On December 03, 2015 Glen White of the North Carolina Department of Environmental Quality;
Department of Energy, Minerals and Land Resources visited the subject facility located at 770
Trollingwood Hawfield Road in Mebane to conduct a compliance inspection for the facilities
Industrial Stormwater Permit. Mr. Rob Ellers, Driver was present at the time of inspection.
Inspection consist of review of the Stormwater Pollution Prevention Plan, training and
monitoring records and observation of the general condition of the site and outfall.
Permit:
This facility holds General Stormwater Permit NCGO80743 to discharge stormwater from
Vehicle Maintenance Areas. The permit became effective November 01, 2012 and expires on
October 31, 2017. A copy of the permit was available for review at the time of inspection.
Records/Reports:
This facility is required to develop and maintain a Stormwater Pollution Prevention Plan (SPPP)
in accordance with Part Il, Section A of the permit. The facility has developed a SPPP and it is
maintained onsite and updated annually. Training records and monitoring records for 2015 were
missing from the plan. To remain compliant, the owner needs to provide one monitoring for
December 2015 and return to bi-annual monitoring in 2016. Training needs to be conducted and
documented as well. Overall the SPPP is well done and is updated annually.
Facility Site Review:
The facility is used to store and warehouse Frito-Lay products for truck delivery throughout the
area. No fueling or truck washing is allowed at the site under current operating procedures.
Maintenance is provided and new and used motor oil is stored on a maintenance truck and does
not remain onsite. Approximately 50 gallons of motor oil is used per year and catch pans are
used during all maintenance.

Effluent Receiving Waters:
This site has one outfall located at the ditch along Trollingwood Hawfields Road. Stormwater
from the site discharges to an unnamed tributary of Back Creek (Class C; NSW) of the Cape
Fear River Basin.
Self Monitoring:
The stormwater permit requires semi-annual Qualitative Monitoring to be conducted at all
outfalls. Review of the records revealed that the site has conducted all required monitoring in
the'past but records for 2015 were missing. The facility must provide one monitoring event for
December 2015 to remain compliant. Return to bi-annual monitoring in 2016 as permit requires.

Dear Mr. Schmitt:
On December 18, 2012, Jenny Graznak of this office performed a Compliance Evaluation
Inspection at the Burlington Bins division of Rolling Frito Lay Sales, LP located at 770 Trollingwood-
Hawfields Road in Mebane, North Carolina. This facility holds General Stormwater Permit
NCG080743 under the National Pollutant Discharge Elimination System (NPDES) to discharge
Stormwater from Vehicle Maintenance Areas. Alan Gallimore, Mobile Maintenance Mechanic, was
present for the inspection. This type of inspection consists of two basic parts: a review of facility files
and self -monitoring data, and an on -site inspection of the facility and its outfalls. This particular
inspection evaluated five (5) areas as designated on the EPA inspection form, and observations
from each area are addressed below:
1. Permit
The permit became effective November 1, 2012 and expires October 31, 2017. A copy of the permit
was available for review during the inspection.
2. Records/Reports
As required by the Permit, the facility has a complete Stormwater Pollution Prevention Plan (SPPP)
that was available for review during the inspection. All employees are trained on the SPPP and its
contents at an annual meeting. Rolling Frito-Lay conducts an Annual Comprehensive Site
Compliance Evaluation at the site to determine if the SPPP requires any changes.
3. Facili Site Review
This facility is used for storage and warehousing for Rolling Frito-Lay Sales, LP. Mr. Gallimore visits
the site with a mobile maintenance service truck on a weekly basis. New and used motor oil as well
as other vehicle maintenance fluids are stored on Mr. Gallimore's truck, not at the facility. No fueling
or truck washing occurs at the site. Motor oil is changed twice per year for each truck, which results

4. Effluent/Receiving Waters
There is one outfall at the site that discharges to a road -side ditch along Trollingwood-Hawfieids
Road. This stormwater ultimately discharges to an unnamed tributary of Back Creek (Class C;
NSW) in the Cape Fear River Basin.

per the specifications of,the permit. Analytical monitoring is not required at this site since the facility
does not utilize more than 55 gallons of new motor. ail per month. Mr. Gallimore also performs a
quarterly inspection of all areas at the site, and reports from these inspections were available for
review.

Thanks for your help with this. Attached is the original submittal for the renewals of High Point DC (NCG0800736) and
Burlington Bin (NCG080743) from September 2007 signed by Jim Schmitt. Please let me know if this is sufficient to
complete the renewals or if we need to resubmit an original form.
After reviewing the COCs for the remaining facilities it appears the reason these may not have been received at facilities
when sent in 2007 is that the individual listed on the letter is unknown to Frito-Lay and I am not sure how that name came
to be on the renewals. Also the address was for a facility in Georgia where the individual previously responsible for some
of the facilities was located. To ensure the contact information is correct for these facilities and all future correspondence
is sent to the appropriate individual, I am putting together a list of the changes to correct this. How best can we work with
you to get this done?
Last Item - We did not receive a COC for the Kings Mountain facility. It was submitted for permitting in April 2008. 1 have
attached the original NOI.
Again I want to thank you for your assistance in cleaning these up.

I should be able to have these for you by Friday, 3/12/10. I've got preparations for several meetings and trips over the next couple of
weeks, but this is doable for me in the timeframe I've indicated.
I'd like to probe a little on the apparently mis-directed mailings. Our database has three addresses associated with each COC:
a) the address of the facility that is covered by the permit;
b) the address of the person responsible (i.e., the entity identified as the `owner/operator' or permittee) and who is to receive all
correspondence related to the permit coverage;
c) and the billing address.
Many times these three (or maybe just two) are the same address, but not always, as in the case for Frito-Lay. If we have sent the
COCs to the wrong address, we need to correct our database. Do I understand correctly from your list below:
a) Aberdeen through Whiteville: facility address as shown in the second column; responsible person and address are Jim
Schmitt at 2700 Nevada Blvd, Charlotte; billing address is Jim Schmitt at 2700 Nevada Blvd, Charlotte?
b) Andrews through Murphy: facility address as shown in the second column; responsible person and address are Jason
Alexander at 725 Progress Industrial Blvd in Lawrenceville, Georgia; billing address is Bill Davis, same street in Lawrenceville?

North Carolina General Statute 143-215.1 and the Memorandum of Agreement between'North
Carolina and the US Environmental Protection Agency dated October 15, 2007 (or as
subsequently amended).
Please take notice that this certificate of coverage is not transferable except after
