Lug books are located in the
lobbies of each building for this purpose_ Visitor badges are located in proxi€Wily to the log books anCf must
be displayed at alE times win le on site. Vehicle tags are also required to be displayed if your vehicle will tie
brought inside the perimeter fencing. Vehicle tags are riot required for the unfenced parking lot areas
Visitors must be accompanied by an AkzoNobel employes during their visit. Your contact person can
answer any Specrf€c questions that you may have auring your stay.
In the unlikely event that an emergency situation occurs during your visit, please review and foiiow these rules
1. For fires, there is an automated alarrn system A steady, loud audio alarm mji Sound In addition, a
flashing strobe light system will also be activated. Alarms will be in all buildings across the site.
2. Upon activation of the tire alarm system, please accompany your contact person to you, designated
mr.ister areas. For Buildings 1, 2. 3, d and 9, the main parking lot on Progress Avenue is the mister point
3. For Building g, the main parking lot on Prospect Avenue. For Building 6, the main parking lot on Bethel
Road. For Building 5, the main parking lot in front of the tank farm is the muster point.

The High Pont facility is a manufacturuig site. Materials are present throughot€t the faC11€ty that have
hazards associated with them. Speedo information rinciud€ng MSDS'sj is available from your AkzoNobel
contact with respect to any materials you may have questions about during your stay If you come into
direct contact with any material during your visit, please notify your contact person unmediately and wash
the material off.
Due to the flammability of marry of the materials used on site, open flame and non -explosion proof
equipment is strictly controlled. Smoking is pennitted in designated areas only. Smoking materials may bra
ht only when you are completely underneath the smoking shelter and a[I rnaiefIals must be eXlIncguished
and disposed of in the provided receptacles prior to exiting the shelter. Coil I cameras, and other
non -explosion proof equipment are prohibited in praduction areas. These items must be left in an office
area or In your vehicle.
Safety glasses with side shields, steel toe shoes, long sleeve shirt and pants. ear plugs, bu€rip caps and
a high visibility vest must be worn in all produCt€cn areas. If you do not have steel toe, shoes you mast
wear shoes which enclose the entire foot and a pair of steel toe overshoes will be provided. All tab areas
require safety glasses with side shields. steel toe shoes. long sleeve shirl and pants. These items or any
additional safety equipment can be provided upon request from your contact person.
The plant Speed limit is 20 mph The use of cell phones or texting while driv€rig is prohibited anywhere on
company property, ux;ludutg parking lots Please be alert and aware of your surroundings and watch for
vehicles {fork trucks, golf earls, cars, trucks. cl i
All stairways are equipped with handrails. Use of handrails is required for all personnel, Moth ascending
