On March 19, 2010, Mike Mickey of this office met with Ricky Craddock to perform a General
StormNAater Permit Inspection at the Republic Services truck maintenance facility in Mocksville, NC The
inspection found the facility to be in compliance with the state issued stormwater pet mit This inspection
evaluated the five (5) areas designated on the attached inspection form Observations from each area are
addressed below
I Permit
Republic Seirvices holds Certificate of Coverage # NCGO80787 for the discharge of stormwater from the site
The permit expires next on October 31, 2012
2 Records/Reports
Part 11, Section A of the permit requires the development of a Stormwater Pollution Pri.vention Plan (SPPP)
A copy of the SPPP, prepared by Golder Associates, Inc , was reviewed during the inspection This plan was
found to be thorough and complete and addressed all items as outlined by the permit The plan was list
updated in December 2008
3 Facility Site Review
A tour of the property found the site to be clean and vGell managed Observations did not reveal any
concerns In regards to the outside storage of liquid materials, the only active container inside the secondary
containment structure was a waste oil tank I here was no release value in the structure It was noted that
truck washing takes place on a concrete pad located in tiont of the service bay entrance The truck wash
water is collected, treated and then discharged into the Town of Mocksville's sewer system
4. EftluentlReceiving Waters
The SPPP site map identified one stormwater outfall for the facility Samples are collected from a storm
drain pipe that discharges outside of the chain link fence Stormwater leaving the property eventually
reaches an unnamed tributary to Leonard Creek, Class ` C" Waters in the Yadkin -Pee Dce River Basin

5 Monitoring Program
Part 11, Section B of the permit requires the collection of semi-annual analytical samples for vehicle
maintenance activities usmg> 55 gallons of new motor oil when averaged over the calendar year Motor oil
usage by the Republic Services facility averaged 149 gallons/month in 2009 The semi-annual sampling was
performed on 3/26/09 and I 1/10/09 All results were below the benchmark values listed in the permit As
you are aware, all analytical monitoring results must be submitted to the Division on form SWU-250-102107
no later than 30 days from the date the facility receives the sampling data from the laboratory
Part II, Section C of the permit contains monitoring requirements for oil water separators and/or petroleum
secondary containment areas Republic Services is not required to perform this monitoring since the
secondary containment structure for the waste oil tank does not discharge to surface waters
Part 11, Section D of the permit requires that qualitative (visual) monitoring be performed and documented
semi-annually at the same time the analytical samples are collected This monitoring was last performed on
5/5109 and 11/10/09 Please make sure that the qualitative monitoring coincides with the dates of the
analytical sampling The qualitative records do not need to be submitted to the Division however, the
records must be kept on file at the facility for a period of five years
